The case of a 62-year-old transsexual female admitted to the trauma service after a car accident is presented. The patient developed new onset of paralysis and pain three days after admission, just as the hospital was preparing to discharge her. Physicians on the trauma, neurology, physical therapy, and psychiatry services initially diagnosed a conversion disorder and nearly missed a spinal cord injury.
