Creating the Healthcare Racial Justice Assessment Tool (HC-RJAT)
Racial and ethnic health inequities are pervasive in the US healthcare system and affect individuals at all stages of life. These inequities are the result of racism and not due to biology. Racism is a multi-level system that embodies institutional, structural, interpersonal, and internalized racism. Racism in the healthcare setting creates hierarchies of power based on the social construct of race that impact opportunity, access, quality, and ex…
The impact of a community health worker intervention on uptake of antenatal care
The provision of high-quality antenatal care (ANC) is important for preventing maternal and newborn mortality and morbidity, but only around half of pregnant women in Tanzania attended four or more ANC visits in 2019. Although there is emerging evidence on the benefit of community health worker (CHW) interventions on ANC uptake, few large-scale pragmatic trials have been conducted. This pragmatic cluster-randomized trial, implemented directly thr…
Burden of HIV-related stigma and associated factors among women living with depression accessing PMTCT services in Dar es Salaam, Tanzania
HIV-related stigma represents a potent risk factor for a range of poor health outcomes, including mental health symptoms, treatment non-adherence, and substance use. Understanding the role of HIV-related stigma in promoting healthcare outcomes is critical for vulnerable populations, such as pregnant women living with HIV, in contexts with continued high rates of HIV and associated stigma, such as sub-Saharan Africa. The current study examined a r…
Prevalence and factors associated with intimate partner violence after HIV status disclosure among pregnant women with depression in Tanzania
Intimate partner violence (IPV) exacts a heavy burden on women, resulting in poor health outcomes. This study had the following aims: (1) estimate the prevalence of IPV post-disclosure of HIV status among pregnant women living with HIV and depression; and (2) evaluate risk and protective factors for IPV post-disclosure. Participants were women accessing PMTCT services at 16 health facilities in Dar es Salaam and screened at the threshold of 9 on …
Measuring experiences of facility-based care for pregnant women and newborns
BACKGROUND: Access to high-quality, person-centred care during pregnancy and childbirth is a global priority. Positive experience of care is key in particular, because it is both a fundamental right and can influence health outcomes and future healthcare utilisation. Despite its importance for accountability and action, systematic guidance on measuring experience of care is limited. METHODS: We conducted a scoping review of published literature t…

An overview of ABoVE airborne campaign data acquisitions and science opportunities
The 2017 Arctic Boreal Vulnerability Experiment Airborne Campaign (AAC) was one of the largest, most complex airborne science experiments conducted by NASA’s Earth Science Division. Between April and November, the AAC involved ten aircraft in more than 200 science flights that surveyed over 4 million km 2 in Alaska and northwestern Canada. Many flights were coordinated with same-day ground-based measurements to link process-level studies with geo…
Disrespectful treatment in primary care in rural Tanzania
Knowing how patients are treated in care is foundational for creating patient-centred, high-quality health systems and identifying areas where policies and practices need to adapt to improve patient care. However, little is known about the prevalence of disrespectful treatment of patients in sub-Saharan Africa outside of maternity care. We used data from a household survey of 2002 women living in rural Tanzania to describe the extent of disrespec…
Missed Opportunities to Improve the Health of Postpartum Women
Beyond the crisis
Resilience was widely identified as a critical attribute for strong health systems following the 2014-15 West Africa Ebola epidemic. In Liberia, Sierra Leone and Guinea, struggles to control the disease and suspension of the operation of many health services demonstrated that health systems must plan for resilience long before a crisis. However, the operational elements of resilience and ways that a crisis experience can shape resilience are not …
Supporting the massive scale-up of antiretroviral therapy
To mitigate the gap between those needing and those receiving ART, more facilities were supported. The proportion of government facilities supported and the median number of persons treated at these facilities increased. This shift could potentially be sustainable as government sites reach more individuals and receive government funding. These results demonstrate that PEPFAR was able to support a massive scale-up of ART services in a short period…
Equitable Distribution of Pepfar-Supported HIV/Aids Services in South Africa
This letter to the editor describes South Africa’s lack of proper health infrastructure which has led to inequalities in access to healthcare. This primarily affects the population’s access to HIV/AIDS and tuberculosis treatments. The South African government and the U.S.’s PEPFAR program have been working to reverse this trend. Recent data indicates that there is an association between the burden of HIV/AIDS and the people tested for HIV/AIDS as…
