Availability of Medical Care Services in Drug Treatment Clinics Associated With Lower Repeated Emergency Department Use
BACKGROUND: Drug users rely heavily on emergency departments (EDs) for care. Medical and other services in outpatient drug treatment clinics may reduce demand for ED care. OBJECTIVE: The objective of this study was to examine the association of services in drug treatment clinics with repeated ED use by clinic patients. DESIGN: This study consisted of telephone interviews of directors of a stratified random sample of 125 New York state outpatient …
Relationship of gender, depression, and health care delivery with antiretroviral adherence in HIV-infected drug users
Outpatient Patterns of Care and Longitudinal Intensity of Antiretroviral Therapy for HIV-Infected Drug Users
OBJECTIVE: To examine the association of drug users' outpatient patterns of care with subsequent intensity of antiretroviral therapy (ART). MATERIALS AND METHODS: Annual types of ART in 8897 New York State Medicaid drug users who were prescribed ART for > or =6 months in 1996 or 1997 were determined. From pharmacy claims, intensity was classified from changes in annual type of ART in 1996 to 97 and 1997 to 98 as: optimal (ie, on or starting highl…
Antiretroviral Use and Pharmacy-Based Measurement of Adherence in Postpartum HIV-Infected Women
BACKGROUND: Antiretroviral treatment for HIV-infected women is standard during pregnancy to prevent vertical transmission, but data on postpartum therapy for the mother are lacking. OBJECTIVE: The objective of this study was to examine the impact of provider and patient characteristics on receipt of antiretroviral therapy and pharmacy-based measurement of adherence by postpartum HIV-infected women. RESEARCH DESIGN: This was a retrospective cohort…
Effect of Enhanced Prenatal and HIV-Focused Services for Pregnant Women Who Are Infected by Human Immunodeficiency Virus on Emergency Department Use
OBJECTIVES: This study examines whether the receipt of enhanced prenatal or human immunodeficiency virus (HIV) medical services is associated with in-pregnancy emergency department (ED) utilization by HIV-infected women. METHODS: Medicaid and vital statistics records were linked for 1,826 women who are infected by HIV and who were delivered from 1993 to 1995 while receiving New York State Medicaid. The authors examined two types of ambulatory car…

Prevention of vertical HIV transmission with zidovudine
We sought to estimate the impact of maternal HIV testing and prenatal care on the potential to reduce vertical transmission through zidovudine (AZT) use by HIV-infected mothers. We evaluated the prepartum maternal HIV diagnosis rate, prenatal care, disease stage, and vertical transmission rate (from a two-part mixture model) using New York State Medicaid and vital statistics data for HIV-infected mothers and their singletons in 1985-90. We used p…

Are bisexually identified men in San Francisco a common vector for spreading HIV infection to women
OBJECTIVES. This article examines sexual risk taking among self-identified bisexual men in San Francisco and whether risk reduction has occurred, with respect to both homosexual and heterosexual behaviors, among human immunodeficiency virus (HIV) antibody-positive and HIV antibody-negative men. It also examines psychosocial correlates of unprotected anal and vaginal intercourse. METHODS. The participants were members of a population-based longitu…
Smokeless tobacco, cardiovascular risk factors, and nicotine and cotinine levels in professional baseball players
BACKGROUND. The use of smokeless tobacco (ST), which has increased in popularity over the past 2 decades, results in considerable systemic exposure to nicotine. Nicotine might contribute to atherosclerosis by an effect on cardiovascular risk factors. METHODS. The effects of ST use on cardiovascular risk factors and cotinine and nicotine levels were studied in 1061 professional baseball players during spring training in 1988 and 1989. RESULTS. Of …
Characteristics of cancer patient referrals to home care
Records of 609 cancer patients referred in 1979 to home health agencies in rural and urban health service regions in Illinois were abstracted. There was no difference in the overall utilization of home care between the two regions, but significant differences in referral characteristics were found between the two regions with respect to patient's disability level, disease stage, presence of concurrent medical conditions, and living arrangement, a…
Transfemoral catheterization of the cavernous sinus outflow with comparison of “central” and “peripheral” gonadotropin patterns in bonnet monkeys
Concentrations of follicle stimulating hormone (FSH) and luteinizing hormone (LH) in central (C) samples obtained by transfemorally catheterizing the inferior petrosal sinus of female bonnet monkeys were compared with those in peripheral (P) samples obtained simultaneously from the saphenous veins of two intact and two oophorectomized bonnet monkeys, before, during, and after luteinizing hormone releasing hormone (LHRH) stimulation. Significant d…
