Trends in spousal age difference at first marriage in Sub-Saharan Africa
Spousal age differences are highest in Sub-Saharan Africa, where trends in age at first marriage indicate an increase for both men and women. However, the net outcome for spousal age difference is difficult to predict without explicit analysis of these distributions. This study examines differentials in spousal age for women in first union. Further, it examines differences within population sub-groups and across countries, focusing on the influen…
Who pays more? Exploring cost disparities in medication abortion access across socio-demographic groups in Ghana
MA cost in Ghana is largely based on providers' discretion and at the facility's management level. Additionally, the cost differs by women's socio-demographic attributes. To reduce the disparities in MA costs, developing guidelines to address the health system challenges regarding MA provision and access is important. Educational programs on MA access, provision and legal framework could also reduce abortion-related stigma and cost variations
Identifying deprived “slum” neighbourhoods in the Greater Accra Metropolitan Area of Ghana using census and remote sensing data
Our approach using ubiquitously available administrative and satellite data can be used to identify deprived neighbourhoods where interventions are warranted to improve living conditions, and track progress in achieving the Sustainable Development Goals aiming to reduce the population living in unsafe or vulnerable human settlements
Impact of Covid‐19 on Maternal Health Seeking in Ghana
The Covid‐19 pandemic is widely speculated to have disrupted the delivery of primary health care in low‐income countries. Yet, there is little rigorous empirical research identifying this effect. This paper estimates the impact of Covid‐19 on facility and skilled delivery and utilisation of antenatal care (ANC) services by comparing these outcomes for women who were pregnant/delivered before and during the Covid‐19 period. The results show that C…
Spatial heterogeneity in drinking water sources in the Greater Accra Metropolitan Area (Gama), Ghana
Universal access to safe drinking water is essential to population health and well-being, as recognized in the Sustainable Development Goals (SDG). To develop targeted policies which improve urban access to improved water and ensure equity, there is the need to understand the spatial heterogeneity in drinking water sources and the factors underlying these patterns. Using the Shannon Entropy Index and the Index of Concentration at the Extremes at …
The impact of a personalized, community-based counselling and referral programme on modern contraceptive use in urban Ghana
Community-based demand-generation family planning programmes have been associated with increased contraceptive use in rural areas of Ghana. However, rigorous evaluations of such programmes in urban contexts are lacking. We used a retrospective, cross-sectional with comparison group design to estimate the immediate and sustained impact of the Willows intervention on modern contraceptive use in Kumasi, Ghana. The Willows intervention is a home-base…
Dynamics of Emergency Contraceptive Use in Accra, Ghana
Recent data suggest increasing rates of emergency contraception (EC) use in Ghana, particularly in urban areas. In 2018, we collected survey data from 3,703 sexually experienced women aged 16–44 years living in low‐income settlements of Accra. We estimated the prevalence of lifetime and current EC use. Multivariate logistic regression was used to assess factors associated with current use of EC. Retrospective monthly calendar data on contraceptiv…
Evaluating health service coverage in Ghana’s Volta Region using a modified Tanahashi model
Background: The United Nations 2030 Sustainable Development Goals have reaffirmed the international community’s commitment to maternal, newborn, and child health, with further investments in achieving quality essential service coverage and financial protection for all. Objective: Using a modified version of the 1978 Tanahashi model as an analytical framework for measuring and assessing health service coverage, this paper aims to examine the system…
Community perceptions of universal health coverage in eight districts of the Northern and Volta regions of Ghana
Background: Ever since Ghana embraced the 1978 Alma-Ata Declaration, it has consigned priority to achieving ‘Health for All.’ The Community-based Health Planning and Services (CHPS) Initiative was established to close gaps in geographic access to services and health equity. CHPS is Ghana’s flagship Universal Health Coverage (UHC) Initiative and will soon completely cover the country with community-located services. Objectives: This paper aims to i…
Adapting a nurse-led primary care initiative to cardiovascular disease control in Ghana
CHPS staff expressed multiple constraints to CVD care, but also cited actions to address them: CVD-focused training, provision of essential equipment and pharmaceuticals, community education campaigns, and referral and outreach transportation equipment. Results attest to the need for trial of these interventions to assess their impact on CVD risk factors such as hypertension, depression, and alcohol abuse
Deliberation for Development
This article poses the problem of public consultation in developing countries and applies a solution in Ghana as a test case. It describes the theoretical rationale for deliberative consultation with random samples, describes specific criteria for success, and then assesses an application under the challenging conditions of a developing country. It builds on notions of “deliberative democracy,” and shows how they can be practically realized in an…
Does the provision of community health services offset the effects of poverty and low maternal educational attainment on childhood mortality? An analysis of the equity effect of the Navrongo experimen…
The Government of Ghana has instituted a National Poverty Reduction Program with an initiative known as the Community-based Health Planning and Services (CHPS) as its core health development strategy. CHPS was derived from a plausibility trial of the Navrongo Health Research Centre testing four contrasting primary health care strategies: i) Training unpaid volunteers to promote health in communities, ii) placing nurses in communities with trainin…
