Why Is End-of-Life Spending So High? Evidence from Cancer Patients
We study the sources of high end-of-life spending for cancer patients. Even among patients with similar initial prognoses, spending in the year postdiagnosis is over twice as high for those who die within the year than those who survive. Elevated spending on decedents is predominantly driven by higher inpatient spending, particularly low-intensity admissions. However, most such admissions do not result in death, making it difficult to target spen…
BNT162b2 mRNA Covid-19 Vaccine in a Nationwide Mass Vaccination Setting
BACKGROUND: As mass vaccination campaigns against coronavirus disease 2019 (Covid-19) commence worldwide, vaccine effectiveness needs to be assessed for a range of outcomes across diverse populations in a noncontrolled setting. In this study, data from Israel's largest health care organization were used to evaluate the effectiveness of the BNT162b2 mRNA vaccine. METHODS: All persons who were newly vaccinated during the period from December 20, 20…

Prediction Accuracy With Electronic Medical Records Versus Administrative Claims
OBJECTIVE: The objective of this study was to evaluate the incremental predictive power of electronic medical record (EMR) data, relative to the information available in more easily accessible and standardized insurance claims data. DATA AND METHODS: Using both EMR and Claims data, we predicted outcomes for 118,510 patients with 144,966 hospitalizations in 8 hospitals, using widely used prediction models. We use cross-validation to prevent overfi…

Preventing the September Epidemic through a Multi-Faceted Approach
Background: The "September Epidemic" is a well-known phenomenon: there is an increased likelihood of asthma exacerbations among children during the first month of school. Likely triggers include a combination of factors such as the stress of starting a new school year, the exposure to classrooms and respiratory viruses from fellow classmates, and concurrent weather patterns due to a change in season.Furthermore, as many children with a known hist…
Second Medical Opinion
BACKGROUND: Second opinion (SO) is common in medical practice and can reduce unnecessary risks and costs. To date, there is no population-based estimation of how many people seek SOs and what the characteristics of second-opinion seekers are. OBJECTIVES: To estimate how many people seek SOs, and what the characteristics of second-opinion seekers are. METHODS: We conducted both a medical records analysis (n=1,392,907) and a cross-sectional nationa…
Predicting 30-Day Readmissions With Preadmission Electronic Health Record Data
BACKGROUND: Readmission prevention should begin as early as possible during the index admission. Early identification may help target patients for within-hospital readmission prevention interventions. OBJECTIVES: To develop and validate a 30-day readmission prediction model using data from electronic health records available before the index admission. RESEARCH DESIGN: Retrospective cohort study of admissions between January 1 and March 31, 2010.…
Is socioeconomic status associated with utilization of health care services in a single-payer universal health care system
The research shows that even in a universal health care system SES is associated with utilization of health care services. In order to improve equity in utilization of services the Israeli public health should reduce economic barriers and in parallel invest in making information accessible to improve "navigation skills" for all

Determinants of emergency response willingness in the local public health workforce by jurisdictional and scenario patterns
Response willingness deficits in the local public health workforce pose a threat to all-hazards response capacity and health security. Local public health agencies and their stakeholders may incorporate key findings, including identified scenario-based willingness gaps and the importance of efficacy, as targets of preparedness curriculum development efforts and policies for enhancing response willingness. Reasons for an increased willingness in r…

Assessing socioeconomic health care utilization inequity in Israel
Identification of socioeconomic differences in health care utilization is an important step in disparity reduction efforts. Adjustment for health-needs using a comprehensive morbidity burden diagnoses-based measure, this study showed relative underutilization in use of specialist and diagnostic services, and thus allowed for identification of inequity in health resources use, which could not be detected with less comprehensive forms of health-nee…
Feeling Vigorous and the Risks of All-Cause Mortality, Ischemic Heart Disease, and Diabetes
OBJECTIVE: To investigate prospectively the effects of vigor at work on the end points of mortality and the prevalence of ischemic heart disease (IHD) and diabetes. METHODS: We tested the hypothesized beneficial effects of feeling vigorous at work at baseline on the risks of all-cause mortality, IHD, and diabetes during a 20-year follow-up. Participants were healthy employees (n = 968) who underwent a routine health check at baseline. We calculat…
Characterizing hospital workers' willingness to report to duty in an influenza pandemic through threat- and efficacy-based assessment
Hospital-based providers' willingness to report to work during an influenza pandemic is a critical yet under-studied phenomenon. Witte's Extended Parallel Process Model (EPPM) has been shown to be useful for understanding adaptive behavior of public health workers to an unknown risk, and thus offers a framework for examining scenario-specific willingness to respond among hospital staff. We administered an anonymous online EPPM-based survey about …
Local public health workers' perceptions toward responding to an influenza pandemic
The perceived risk among public health workers was shown to be associated with several factors peripheral to the actual hazard of this event. These risk perception modifiers and the knowledge gaps identified serve as barriers to pandemic influenza response and must be specifically addressed to enable effective local public health response to this significant threat
