Provision of safer smoking equipment to reduce health harms and enhance service engagement among people who use crack
BACKGROUND: Safer smoking equipment interventions for people who smoke crack cocaine have been unevenly implemented globally with most harm reduction policy and practice focused on preventing opioid and injecting related harms. There is a need to synthesise the international evidence on safer inhalation equipment provision for crack and understand how, why, for whom, and in what context can such interventions improve engagement with services, pro…
Crack-cocaine use practices, harms and respiratory problems
The use of stimulants, such as crack-cocaine, is a global public health concern. Crack-cocaine use is increasing in the UK, but available data is focused on those who inject or also use opioids. To address this gap, characteristics of people using crack-cocaine in England, including respiratory problems among those who smoke, and variation in these by gender are described. Adults self-reporting crack-cocaine use in the past 28 days, recruited by …

Building and Boosting Capitals for Health Care Access
Peer support is widely promoted as enabling health care access with people experiencing homelessness, including through enabling independence, empowering individuals and managing stigma. However, there is little understanding of these processes, including how they relate to the wide range of potential activities involved in peer support. We studied peer support within a peer advocacy service at a charity run by and for people experiencing homeles…
Why were Covid-19 infections lower than expected amongst people who are homeless in London, UK in 2020? Exploring community perspectives and the multiple pathways of health inequalities in pandemics
High rates of COVID-19 infections and deaths amongst people who are homeless in London, UK were feared. Rates however stayed much lower than expected throughout 2020; an experience that compares to other settings globally. This study sought a community level perspective to explore this rate of infections, and through this explore relationships between COVID-19 and existing health inequalities. Analyses are reported from ongoing qualitative studie…
Policing and public health interventions into sex workers’ lives
While extensive literature documents how criminalisation harms sex workers’ health and rights, limited research has critically examined how interactions between criminal-justice, health, and other systems shape support and justice for and by people who sell sex. We attend to this question by drawing on participatory, qualitative research with a diverse group of sex workers and other stakeholders in East London, UK. In addition to directly and str…
The Effect of Systemic Racism and Homophobia on Police Enforcement and Sexual and Emotional Violence among Sex Workers in East London
There is extensive qualitative evidence of violence and enforcement impacting sex workers who are ethnically or racially minoritized, and gender or sexual minority sex workers, but there is little quantitative evidence. Baseline and follow-up data were collected among 288 sex workers of diverse genders (cis/transgender women and men and non-binary people) in London (2018-2019). Interviewer-administered and self-completed questionnaires included r…
Prevention of Covid-19 among populations experiencing multiple social exclusions
Ten insulin dependent diabetic patients were maintained in normoglycaemia for 36 h with an artificial pancreas. Circulating polymorphonuclear leucocytes were extracted and incubated with staphylococcus before and after treatment; phagocytic and bactericidal activities were studied. Granulocytes from diabetic patients showed a decreased ability to absorb staphylococcus and a reduced capacity for intracellular destruction of bacteria. These two fun…

What determines violence among female sex workers in an intimate partner relationship? Findings from North Karnataka, south India
Clinical Trials NCT02807259
Associations between sex work laws and sex workers’ health
Incarceration history and risk of HIV and hepatitis C virus acquisition among people who inject drugs
Needle and syringe programmes and opioid substitution therapy for preventing HCV transmission among people who inject drugs
Differences in risk behaviours and HIV status between primary amphetamines and opioid injectors in Estonia and Russia
Fentanyl self-testing outside supervised injection settings to prevent opioid overdose
Since 2013, North America has experienced a sharp increase in unintentional fatal overdoses: fentanyl, and its analogues, are believed to be primarily responsible. Currently, the most practical means for people who use drugs (PWUD) to avoid or mitigate risk of fentanyl-related overdose is to use drugs in the presence of someone who is in possession of, and experienced using, naloxone. Self-test strips which detect fentanyl, and some of its analog…
Large-scale implementation of alcohol brief interventions in new settings in Scotland
This qualitative study suggests that even with significant national support, funding and a specific delivery target, ABI implementation in new settings is not straightforward. Those responsible for planning similar initiatives should critically consider the relevance and value of the five implementation strategies identified

HIV infection and risk behaviour of primary fentanyl and amphetamine injectors in Tallinn, Estonia
Knowledge of HIV serostatus and risk behaviour among injecting drug users in Estonia
We used the findings from two, cross-sectional studies of HIV serostatus and risk behaviours to assess the effects of knowledge of HIV serostatus and risk behaviours (relating to sex and injection drug use) among injecting drug users (IDUs). Respondent-driven sampling was used simultaneously at two sites in Estonia (the capital Tallinn, and the second-largest city of Ida-Virumaa County, Kohtla-Jarve). The research tool was an interviewer-administ…
Police violence and sexual risk among female and transvestite sex workers in Serbia
OBJECTIVE: To explore female and transvestite sex workers' perceptions of risk in the sex work environment in Serbia. DESIGN: Qualitative interview study. SETTING: Street based locations for sex work in Belgrade and Pancevo, Serbia. PARTICIPANTS: 31 female and transvestite sex workers. RESULTS: Violence, including police violence, was reported as a primary concern in relation to risk. Violence was linked to unprotected sex and the reduced capacit…

Effects of Sex Work on the Prevalence of Syphilis Among Injection Drug Users in 3 Russian Cities
Objectives. We examined risk factors for syphilis infection among injection drug users in 3 Russian Federation cities, focusing particular attention on the potential roles of gender and sex work. Methods. We conducted a cross-sectional survey of injection drug users in Moscow, Volgograd, and Barnaul, collecting behavioral data and testing for antibodies to Treponema pallidum. Associations between presence of antibodies to T pallidum and covariate…
Methods to Recruit Hard-to-Reach Groups
Evidence suggests rapid diffusion of injecting drug use and associated outbreaks of HIV among injecting drug users (IDUs) in the Russian Federation and Eastern Europe. There remains a need for research among non-treatment and community-recruited samples of IDUs to better estimate the dynamics of HIV transmission and to improve treatment and health services access. We compare two sampling methodologies "respondent-driven sampling" (RDS) and chain …
Exploring Barriers to ‘Respondent Driven Sampling’ in Sex Worker and Drug-Injecting Sex Worker Populations in Eastern Europe
Respondent driven sampling (RDS) has been used in several counties to sample injecting drug users, sex workers (SWs) and men who have sex with men and as a means of collecting behavioural and biological health data. We report on the use of RDS in three separate studies conducted among SWs between 2004 and 2005 in the Russian Federation, Serbia, and Montenegro. Findings suggest that there are limitations associated with the use of RDS in SW popula…
