Adventures in the Evidence-Based Management Trade
I have spent more than a decade trying to get managers and those who study management to use an evidence-based process where interventions to improve performance are concerned. This chapter describes my experiences including teaching a graduate capstone course, writing a text on evidence-based management in health-care, decision making as a board member of a large hospital, and attempting to launch evidence-based management research with practiti…
Assessing medicaid managed care in eastern state
This case examined the expanding role of so-called managed care programs in improving health care for the poor while controlling runaway health care costs. The case asked what the commissioner of health, Dr. Lorna Hill, in a large eastern state should do to effectively monitor Medicaid managed care programs in her state. The commissioner faced intense pressures for cost containment and strong, but not universal, support for the managed care solut…

State Health Policy and Rural Hospitals
Many rural hospitals are experiencing difficulties. This article explores the views of various government and hospital officials on state health policy for rural hospitals. The authors discuss how these officials define the rural hospital issues and suggest appropriate state interventions to assure hospital viability and local access to care. The authors recommend that states, hospitals, and communities decide through a formal process what level …
Implementing Swing‐Bed Services in Small Rural Hospitals
Since 1983, twenty‐six small rural hospitals in five states have been developing models of the “swing‐bed” concept as part of a coordinated national demonstration project. Based on the experiences of these hospitals, swing‐bed programs use excess hospital capacity to provide short‐term, post‐acute care in rural communities where there are nursing home shortages, and, thus, help avoid the need for new nursing home construction. The availability of…
The Sick Citadel
Improving Community Hospital Board Performance
There has been little focus on improving hospital governance as an approach to controlling costs and improving the quality of care in community hospitals. The author examines assumptions underlying such an approach, describes various proposals, and presents criteria to be used in evaluating them. The author recommends governing board involvement in goal setting and monitoring activities. This requires the development of standards on hospital perf…

State Regulation of Health Care Costs
State regulation of health care costs has been increasing at the same time as criticism of governmental regulation in other sectors of the economy has been increasing. The authors examine major system elements necessary to effect cost control in health care, present normative models of value preferences which underlie alternative state cost control systems, and indicate control foci and intervention strategies for state government under the alter…

Home Care and Extended Care in a Comprehensive Prepayment Plan
Hospital Board Members as Policy-Makers
The role of hospital boards in making policy is examined. Board members of 47 large hospitals in southeastern Pennsylvania and southwestern New Jersey are surveyed concerning their assessments of 1) board power relative to the executive director, 2) key hospital priority areas for the next three years, and 3) their qualifications to make decisions in these priority areas. Relative to other operational areas, a greater number of governing board me…
