  • Effects of Prescription Coinsurance and Income-Based Deductibles on Net Health Plan Spending for Older Users of Inhaled Medications

    Colin R Dormuth, Peter Neumann et al.•ARTICLE•Medical Care•2009•Referencias: 15

BACKGROUND: Health plans that increase prescription cost-sharing for their patients may increase overall plan costs. We analyzed the impact on health plan spending of a switch in public drug insurance from full coverage to a prescription copayment (copay), and then to income-based deductibles plus coinsurance (IBD). METHODS: We studied British Columbia residents 65 years of age or older who were dispensed inhaled steroids, beta2 agonists or antic…

  • Designed Delays Versus Rigorous Pragmatic Trials

Malcolm Maclure, Bruce Carleton et al.•ARTICLE•Medical Care•2007•Referencias: 18

BACKGROUND: Centralized administrative databases enable low-cost pragmatic randomized trials (PRTs) of drug effectiveness and safety. We simplified the PRT strategy by using designed delays (DD) to evaluate drug policies. OBJECTIVES: To reassess our DD trial of a cost-saving nebulizer-to-inhaler conversion policy and a proposed DD trial of reduced restrictions on Cox-2 inhibitors. RESEARCH DESIGN: We randomized 52 pairs of communities and cluster…

  • Increasing Levels of Restriction in Pharmacoepidemiologic Database Studies of Elderly and Comparison With Randomized Trial Results

    Sebastian Schneeweiss, Amanda R Patrick et al.•ARTICLE•Medical Care•2007•Referencias: 24

BACKGROUND: The goal of restricting study populations is to make patients more homogeneous regarding potential confounding factors and treatment effects and thereby achieve less biased effect estimates. OBJECTIVES: This article describes increasing levels of restrictions for use in pharmacoepidemiology and examines to what extent they change rate ratio estimates and reduce bias in a study of statin treatment and 1-year mortality. METHODS: : The s…

  • Net Health Plan Savings From Reference Pricing for Angiotensin-Converting Enzyme Inhibitors in Elderly British Columbia Residents

    Sebastian Schneeweiss, Colin R Dormuth et al.•ARTICLE•Medical Care•2004•Referencias: 10

BACKGROUND: Reference drug pricing (RP) is a cost-sharing strategy commonly used to control drug expenditures. Under RP, a benefit plan fully reimburses medications that are equally or less expensive than the reference price, and requires patients to pay the extra cost of therapeutically equivalent but higher priced drugs. Critics argued that drug plan savings are offset by administrative costs and increased spending on other health services. OBJ…

  • The Case-Crossover Design

    Malcolm Maclure•ARTICLE•American Journal of Epidemiology•1991

A case-control design involving only cases may be used when brief exposure causes a transient change in risk of a rare acute-onset disease. The design resembles a retrospective nonrandomized crossover study but differs in having only a sample of the base population-time. The average incidence rate ratio for a hypothesized effect period following the exposure is estimable using the Mantel-Haenszel estimator. The duration of the effect period is as…

  • Elevated blood levels of carcinogens in passive smokers

Malcolm Maclure, Ronit B-A Katz et al.•ARTICLE•American Journal of Public Health•1989•Citada por: 6•Referencias: 9

The hypothesis that involuntary exposure to tobacco smoke--passive smoking--results in greater risk of cancer was assessed by measuring the levels of two known carcinogens in the blood of 57 nonsmokers with varying degrees of involuntary exposure, including six heavily exposed bartenders. The concentrations of hemoglobin adducts of 4-aminobiphenyl, a bladder carcinogen, were significantly higher in subjects with confirmed involuntary exposure (pl…