A qualitative appraisal of stakeholder reactions to a tool for burden of disease–based health system budgeting in Ghana
BACKGROUND: In 2010, the Ghana Health Service launched a program of cooperation with the Tanzania Ministry of Health and Social Welfare that was designed to adapt Tanzania's PLANREP budgeting and reporting tool to Ghana's primary health care program. The product of this collaboration is a system of budgeting, data visualization, and reporting that is known as the District Health Planning and Reporting Tool (DiHPART). OBJECTIVE: This study was con…
Child Wanted and When? Fertility Intentions, Wantedness, and Child Survival in Rural Northern Ghana
Panel survey data collected in rural northern Ghana asked women about the “wantedness” status of their children. Parous women were asked whether they wanted more children, while those who had never had a child were asked whether they wanted to have children in the future; those who said that they did not want to have any more children in the future were asked whether they wanted to become pregnant when they last became pregnant and, if so, whethe…
Respondents’ Exposure to Community‐based Services and Reported Fertility‐Regulation Behavior
This article examines a decade of demographic surveillance data from rural northern Ghana linked with prospective panel survey data recording respondents’ reported fertility‐regulation behavior. We assess the impact of access to community‐based contraceptive services, reported fertility‐regulation behavior, and their interaction on the risk of a conception that results in a birth. The effects of service exposure differ by marital status. Reported…
Socio-economic and demographic determinants of under-five mortality in rural northern Ghana
BACKGROUND: In spite of global decline in under-five mortality, the goal of achieving MDG 4 still remains largely unattained in low and middle income countries as the year 2015 closes-in. To accelerate the pace of mortality decline, proven interventions with high impact need to be implemented to help achieve the goal of drastically reducing childhood mortality. This paper explores the association between socio-economic and demographic factors and…
The Long‐Term Fertility Impact of the Navrongo Project in Northern Ghana
This study assesses the long‐term fertility impact of the Community Health and Family Planning Project of the Navrongo Health Research Centre in Ghana and addresses policy debates concerning the role of family planning programs in rural Africa. Conducted in a remote traditional area on Ghana's northern border, the study tests the hypothesis that convenient family planning service delivery can induce and sustain reproductive change in a societal c…
Monitoring the millennium development goals
The Millennium Declaration, adopted by the United Nations (UN) in 2000, set a series of Millennium Development Goals (MDGs) as priorities for UN member countries, committing governments to realising eight major MDGs and 18 associated targets by 2015. Progress towards these goals is being assessed by tracking a series of 48 technical indicators that have since been unanimously adopted by experts. This concept paper outlines the role member Health …
Health inequalities among older men and women in Africa and Asia
BACKGROUND: Declining rates of fertility and mortality are driving demographic transition in all regions of the world, leading to global population ageing and consequently changing patterns of global morbidity and mortality. Understanding sex-related health differences, recognising groups at risk of poor health and identifying determinants of poor health are therefore very important for both improving health trajectories and planning for the heal…
Ageing and adult health status in eight lower-income countries
BACKGROUND: Globally, ageing impacts all countries, with a majority of older persons residing in lower- and middle-income countries now and into the future. An understanding of the health and well-being of these ageing populations is important for policy and planning; however, research on ageing and adult health that informs policy predominantly comes from higher-income countries. A collaboration between the WHO Study on global AGEing and adult h…
Population and African Society
African society is quite distinct in both its relationship to the rest of the world and its history. Africa seems beset by a myriad of crises, from famines, to civil wars, to economic meltdowns. In addition to these crises, profound population and social change has occurred. These changes are having extraordinary socioeconomic, political, and cultural consequences. Africa's demographic regimes have also been profoundly affected by the HIV/AIDS ep…
Spousal Communication and Family Planning Behavior in Navrongo
The poor performance of most family planning programs in the 1980s, especially in sub‐Saharan Africa, generated concern among researchers and led to a quest for explanations. In most countries, the alienation of men from participation in these programs was subsequently identified as one of the major causes, a finding that led researchers to redirect their attention to couples instead of individuals as the focus of such programs. Lack of spousal c…
Maternal Mortality Among the Kassena‐Nankana of Northern Ghana
This report presents key findings from a maternal mortality study conducted in the Kassena‐Nankana District of northern Ghana in 1997–98. Sibling history data collected in the course of this survey are analyzed together with longitudinal data from the Navrongo Demographic Surveillance System (NDSS). A comparison between mortality data from these two sources indicates that obtaining reasonably accurate estimates of age‐specific death rates is poss…
Women's Fears and Men's Anxieties
The Navrongo experiment, a family planning and health project in northern Ghana, has demonstrated that an appropriately designed, community‐based family planning program can produce a change in contraceptive practice that had been considered unattainable in such a setting. Simultaneously, however, evidence suggests that newly introduced family planning services and contraceptive availability can activate tension in gender relations. In this socie…
