The implementation of pharmacogenomics into UK general practice
Pharmacogenomics describes interpatient genetic variability in drug responses. Information based on whole genome sequencing will soon open up the field of pharmacogenomics and facilitate the use of genomic information relating to drug metabolism and drug responses. We undertook a qualitative study, aiming to explore the potential barriers, opportunities and challenges facing the implementation of pharmacogenomics into primary care. Semi-structure…
Health Workforce Governance
Health policy
Providing a critical assessment of developments in health and health-care policy, this book is primarily focused on the UK, covering issues such as the policy-making process, the development of the National Health Service, health-care governance and health promotion
Coronary heart disease and the management of risk
This paper explores developments in the risk management of coronary heart disease (CHD) that have followed the implementation of the Coronary Heart Disease National Service Framework (NSF) guidelines from the patient's perspective. The CHD NSF introduced in 2000 set national threshold targets for blood pressure and cholesterol levels for primary care managed patients, it also laid down guidelines for the clinical management of those patients deem…
Nursing Practice and Organisational Change within the NHS
This methodological paper draws upon the results of a piece of qualitative research which explored nurses perceptions of the ways in which the organisational changes that occurred within the NHS over the past decade have mediated their working practices. This paper seeks to go beyond traditional forms of qualitative analysis in posing the question: Can the views and beliefs of social agents captured in social research inform a realist understandi…
The direction of health policy in New Labour’s third term
The New Labour government would have appeared to have demonstrated a strong commitment to the state health care sector since coming to power in 1997. It has more than doubled spending on the National Health Service, and this investment is projected to rise over the next four years. However, whether this ‘modernization strategy’ will be able to deliver a more equitable health care service in New Labour’s third term in office is open to question. I…
The Politics of Regulation within the ‘Modernized’ NHS
A case study is presented which examines the political and organizational processes associated with the attempt by the Department of Health, through its appraisal body NICE, to regulate the introduction of a new drug treatment (beta interferon) for patients with multiple sclerosis on ‘cost-effectiveness’ grounds. Underpinning this case is a concern to engage critically with the claim that we are witnessing the emergence of a de-regulated contract…
Putting patients first
This article seeks to comment on the extent to which the 'liberal conser vatism' of the New Labour government—liberal as in a celebration of market mechanisms, 'conservative' as in a prescriptive moralism—incor— porates the now less fashionable, but enduringly influential discourse of consumerism in its political agenda for the National Health Service, arguing that this marks a continuity with the politics of the last Conservative government
