Alvan R Feinstein did not write extensively about the history of medicine. But in the 20th century he himself was a protagonist. He influenced the very meaning of clinical research. He eschewed the concept of the patient as merely the ultimate beneficiary of clinical research and closed the loop by demonstrating that the best clinical data were provided by the patient. A questionnaire containing 11 patient management problems was completed by 495 physicians and medical students at an American and a Canadian medical school. Respondents indicated whether they would order a particular diagnostic test in each case, given five different prices for the test. For several years, the government of Ontario has undertaken to foster and develop experimental systems for the delivery of primary health care in the province. At the same time, the government wished to devise a strategy whereby the new health care programs could be rigorously evaluated.
