The Effects of Medicaid Expansion Under the Affordable Care Act on Health Insurance Coverage, Health Care Access, and Health Care Use for People with Disabilities
More than 27% of individuals in the United States experience a disability. We conducted a scoping review of the literature to examine what is known about the impact of the 2010 Affordable Care Act Medicaid expansion on individuals with disabilities’ health care insurance coverage, health care access, and health care use. We followed the approach of Arskey and O’Malley in conducting our review. Electronic journal databases, hand searching of key h…
Relationships Among Race/Ethnicity, Gender, and Mental Health Status in Patient–Provider Interactions
Patient–provider interactions are associated with improved health outcomes and are of particular importance to individuals with mental health disabilities. We examine the relation between having a severe mental health–related disability (SMD) and patient–provider interactions and whether this is moderated by patient race/ethnicity and gender. We pooled Medical Expenditure Panel Survey data over multiple years and used multivariate techniques to e…
The Relation Between Health Insurance and Health Care Disparities Among Adults With Disabilities
Objectives. We examined disparities among US adults with disabilities and the degree to which health insurance attenuates disparities by race, ethnicity, and socioeconomic status (SES). Methods. We pooled data from the 2001–2007 Medical Expenditure Panel Survey on individuals with disabilities aged 18 to 64 years. We modeled measures of access and use as functions of predisposing, enabling, need, and contextual factors. We then included health in…

The Future of Public–Private Partnership in Child Welfare
The child welfare system operates as a joint endeavor of the public and private sectors, while in each state the roles of the sectors and the functioning of their relationship varies across a continuum. The system and the sectors operating within it face both facilitators and challenges as they face a future of serving children and families. In this article, leaders from national organizations with differing constituencies who operate as stakehol…
Relations Among Home- and Community-Based Services Investment and Nursing Home Rates of Use for Working-Age and Older Adults
Objectives. I examined state-level rates of nursing home use for the period from 2000 to 2007. Methods. I used multivariate fixed-effects models to examine associations between state sociodemographic, economic, supply, and programmatic characteristics and rates of use. Results. Nursing home use declined among older adults (aged ≥65 years) in more than two thirds of states and the District of Columbia but increased among older working-age adults (…
Developing Personal Care Programs
This paper examines the development of programs delivering personal care to the elderly and disabled. First, we report the latest national participant and expenditure trend data for the three main personal care programs: the Medicaid Personal Care Services (PCS) benefit, Medicaid 1915(c) waivers, and the Older Americans Act Title III. Second, to examine interstate variation revealed in the trend analysis, we present three time-series regression m…
African Americans’ Decisions Not to Evacuate New Orleans Before Hurricane Katrina
Objectives. We examined the psychosocial and personal factors that influenced African Americans’ decision not to evacuate New Orleans, La, before Hurricane Katrina’s landfall. Methods. We conducted 6 focus groups with 53 African Americans from New Orleans who were evacuated to Columbia, SC, within 2 months of Hurricane Katrina. Results. The major themes identified related to participants’ decision to not evacuate were as follows: (1) perceived su…
Participation and knowledge related to a nursing home admission decision among a working age population
Access to Community-Based Long-Term Care
Recent legal rulings, combined with the demonstrated preferences of most individuals to receive care in the community, require policies to foster the expansion of Medicaid community-based care. The most consistent relationships that are amenable to policy intervention relate to state fiscal resources and long-term care supply regulation
