Does pay for performance affect socioeconomic inequalities in access? Evidence from hospital specialised care in England
Understanding the Value of Molecular Testing in Cancer in Aotearoa New Zealand
Molecular testing is fundamental to the modern precision oncology toolkit. However, set on a backdrop of inconsistent use of the term 'value' in healthcare literature, evidentiary limitations in what determines 'high-value' molecular testing have contributed to its variable adoption in the clinic. Key to an improved understanding of value in this setting is eliciting what outcomes (health and non-health) and costs of molecular testing are importa…

Equity in national healthcare economic evaluation guidelines
Equity is given less attention than efficiency in HEE guidelines. This indicates that HTA agencies while subscribing to an extra-welfarist approach have a narrow evaluative space - focusing on maximising health and not considering the opportunity cost of the equity constraint. The omission of equity and the lack of systematic approaches in guidelines poses a threat to the international endeavours to reduce inequities. It is timely for HTA agencie…
A new sexual wellbeing paradigm grounded in capability approach concepts of human flourishing and social justice
Over the last twenty years, there has been a growing interest in measuring sexual wellbeing, including by a WHO/UNFPA working group in 2007, which sought clarity on key dimensions and asked for indicators of these to be devised. However, there remains a lack of conceptual clarity surrounding the concept of sexual wellbeing, which may create variation in what is being assessed and to what we are referring. This paper proposes one way in which to a…
Which countries need Covid-19 vaccines the most? Development of a prioritisation tool
Several factors exist, extending beyond those currently used, which may lead to some countries having a greater need for vaccines compared to others. By assessing relative needs, this scoring tool can build on existing methods to further the role of equity in global COVID-19 vaccine allocation
A Rapid Review of Sexual Wellbeing Definitions and Measures
An increasing number of studies refer to sexual wellbeing and/or seek to measure it, and the term appears across various policy documents, including sexual health frameworks in the UK. We conducted a rapid review to determine how sexual wellbeing has been defined, qualitatively explored and quantitatively measured. Eligible studies selected for inclusion from OVID Medline, PsychInfo, PubMed, Embase, CINAHL were: in English language, published aft…

BACKGROUND: HIV/AIDS has led to increased mortality and morbidity, negatively impacting adult labour especially in HIV/AIDS burdened Sub-Saharan Africa. There has been some exploration of the effects of HIV/AIDS on paid child labour, but little empirical work on children's non-paid child work. This paper provides quantitative evidence of how child and household-level factors affect children's involvement in both domestic and family farm work for …
Condition-specific or generic preference-based measures in oncology? A comparison of the EORTC-8D and the EQ-5D-3L
When comparing a generic and condition-specific preference-based instrument, divergences are apparent in both baseline health state values and in the estimated QALYs over time for cancer patients. The variability in sensitivity between the baseline values and the QALY estimations means researchers and decision makers are advised to be cautious if using the instruments interchangeably
Burden attributable to Cardiometabolic Diseases in Zimbabwe
The findings of this study indicate a growing prevalence of CMDs and related mortality in Zimbabwe. Health policy decisions and cost-effective preventive strategies to reduce the burden of CMDs are urgently required
Mapping the EORTC QLQ-C30 onto the EQ-5D-3L
Operationalising the capability approach as an outcome measure in public health
Anchoring vignettes for health comparisons
Does social support in addition to ART make a difference? Comparison of households with Taso and MOH PLWHA in Central Uganda
Social support in addition to antiretroviral therapy (ART) has been indicated to be beneficial to person living with HIV/AIDS (PLWHA) and their families, but very few ART service providers go beyond ART. This study investigates whether receipt of social support in addition to ART for PLWHA makes the households that they reside in better off than households that have PLWHA but are without social support. The analysis uses data comprising of 450 ho…

Population ageing in Scotland
Objective It is important for health policy and expenditure projections to fully understand the relationship between age, death and expenditure for health care (HC). Evidence shows that older age groups incur less acute HC costs than previously anticipated and that time to death (TTD) may be a much stronger indicator for HC expenditure than age alone. Our objective was to undertake research into the relationship between TTD, age and HC expenditur…
Concepts of Capability and Overlooked Applications
We read with interest the recent paper by Ruger on conceptualizing and operationalizing health capability. She stated that ‘‘No work to date has developed a conceptual model or health capability profile leading to methods for addressing health capability deprivations,’’ and then goes on to develop such a model and profile. We agree that literature in this area is limited but draw readers’ attention to a number of articles from the health economic…
