Assessing the contribution of the dental care delivery system to oral health care disparities
Objectives Existing studies of disparities in access to oral health care for underserved populations often focus on supply measures such as number of dentists. This approach overlooks the importance of other aspects of the dental care delivery system, such as personal and practice characteristics of dentists, that determine the capacity to provide care. This study aims to assess the role of such characteristics in access to care of underserved po…

Reflections in a glass door: Memory and melancholy in the personal writings of Natsume Sōseki
Reflections in a Glass Door: Memory and Melancholy in the Personal Writings of Natsume Soseki
Much has been written about Natsume Soseki (1867-1916), one of Japan's most celebrated writers. Known primarily for his novels, he also published a large and diverse body of short personal writings (shohin) that have long lived in the shadow of his fictional works. The essays, which appeared in the Asahi shinbun between 1907 and 1915, comprise a fascinating autobiographical mosaic, while capturing the spirit of the Meiji era and the birth of mode…
Characteristics of Dentists Providing Dental Care to Publicly Insured Patients
Dentists who care for publicly insured patients appear to have practices that are different from those who do not, in terms of delivery of patient care and practice structure. Such differences have implications for the access to and quality of dental health care of publicly insured patients. The success of public programs and policies aimed at ensuring access to dental care depends on ability and willingness of dentists to accommodate public pati…
A Longitudinal Analysis of Unmet Need for Oral Treatment in a National Sample of Medical HIV Patients
This longitudinal study examines perceived unmet dental need in a nationally representative probability sample of HIV-infected persons in medical care. A logistic regression analysis modeled the relationship between unmet need and explanatory variables. We estimate that 40% of HIV/AIDS patients report an unmet need associated with being male, being unemployed, injecting drugs, being heterosexual, lacking dental insurance, and having less educatio…
The Impact of HIV on Oral Health and Subsequent Use of Dental Services
Persons with more HIV-related symptoms and a diagnosis of AIDS have a greater need for dental care than those with fewer symptoms and without AIDS, but more pressing needs for physical and mental health services limit their access to dental services. Providers should better attend to the oral health needs of persons with HIV who are in poor physical and mental health
A Comparison of Unmet Needs for Dental and Medical Care Among Persons with HIV Infection Receiving Care in the United States
To serve both the dental and medical needs of diverse populations affected by HIV disease, greater coordination of services is needed. In addition, state insurance programs for people with HIV should consider the feasibility of expanding their benefit structure to include dental care benefits
Managed Care in Dental Markets: Is the Experience of Medicine Relevant
Despite the observed relationship between medical and dental HMO penetration rates, there are potential barriers to managed care in the case of dentistry that may explain the slower growth to date, and that may ultimately decide the extent of managed care penetration into the dental market
The Care of HIV-Infected Adults in the United States
BACKGROUND AND METHODS: In order to elucidate the medical care of patients with human immunodeficiency virus (HIV) infection in the United States, we randomly sampled HIV-infected adults receiving medical care in the contiguous United States at a facility other than military, prison, or emergency department facility during the first two months of 1996. We interviewed 76 percent of 4042 patients selected from among the patients receiving care from…
Comparison of Access and Costs of Medicaid Dental Services in a Hospital Clinic and Community Practices
To understand the mix of utilization rates, visits, and costs, one must take into account the way in which the HMO hospital plan is reimbursed, the way in which the dental department is reimbursed, and the way in which the dental provider is reimbursed

Ogai's (1862-1922) stature among modern Japanese writers is unparalleled, but until recently his work in translation has languished in scholarly monographs and journals. Japan scholar Rimer has gathered several of Ogai's best-known stories and the first complete translation of a major work, Seinen (
Paragons of the Ordinary: The Biographical Literature of Mori Ogai
Paragons of the Ordinary is about a quite extraordinary literary achievement: a series of biographies of obscure scholar-literati written by Mori Ogai, one of Japan's most prominent writers and intellectuals. Deeply concerned about the cultural toll taken by Japan's headlong modernization early in this century, Ogai employed the format of newspaper serialization in presenting meticulously researched accounts of individuals who had come to embody …

The Impact of Western Autobiography on the Meiji Literary Scene
Japan's confrontation with Western culture during the Meiji period (1868-1912) involved a quest for literary styles and techniques that would serve as models for a new generation of writers. What dominated this long-term experiment in literary modernization was an abiding interest in selfhood and self-representation. This paper explores the role of Western autobiography in the development of this literature

A Proposed New System for Valuing Dental Procedures: The Relative Time-Cost Unit
The relative time cost unit (RTCU) is a proposed new system for valuing dental procedures that provides an alternative to traditional relative value units in fee-setting and reimbursement allowances. It incorporates personnel costs, task mixes, and task times into relative weights for dental procedures. The frequently performed procedure, "2-surface amalgam restoration," is used to illustrate how the RTCU values are derived from hospital task ana…
Mori Ōgai and Biography as a Literary Genre in Japan
This paper traces landmarks in the evolution of biographical writing in Japan, focusing on the pivotal role of Mori Ōgai (1862-1922) and his pioneering trilogy of biographies, in particular the first of these, Shibue Chūsai (1916), the biography of a physician and Confucian scholar of nineteenth-century Japan

Did: A Direct Measure of Delegation
The DID measures the degree of delegation existing in a dental practice at a given time. Its four elements are: 1) the delegable tasks, 2) the personnel involved, 3) the task frequency, and 4) the task time. The formula yields the ratio of the Maximum State of Delegation to the Actual State of Delegation or, in other words, the theoretical potential for delegation divided by the observed state of actual delegation. Both are expressed in cost-rela…

Differences in Social Perceptions Between Male and Female First‐year Dental Students
Male and female first-year dental students generally possess the same attitudes and the same perceptions about themselves, dentists they observe, and about their ideals for dentists and patients. The null-hypothesis, that is, assuming no difference between the two groups on the basis of these data, is accepted. In general, one must assume that those women who choose dentistry as a career are not significantly different from their men counterparts…
