How PCORnet® Could Advance Postmarket Evidence Generation
The PCORnet vision to accelerate progress towards a learning health system by enabling more efficient generation of evidence is shared by the US Food and Drug Administration, where we recently served. One way of achieving this goal is to develop national networks capable of running streamlined clinical trials in the postmarket setting. In this invited commentary, we discuss how PCORnet ® and similar networks could facilitate streamlined trials in…
Needles in a Haystack: Finding Qualitative and Quantitative Collaborators in Academic Medical Centers
Translational research is a data-driven process that involves transforming scientific laboratory- and clinic-based discoveries into products and activities with real-world impact to improve individual and population health. Successful execution of translational research requires collaboration between clinical and translational science researchers, who have expertise in a wide variety of domains across the field of medicine, and qualitative and qu…
Life expectancy and voting patterns in the 2020 U.S. presidential election
Counties with a less positive trajectory in life expectancy were more likely to vote for the Republican candidate in the 2020 U.S. Presidential election, but the Republican candidate's share improved in some counties that experienced marked gains in life expectancy. Associations were moderated by demographic, social and economic factors
Prescriber Continuity and Disease Control of Older Adults
BACKGROUND: Medicare beneficiaries with multiple chronic conditions are typically seen by multiple providers, particularly specialists. Clinically appropriate referrals to multiple specialists may compromise the continuity of care for multiple chronic condition beneficiaries and create care plans that patients may find challenging to reconcile, which may impact patient outcomes. OBJECTIVE: The objective was to examine whether glycemic control or …
Linkage of Laboratory Results to Medicare Fee-for-Service Claims
BACKGROUND: Medicare is the single largest purchaser of laboratory testing in the United States, yet test results associated with Medicare laboratory claims have historically not been available. OBJECTIVE: The purpose of this study was to describe both the linkage of laboratory results data to Medicare claims and the completeness of these results data. In a subgroup of beneficiaries initiating angiotensin-converting enzyme inhibitors or angiotens…
Early life predictors of atrial fibrillation-related mortality: Evidence from the health and retirement study
Process of Care Performance Measures and Long-Term Outcomes in Patients Hospitalized With Heart Failure
BACKGROUND: Recent efforts to improve care for patients hospitalized with heart failure have focused on process-based performance measures. Data supporting the link between current process measures and patient outcomes are sparse. OBJECTIVE: To examine the relationship between adherence to hospital-level process measures and long-term patient-level mortality and readmission. RESEARCH DESIGN: Analysis of data from a national clinical registry link…
External Validity of the Cardiovascular Health Study: A Comparison With the Medicare Population
BACKGROUND: The Cardiovascular Health Study (CHS), a population-based prospective cohort study, has been used to identify major risk factors associated with cardiovascular disease and stroke in the elderly. OBJECTIVE: To assess the external validity of the CHS. RESEARCH DESIGN: Comparison of the CHS cohort to a national cohort of Medicare beneficiaries and to Medicare beneficiaries residing in the CHS geographic regions. SUBJECTS: CHS participant…
Using Inverse Probability-Weighted Estimators in Comparative Effectiveness Analyses With Observational Databases
Inverse probability-weighted estimation is a powerful tool for use with observational data. In this article, we describe how this propensity score-based method can be used to compare the effectiveness of 2 or more treatments. First, we discuss the inherent problems in using observational data to assess comparative effectiveness. Next, we provide a conceptual explanation of inverse probability-weighted estimation and point readers to sources that …
The Insurance Effect on Prescription Drug Expenditures Among the Elderly: Findings From the 1997 Medical Expenditure Panel Survey
BACKGROUND: Despite continuing debate over a prescription drug benefit for the Medicare program, there has been relatively little research estimating the potential cost of providing such a benefit. OBJECTIVE: The objective of this study was to estimate the effect of prescription drug insurance on outpatient prescription drug expenditures among the elderly. RESEARCH DESIGN: We studied respondents aged > or =65 years to the 1997 Medical Expenditure…
Does the Ownership of the Admitting Hospital Make a Difference: Outcomes and Process of Care of Medicare Beneficiaries Admitted With Acute Myocardial Infarction
BACKGROUND: Concerns have been expressed about quality of for-profit hospitals and their use of expensive technologies. OBJECTIVE: To determine differences in mortality after admission for acute myocardial infarction (AMI) and in the use of low- and high-tech services for AMI among for-profit, public, and private nonprofit hospitals. STUDY DESIGN, SETTING, AND PATIENTS: Cooperative Cardiovascular Project data for 129,092 Medicare patients admitte…
Welfare Reform and Latinas' Use of Perinatal Health Care
We read with concern the conclusion of Joyce et al. that the Personal Responsibility and Work Opportunity Reconciliation Act (PRWORA) had little effect on perinatal health care use among foreign-born Latinas.1 We are particularly concerned about whether the data can be interpreted clearly enough to support this conclusion. Also, regardless of the findings, we wonder about the relevance of this study to our understanding of welfare reform's impact…
The Value of Patient-Reported Health Status in Predicting Short-Term Outcomes After Coronary Artery Bypass Graft Surgery
BACKGROUND: Risk stratification for comparison of outcomes after coronary artery bypass grafting (CABG) typically includes only clinical measures of risk. Patient-reported health status may be an important independent predictor of short-term health outcomes. OBJECTIVE: To determine whether patient-reported health status, as measured by the Physical and Mental Component Summary scores of the SF-36, predicts in-hospital mortality and prolonged leng…
