Menstrual cycle phase and single tablet antiretroviral medication adherence in women with HIV
Suboptimal adherence to antiretroviral (ARV) therapy among HIV-infected individuals is associated with increased risk of progression to AIDS and the development of HIV resistance to ARV medications. To examine whether the luteal phase of the menstrual cycle is independently associated with suboptimal adherence to single tablet regimen (STR) ARV medication, data were analyzed from a multicenter cohort study of HIV-infected women who reported regul…

Association of Cigarette Smoking With HIV Prognosis Among Women in the Haart Era
Objective. We assessed the association of cigarette smoking with the effectiveness of highly active antiretroviral therapy (HAART) among low-income women. Methods. Data were analyzed from the Women’s Interagency HIV Study, a multisite longitudinal study up to 7.9 years for 924 women representing 72% of all women who initiated HAART between July 1, 1995, and September 30, 2003. Results. When Cox’s regression was used after control for age, race, h…

Changes in Sexual Behavior Among HIV-Infected Women After Initiation of Haart
Objectives. We assessed the association between initiation of highly active antiretroviral treatment (HAART) regimens and sexual risk behaviors among HIVinfected women. Methods. We analyzed data from 724 women who initiated HAART between January 1996 and January 2001 and who had pre-HAART viral loads at or above 400 copies per milliliter. Results. Sexually active women were less likely (odds ratio [OR] = 0.79) to report 2 or more partners during …
Joyce Et Al. Respond
We appreciate the comments by Seils et al. As we stated in our article, we agree that relatively few foreign-born Latinas lost eligibility for publicly financed prenatal care under the Personal Responsibility and Work Opportunity Reconciliation Act (PRWORA). However, there was substantial fear and confusion in immigrant communities as to the ramifications of PRWORA. Governor Wilson of California was explicit about his desire to cut off public fun…
Welfare Reform and the Perinatal Health and Health Care Use of Latino Women in California, New York City, and Texas
Objectives. This study analyzed changes in the financing of prenatal care and delivery, the use of prenatal care, and birth outcomes among foreign-born vs US-born Latino women following enactment of the Personal Responsibility and Work Opportunity Reconciliation Act (PRWORA) in August 1996. Methods. We used a pre–post design with a comparison group. The sample consisted of resident Latinas in California, New York City, and Texas who delivered a l…

An association between the heat-humidity index and preterm labor and delivery
OBJECTIVES: The goal of this study was to determine whether a relationship exists between heat-humidity indexes and rates of preterm labor and preterm delivery. METHODS: Preterm labor and delivery rates were compared during the 2 summer and 2 winter weeks with the highest and lowest heat-humidity indexes for each season. RESULTS: The rate of preterm labor increased consistently from 1.23% to 3.00% as the heat-humidity index rose. When preterm bir…

Obstetrical Factors and the Transmission of Human Immunodeficiency Virus Type 1 from Mother to Child
Background A substantial proportion of perinatally acquired infections with the human immunodeficiency virus type 1 (HIV-1) occur at or near delivery, which suggests that obstetrical factors may have an important influence on transmission. We evaluated the relation of such factors and other variables to the perinatal transmission of HIV-1. Methods The Women and Infants Transmission Study is a prospective, observational study of HIV-1–infected wom…
Changes in perinatal cocaine use in an inner-city hospital, 1988 to 1992
Temporal trends in perinatal drug use among parturients at an inner-city hospital were assessed in a cohort study of 1300 parturients in 1991 through 1992 and 1111 parturients in 1988 through 1989. Toxicology results were coupled to data sheets containing demographic and obstetrical information. A decrease was noted between 1988 and 1992 in the prevalence of cocaine metabolites, independent of the utilization of prenatal services. An increase in …
A cohort study of the impact of perinatal drug use on prematurity in an inner-city population
Perinatal illicit drug use was determined by anonymously testing the urine of 1111 inner-city parturients. Fourteen percent tested positive, 11.5% for cocaine. As determined by self-report or by positive test results, 17% used drugs. Twenty-eight percent of low birthweights and 20% of premature births in this group were attributable to drug use. Risk of prematurity was reduced by a factor of 2 to 3 with adequate prenatal care. These data suggest …
