Essential public health functions: The key to resilient health systems
Integrated multisectoral non-communicable disease prevention and control in China: A review of agencies and policies
T he epidemic of non-communicable diseases (NCDs) has become a global threat to human life, health, and sustainable development, particularly in low-and middle-income countries In 2017, NCDs accounted for 73.4% of all deaths worldwide and 89.5% for China The most common NCDs, eg, cardiovascular diseases (CVD), cancer, chronic respiratory diseases, and diabetes, are preventable. Mental disorders have recently been added to the list of major NCDs
The Role of Cities in Reducing Smoking in China
China is the epicenter of the global tobacco epidemic. China grows more tobacco, produces more cigarettes, makes more profits from tobacco and has more smokers than any other nation in the world. Approximately one million smokers in China die annually from diseases caused by smoking, and this estimate is expected to reach over two million by 2020. China cities have a unique opportunity and role to play in leading the tobacco control charge from t…
Challenges for philanthropy and tobacco control in China (1986–2012)
International engagement has been an important dimension of tobacco control in China. Recognising the large burden of illness and capitalising on proven effective control measures, philanthropic organisations understandably seized the opportunity to achieve major health gains. Much of the international philanthropic investment has been directed at public information, policy change and building the Chinese research knowledge base. Documenting rese…
The value of research as a component of successful tobacco control in China
Research has been shown to be a critical component of successful national tobacco control programmes. China currently has a small number of dedicated researchers addressing tobacco use and control. We encourage the growth of tobacco research as an academic and governmental field of inquiry. Such research would include multiple foci: biologic and toxicologic, epidemiologic, economic, health promotion, evaluation, policy and regulatory, and legal. …
Smoking behaviours and cessation services among male physicians in China: Evidence from a structural equation model
The smoke-free policy is not strictly implemented at healthcare facilities, and smoking remains a public health problem among male physicians. A holistic approach, including a stricter implementation of the smoke-free policy, comprehensive education on the hazards of smoking, training in standard smoking-cessation techniques and provision of cessation resources, is needed to curb the smoking epidemic among male physicians and to promote smoking c…

Los institutos nacionales de salud pública: Un enfoque coordinado e integral contra la influenza A H1N1 y otras amenazas a la salud pública
The current influenza A H1N1 epidemic has demonstrated once again the importance of being able to count on robust, coordinated, and comprehensive public health systems. Countries that do rely on such health systems have proven to recognize, diagnose, and treat influenza in a timely manner, and to provide the public with the education needed to minimize the number of deaths and acute cases. The International Association of National Public Health I…

Managed care and approaches to tobacco control
This issue of Tobacco Control contains the proceedings of a conference, Addressing tobacco in managed care: partnering for success , held on 3 and 4 February 1998 in Arlington, Virginia, United States. The conference was intended to encourage the use of the Agency for Health Care Policy and Research (AHCPR) smoking cessation guideline1 in health plans and to provide an opportunity for such plans to share their experiences and best practices regar…

Vitamin and mineral supplement use and mortality in a US cohort
OBJECTIVES. Vitamin and mineral supplementation is a common practice in the United States, yet little is known about the long-term health effects of regular supplement use. METHODS. To examine the relationship between reported use of supplements and mortality, we analyzed data from US adults 25 to 74 years of age who were examined in the First National Health and Nutrition Examination Survey (1971 to 1975), with vital status determined through 19…
Live or inactivated poliomyelitis vaccine: An analysis of benefits and risks
Using decision analysis we evaluated the benefits and risks of continued primary reliance on oral poliomyelitis vaccine (OPV) compared to use of inactivated poliovirus vaccine (IPV). We followed a hypothetical cohort of 3.5 million children from birth to age 30 assuming 95 per cent coverage with 98 per cent effective vaccine. Primary reliance on IPV would result in more cases of paralytic poliomyelitis as well as more susceptibles remaining in th…
A cost-effectiveness analysis of exercise as a health promotion activity
We used cost-effectiveness analysis to estimate the health and economic implications of exercise in preventing coronary heart disease (CHD). We assumed that nonexercisers have a relative risk of 2.0 for a CHD event. Two hypothetical cohorts (one with exercise and the other without exercise) of 1,000 35-year-old men were followed for 30 years to observe differences in the number of CHD events, life expectancy, and quality-adjusted life expectancy.…
Decision analysis and polio immunization policy
Dr. Salk's comments on our paper fall into four major categories: our use of the techniques of decision analysis, the assumptions we used, the fact that we did not include individual and social values in the model, and the way in which vaccine policies are developed in the United States. We believe that the methods were used correctly, that the assumptions we used are defensible, and that our conclusions were both appropriate and appropriately wo…
Nutrient intake and supplementation in the United States (NHANES II)
Data from the second United States Health and Nutrition Examination Survey (NHANES II) suggest that almost 35 per cent of the US population 18 to 74 years of age takes vitamin/mineral supplements regularly. Both higher nutrient intake and use of vitamin supplementation are associated with older ages, higher income, higher education level, and White race. Accounting for differences associated with sex, age, race, income, and education, persons wit…
Benefits, risks and costs of immunization for measles, mumps and rubella
For a single year, 1983, we compared the actual and estimated morbidity, mortality, and costs attributable to measles, mumps, and rubella with having or not having a childhood immunization program using the combined measles-mumps-rubella (MMR) vaccine. Without an immunization program, an estimated 3,325,000 cases of measles would occur as compared to 2,872 actual cases in 1983 with a program. Instead of an expected 1.5 million rubella cases annua…
The barefoot doctor: Shanghai County revisited
Since a 1981 review of health status and health services in Shanghai County, there have been considerable social and economic changes in the People's Republic of China. A major question currently is the impact of the new economic "responsibility system," which was introduced in 1982, on the cooperative health structure and the "barefoot doctors" (BFDs). Investigators in other areas of China have reported the collapse of the cooperative systems an…
