Early appraisal of China's huge and complex health-care reforms
Equity in Health Care Financing: Evaluation of the Current Urban Employee Health Insurance Reform in China
This paper uses multi-year, individual-level data to evaluate the effects of the current Chinese urban employee health insurance reform on equity in health care financing. It found that the new Urban Employee Basic Health Insurance Scheme financed by personal medical savings accounts (MSAs) plus a social-risk pooling account (SPA) is regressive. The contributions to the SPA, although slightly regressive, played an important role in equalizing hea…

Standardized alcohol education: A hit or miss affair?
A study was undertaken to evaluate the effects of akohol education on patients drinking to excess. Four hundred and twenty-eight patients admitted to nine medical and orthopaedic wards of a District General Hospital were identified as drinking at unsafe levels. Patients were allocated to an alcohol-education group or a no-intervention group; all patients received routine medical care. Three hundred and sixty-one patients were re-interviewed a yea…
Developing the Health Care Market
The structure of health care services varies enormously from country to country and the responses of policy makers to these difficulties exhibit similar characteristics. There are a number of features of health care systems which make monitoring outcomes and policy formulation especially problematic poor data on outcomes and perverse incentives facing agents are good examples. In the UK, resource allocation has been effected within the National H…

Economic aspects of addiction policy
One definition of policy or government action in the Oxford English Dictionary is "craftiness" i.e. cunning or deceit. Such qualities have to be employed by governments because of the potential vote-losing effects of radical addiction policies. Health promotion, in relation to addictive substances such as alcohol and tobacco in particular, involves a trade-off between the costs of such policies, especially to industry (which seeks regulation to p…

Dental Manpower
A recent report by the Department of Health and Society Security (DHSS) has argued that a 10% reduction in the intake to dental schools is needed if an oversupply of dentists over the next forty years is to be avoided. Whilst welcoming this recommendation it is likely that it may not go far enough. In this article we describe the approach used in the DHSS report to estimate the demand for and supply of dental care and examine the relevance and im…

Intergenerational Continuities in Housing
This paper uses data from the Rowntree Follow-up Survey to provide new empirical evidence on the association between the housing status of parents and children. The paper concludes that there appear to be some significant continuities across successive family generations in tenure, density and housing expenditure. The results are relevant to assessments of the 'fairness' of the distribution of housing and raise interesting questions for future wo…
Inequlity, the National Health Service and Health Policy
The National Health Service (NHS) was founded in 1948 to replace an inefficient, ill-coordinated and financially unstable health care system and to reduce the barriers to the consumption of health care. In this article, it is argued that the geographic and social class equity objectives of the NHS have been poorly specified and that attempts to achieve them through government policy have been inadequate. The renewed interest in inequalities in he…

Budget Allocation in the National Health Service
The objective of this article is to analyse the way in which budget allocation decisions have been made in the National Health Service (NHS) of Britain. Despite the criticisms of the pre-1948 health care system on the grounds of geographical inequity, it was not until nearly twenty-five years after the service's inception that explicit allocation formulae were devised and implemented in an attempt to achieve equity, defined as equal financial res…
Pricing, Insurance and the National Health Service
Since the birth of the National Health Service in 1948 there have been periodic discussions of the potential role of pricing and insurance in the United Kingdom health care system. This article is concerned with discussing the problems inherent in these mechanisms and it advocates more careful articulation of the cost and benefits of such policies. The first section gives a description of some quite recent proposals to extend the role of the pric…

The Objectives and Performance of the Mental Health Services in England and Wales in the 1960s
The objective of this paper is to take the published Mental Health Service objectives of the British Government and to assess the extent to which these objectives had been attained by 1970. Our measures are those of input and the criteria by which we judge the services are those which were set forth in the White papers, the Hospital Plan for England and Wales (1962), and Health and Welfare (1963). By and large we find that the objectives were not…

The Costs of Dangerous Drugs Legislation in England and Wales
The purpose of this report is to attempt to estimate the hidden costs of Dangerous Drugs legislation. We attempted to quantify the amount of time consumed by prescribing, obtaining, and using dangerous drugs and non-dangerous drugs. The difference between these two series of measurement gives an estimate of the time cost of using dangerous rather than non-dangerous drugs. This time cost was translated into money terms by the use of current nursin…
