Childhood asthma along the United States/ Mexico border: Hospitalizations and air quality in two California counties
Since the passage of the North American Free Trade Agreement in 1993, there has been an increasing need to monitor environmental health trends that may be related to the rapid industrialization of the United States/Mexico border. We studied two counties on the California/Baja California border to obtain baseline data on trends in childhood asthma hospitalizations and two pollutants that aggravate asthma, ozone and particulate matter (less than 10…
The Case-Control Method in Medical Care Evaluation
The case-control method has been applied extensively to the study of chronic diseases, largely because of its advantage in cost and statistical power over other study designs. However, problems in ensuring the validity of case-control results have led to certain reservations regarding the general utility of the method. We discuss how several of the major validity problems in case-control studies of chronic disease are limited or absent in case-co…
Impact of Patient Perceptions on Compliance with Treatment for Hypertension
Patient noncompliance with medical regimens is a major obstacle in achieving hypertension control. In this study the relationships between patients' perceptions of health, disease and medical treatment and compliance were examined in personal interviews with 142 patients under treatment for hypertension. Compliance was measured in terms of blood pressure control, self-reported medication-taking, and appointment keeping. A multivariate log linear …
Appendicitis: Decreasing Normal Removals Without Increasing Perforations
There is wide interhospital variation in the removal rates of histologically normal appendices, and variation also in appendectomy rates among countries and among hospital catchment areas. Decision theory suggests that better patient workup and careful observation of doubtful cases result in improved discrimination between appendicitis patients and those with nonspecific abdominal pain. By improving the data base and weighing evidence with care, …
