Decision Support Indicators (DSIs) and their role in hydrological planning
Decision Support Indicators (DSIs) are metrics designed to inform local and regional stakeholders about the characteristics of a predicted (or ongoing) event to facilitate decision-making. In this paper, the DSI concept was developed to clarify the different aims of different kinds of indicators by naming them, and a framework was developed to describe and support the usage of such DSIs. The framework includes three kinds of DSI: hydroclimatic DS…
Insights from obstetric providers and emergency medical technicians on determinants of maternal morbidity and mortality among underserved, rural patients in the United States
Over the past two decades, individuals in the United States (US) have faced a steadily increasing risk of experiencing severe morbidities attributable to pregnancy and dying from pregnancy-related causes. The prevalence of severe maternal morbidity (SMM), defined as the occurrence of end-organ damage in the mother encompassing unexpected outcomes of labor and delivery, has nearly tripled since the turn of the 21st century (Creanga et al., 2014; N…
The museum as a social space and a place for lifelong learning
This article explores the social dimensions of the museum by focusing on two questions: 1) What elements in museums have proved successful in creating socially interactive environments? 2) Does social interaction stimulate learning in museums and if so: how? We argue that museums are fundamentally social spaces, which are often appreciated and used as spaces to be social, but also that there is a lack of research and knowledge on how museums act …
Rural residency as a risk factor for severe maternal morbidity
PURPOSE: The goal of this study was to evaluate how rural/urban status and other risk factors alter women's odds of severe maternal morbidity (SMM) at delivery. METHODS: This study used 48,608 Kentucky resident delivery hospitalization records from 2017. We used multiple logistic regression with interaction terms to evaluate the moderating effect of rural/urban residence with other risk factors. We reported adjusted odds ratios (aORs) and 95% con…
Sources of Stress and Strength
In the rural United States (US), high rates of substance abuse have precipitated the need for many older adults to assume the role of primary caregiver for their grandchildren. This situation is particularly common in rural Appalachia, a region known for both sparse resources and strong kinship ties. Drawing on in-depth interviews with twenty-one grandparent caregivers in rural Kentucky, we explore the fraught relationships these caregiving grand…
Preterm Birth Prevention in Appalachia
Purpose: This perspective piece reflects off previously published qualitative work to explore (1) themes surrounding equitable prenatal care in Appalachia and (2) strategies to restructure care delivery in a population with disparate rates of preterm birth (PTB). Methods: This study reflects in-depth interviews with 22 Appalachian women who experienced PTB and 14 obstetric providers. Results: Our findings underscore the need for greater cultural …
Pregnancy‐Related Mortality and Severe Maternal Morbidity in Rural Appalachia
PURPOSE: The Centers for Disease Control and Prevention (CDC) and the American College of Obstetrics and Gynecology have called for researchers to further elucidate medical and social determinants of pregnancy-related death and severe maternal morbidity. This report begins to answer this call in the context of rural Appalachia. METHODS: This report identifies risk factors exposing women in rural Appalachia to pregnancy-related death and severe ma…
Fosterage and dependency in medieval Iceland and its significance in Gísla Saga
Saga characters often exercised the option of fostering their children. This meant that a portion of the responsibilities of parenthood were delegated to people who were not the child's biological parents. This chapter uses Gragas as a guide, to separate, delineate and re-label two parenting practices which are regulated closely by the law: legal fosterage and guardianship. It compares them, in order to show that an understanding of the nuances o…
