Validity of the AHRQ Patient Safety Indicator for Postoperative Physiologic and Metabolic Derangement Based on a National Sample of Medical Records
OBJECTIVE: The Agency for Healthcare Research and Quality Patient Safety Indicator (PSI) 10, "Postoperative Physiologic and Metabolic Derangement" (PPMD), uses administrative data to detect postoperative acute kidney injury (AKI) requiring dialysis and diabetes-related complications. We sought to evaluate the indicator's criterion validity. RESEARCH DESIGN: We conducted a retrospective cross-sectional study of hospitalization records flagged posi…
Expanding the Uses of AHRQ's Prevention Quality Indicators: Validity From the Clinician Perspective
BACKGROUND: The Agency for Healthcare Research and Quality's prevention quality indicators (PQIs) are used as a metric of area-level access to quality care. Recently, interest has expanded to using the measures at the level of payer or large physician groups, including public reporting or pay-for-performance programs. However, the validity of these expanded applications is unknown. RESEARCH DESIGN: We conducted a novel panel process to establish …
How Valid is the ICD-9-CM Based AHRQ Patient Safety Indicator for Postoperative Venous Thromboembolism
BACKGROUND: Hospital administrative data are being used to identify patients with postoperative venous thromboembolism (VTE), either pulmonary embolism (PE) or deep-vein thrombosis (DVT). However, few studies have evaluated the accuracy of these ICD-9-CM codes across multiple hospitals. METHODS AND MATERIALS: The Agency for Healthcare Research and Quality (AHRQ) Patient Safety Indicator (PSI)-12 was used to identify cases with postoperative VTE i…

Mortality Among Very Low-Birthweight Infants in Hospitals Serving Minority Populations
Objective. We investigated whether the proportion of Black very low-birth-weight (VLBW) infants treated by hospitals is associated with neonatal mortality for Black and White VLBW infants. Methods. We analyzed medical records linked to secondary data sources for 74050 Black and White VLBW infants (501 g to 1500 g) treated by 332 hospitals participating in the Vermont Oxford Network from 1995 to 2000. Hospitals where more than 35% of VLBW infants …
Predictors and Outcomes of Outpatient Mental Health Care: A 4-Year Prospective Study of Elderly Medicare Patients With Substance Use Disorders
BACKGROUND: Many elderly inpatients have substance use disorders; recent treatment guidelines suggest that they should receive regular outpatient mental health care after discharge from hospital. OBJECTIVE: The prevalence, predictors, and outcomes of outpatient mental health care obtained by elderly Medicare patients with substance use disorders were examined. RESEARCH DESIGN: A longitudinal prospective follow-up was performed. SUBJECTS: Data fro…
