Simulating the Fiscal Impact of Anti-Obesity Medications as an Obesity Reduction Strategy
While substantial public health investment in anti-smoking initiatives has had demonstrated benefits on health and fiscal outcomes, similar investment in reducing obesity has not been undertaken, despite the substantial burden obesity places on society. Anti-obesity medications (AOMs) are poorly prescribed despite evidence that weight loss is not sustained using other strategies alone. We used a simulation model to estimate the potential impact of…
Predicting Response to Regulatory Change in the Small Group Health Insurance Market
Lack of health insurance continues to be a concern for many people, even among those who are employed, and employees of small firms are much less likely to be insured than employees of larger firms. For several years, the U.S. Congress has considered legislation that would establish two new vehicles for offering health insurance coverage to small employers: association health plans (AHPs) and HealthMarts. In this paper, we present a model for est…
Specialization Among Obstetrician/Gynecologists
This study uses the Herfindahl index to investigate the specialization patterns of physicians. "Specialization" is defined as the degree to which an individual doctor concentrates his practice into a narrow range of disease categories. This application is the specialization pattern of office-based obstetrician/gynecologists in the United States. Physician's age and solo practice both exhibit a systematic negative effect on specialization. Both th…
Physicians' Services and the Division of Labor Across Local Markets
This paper reports empirical evidence of systematic cross-locale variation in the degree of division of labor among physicians. A theoretical model-based on an individual producer's trade-off between increasing returns and falling marginal revenue within each activity-motivates the empirical tests. At two levels of aggregation, specialization is correlated with local demand shifters for medical services. At the individual level, the author finds …
The Division of Labor, Local Markets, and Worker Organization
The model developed here explains differences in the degree of the division of labor across local markets. For example, the typical doctor in one local market may treat a wider range of patients' problems than his counterpart in another local market. The extent of the division of labor depends on variables affecting the local demand for services. Two different forms of worker organization, cooperation and noncooperation, yield divergent results. …
