A New Framework for Describing and Quantifying the Gap Between Proof and Practice
BACKGROUND: Substantial gaps often exist between every day practice and best practice as defined by research evidence. We present a framework for defining, analyzing, and quantifying such proof-to-practice gaps. METHOD: An intervention's use can be plotted over time as ideal and actual uptake curves among candidates and noncandidates. Gaps of underuse are deviations from ideal uptake among candidates and can be quantified as underuse NNPs (Number…
Staff involvement and special follow-up time increase physicians' counseling about smoking cessation
We compared the counseling behaviors of two groups of health maintenance organization physicians: one group received training about smoking cessation counseling; the other group received the same training plus staff support and appointment time specially designated for follow-up of smokers. We interviewed patients after their office visits to measure smoking counseling. The group receiving staff support and designated follow-up time counseled mor…
Optimum cutoff points for biochemical validation of smoking status
Selection of cutoff points for tests to validate smoking cessation should take account of the prevalence of deception. When the prevalence of deception is relatively low, the cutoff points to validate quitting should be relatively high. Many studies have used cutoff points that are too low and may have underestimated cessation rates. We present a method for determining the best cutoff points that takes account of the prevalence of deception
