The illusion of reality
It is surprisingly difficult to provide an account of hallucination that captures all the cases that clinicians typically class as hallucinations, without at the same time admitting cases that would not typically be classed as such. Most attempts to specify exactly what a hallucination is supposed to be fails to meet at least one of these conditions, as we show in the first part of the paper. We then proceed to develop an account of hallucination…
Results of a study of mentally ill vagrants
This classic text by the German psychiatrist Karl Wilmanns stands out as exceptional in the literature describing the psychopathology of mentally ill homeless people. Wilmanns’ psychopathological descriptions are excellent, as are his observations of the disheartening failure to recognize dementia praecox in courts, prisons and workhouses, and the significant consequences of this failure for the patients. As he vividly shows, most patients manife…
Fundamental issues in epistemic injustice in healthcare
The research field of epistemic justice in healthcare has gained traction in the last decade. However, the importation of Miranda Fricker’s original philosophical framework to medicine raises several interrelated issues that have largely escaped attention. Instead of pushing forward, crafting new concepts or exploring other medical conditions, we suggest that it is time to take stock, reconsider, and articulate some fundamental issues that confro…
Everyday life for patients with schizophrenia and severely impaired social functioning
All the participants had very little structure and routines in their everyday life. The shelters provided the homeless participants with some structuring elements, whereas the domiciled participants had no external structuring elements. The findings have important implications for psychosocial treatment of severe social impairment in schizophrenia. The lack of structure in these patients' everyday lives highlights the need for targeted interventi…

Disturbances of Shared Intentionality in Schizophrenia and Autism
Schizophrenia and autism are today considered complex spectrum disorders characterized by difficulties in social behavior. Drawing on recent advances in collective or shared intentionality studies, we present a novel theoretical approach to these social difficulties by exploring them from the angle of shared intentionality. We begin by describing two forms of shared intentionality: joint intentionality and we-intentionality. Joint intentionality …
Thought insertion and disturbed for-me-ness (minimal selfhood) in schizophrenia
In contemporary consciousness research, we have defended a position of experiential minimalism, arguing that for-me-ness (or minimal selfhood) is a necessary, universal feature of phenomenal consciousness. The concept of for-me-ness refers to the fact that experiences are given first-personally to the subject of experience. To challenge the universality of for-me-ness, several authors have referred to the case of thought insertion as a clear coun…
Mysticism and schizophrenia
Mysticism and schizophrenia are different categories of human existence and experience. Nonetheless, they exhibit important phenomenological affinities, which, however, remain largely unaddressed. In this study, we explore structural analogies between key features of mysticism and major clinical-phenomenological aspects of the schizophrenia spectrum disorders-i.e. attitudes, the nature of experience, and the 'other', mystical or psychotic reality…
