The impact of heat waves on mortality in 9 European cities: Results from the EuroHeat project
BACKGROUND: The present study aimed at developing a standardized heat wave definition to estimate and compare the impact on mortality by gender, age and death causes in Europe during summers 1990-2004 and 2003, separately, accounting for heat wave duration and intensity. METHODS: Heat waves were defined considering both maximum apparent temperature and minimum temperature and classified by intensity, duration and timing during summer. The effect …
Surveillance of Summer Mortality and Preparedness to Reduce the Health Impact of Heat Waves in Italy
Since 2004, the Italian Department for Civil Protection and the Ministry of Health have implemented a national program for the prevention of heat-health effects during summer, which to-date includes 34 major cities and 93% of the residents aged 65 years and over. The Italian program represents an important example of an integrated approach to prevent the impact of heat on health, comprising Heat Health Watch Warning Systems, a mortality surveilla…
Temporal and geographic heterogeneity of the association between socioeconomic position and hospitalisation in Italy: An income based indicator
Our study confirms an inverse association between income level and the use of hospitalization in four Italian cities, using a small area economic indicator, based on population tax data. Further analysis of the association between income and cause-specific hospitalization rates will allow to better understand the capability of the Italian National Health System to compel with socio-economic inequalities in health needs.Furthermore the SEP indicat…
Income level and chronic ambulatory care sensitive conditions in adults: A multicity population-based study in Italy
The socioeconomic gradient in ACSC hospitalization rates confirms the gap in health status between social groups in our country. Insufficient or ineffective primary care is suggested as a plausible additional factor aggravating inequality. This finding highlights the need for improving outpatient care programmes to reduce the excess of unnecessary hospitalizations among poor people
The association of socioeconomic disadvantage with postoperative complications after major elective cardiovascular surgery
BACKGROUND: Understanding the mechanism by which both patient- and hospital level factors act in generating disparities has important implications for clinicians and policy-makers. OBJECTIVE: To measure the association between socioeconomic position (SEP) and postoperative complications after major elective cardiovascular procedures. DESIGN: Multicity hospital-based study. SUBJECTS: Using Hospital Discharge Registries (ICD-9-CM codes), 19 310 pat…
Factors affecting in-hospital heat-related mortality: A multi-city case-crossover analysis
BACKGROUND: Several studies have identified strong effects of high temperatures on mortality at population level; however, individual vulnerability factors associated with heat-related in-hospital mortality are largely unknown. The objective of the study was to evaluate heat-related in-hospital mortality using a multi-city case-crossover analysis. METHODS: We studied residents of four Italian cities, aged 65+ years, who died during 1997-2004. For…
Socioeconomic status and hospitalization in the very old: A retrospective study
Lower social status and associated comorbidity, more than age per se, are associated with a higher rate of hospitalization in very old patients
Exposure to fine and ultrafine particles from secondhand smoke in public places before and after the smoking ban, Italy 2005
The application of the smoking ban led to a considerable reduction in the exposure to indoor fine and ultrafine particles in hospitality venues, confirmed by a contemporaneous reduction of urinary cotinine
Indicators of breast cancer severity and appropriateness of surgery based on hospital administrative data in the Lazio Region, Italy
Our findings suggest that administrative data need to be used with caution when evaluating surgical appropriateness, mainly because of the limited ability of SI to predict tumor size and the questionable quality of HIS data as observed in other studies
Socioeconomic differentials in premature mortality in Rome: Changes from 1990 to 2001
Socioeconomic inequalities in health in Rome are still present at the beginning of the 21st century. Strategies to monitor the impact of SEP on mortality over time in different populations should be implemented to direct health policies
Temperature and summer mortality: Geographical and temporal variations in four Italian cities
Study objective: To investigate geographical and temporal variations in the temperature-mortality relation. Design: The relation between mortality and maximum apparent temperature (T appmax ) in 2003, 2004, and a previous reference period was explored by using segmented regression and generalised additive models. Setting: Four Italian cities (Bologna, Milano, Roma, and Torino), included in a national network of prevention programmes and heat heal…
Associations of area based deprivation status and individual educational attainment with incidence, treatment, and prognosis of first coronary event in Rome, Italy
Background: Socioeconomic gradients in the occurrence of myocardial infarction are well known, but few studies have examined socioeconomic disparities in post-infarction outcomes. The objective of this study was to explore relations of socioeconomic status with the incidence, treatment, and outcome of first coronary event in Rome, Italy, during the period 1998–2000, examining effect modification by gender. Methods: Subjects were Rome residents ag…

Classification and regression trees, which are used in defining iso-resource groups, can be affected by overfitting and are based on a priori choices of the splitting attributes. Finally,…
Outcome of Patients With Cardiac Diseases Admitted to Coronary Care Units: A Report From Lazio, Italy
BACKGROUND: Coronary care units (CCUs) currently treat a variety of diseases, but little is known about the effectiveness of CCUs on heart conditions other than acute myocardial infarction. OBJECTIVES: The objectives of this study were to evaluate the association between direct admission to CCUs and the risk of inhospital death in patients with heart disease, to investigate factors affecting direct admission to a CCU, and to assess the effect of …

Assessing effectiveness of home care for persons with Aids: Analysis of methodological problems
The aim of this paper is to assess the methodological problems of an unsuccessful randomized controlled trial (RCT) conducted to evaluate the effectiveness, in terms of survival and quality of life, of the early offer of home care (HC) to persons with AIDS (PWA). The study carried out was an intention-to-treat RCT. Persons in the treatment group (TG) received the offer of HC at the moment of AIDS diagnosis; those in the control group (CG) receive…
Prevalence of HIV infection and risk behaviour among street prostitutes in Rome, 1997-1998
A cross-sectional survey was carried out between April 1997 and February 1998 among street prostitutes in Rome. The study population (n = 142) consisted of 102 women and 40 transsexuals: 20% from Western Europe, 38% Eastern Europe, 23% Latin America and 17% Africa. Two-thirds of the population had more than 20 clients during the last week and most respondents (95%) reported always using condoms with clients. Eight per cent of the women and 2% of …
Coronary artery bypass graft surgery: Socioeconomic inequalities in access and in 30 day mortality. A population-based study in Rome, Italy
Inequality in health: Socioeconomic differentials in mortality in Rome, 1990-95
STUDY OBJECTIVE: Population groups with a lower socioeconomic status (SES) have a greater risk of disease and mortality. The aim of this study was to investigate the relation between SES and mortality in the metropolitan area of Rome during the six year period 1990-1995, and to examine variations in mortality differentials between 1990-92 and 1993-95. DESIGN: Rome has a population of approximately 2,800,000, with 6100 census tracts (CTs). During …
Heterogeneity of home care assistance needs of people with Aids
A multidisciplinary home care service for people with AIDS (PWAs) was started in Rome in September 1990. This paper describes the features of the home care service offered by the Associated Health Care Workers' Co-operative (OSA), an example of the integration of private and state systems. We detail the types and numbers of visits that PWAs have needed, and we explore the possible correlation between demographic and clinical variables and the car…
