A Medicaid-Funded Statewide Diabetes Quality Improvement Collaborative
We used a collective impact model to form a statewide diabetes quality improvement collaborative to improve diabetes outcomes and advance diabetes health equity. BACKGROUND: The prospective payment system (PPS) for nursing homes was designed to curtail the rapid expansion of Medicare costs for skilled nursing care.
