Development of instruments to measure the quality of breast cancer treatment decisions
Background Women with early‐stage breast cancer face a multitude of decisions. The quality of a decision can be measured by the extent to which the treatment reflects what is most important to an informed patient. Reliable and valid measures of patients’ knowledge and their goals and concerns related to breast cancer treatments are needed to assess the decision quality. Objective To identify a set of key facts and goals relevant to each of three …
Patient‐centred diagnosis
Nicholas Summerton, Radcliffe: Oxford. Nowhere in clinical practice is the interplay between science and art more evident than it is in the diagnostic process. Diagnosis has never been easy, requiring as it does the myriad skills essential to elicit an accurate medical history from the patient, perform a physical examination, and then discern which among available imaging and laboratory investigations might add useful information. And the difficu…

Involving Patients in Clinical Decisions
BACKGROUND: Back surgery rates are rapidly rising in the United States. This surgery is usually elective, so patient preferences are important in the treatment decision. OBJECTIVES: The objective of this study was to determine the impact on outcomes and surgical choices of an interactive, diagnosis-specific videodisk program for informing patients about treatment choices. RESEARCH DESIGN: This was a randomized, controlled trial at 2 sites compari…
Patient Reactions to a Program Designed to Facilitate Patient Participation in Treatment Decisions for Benign Prostatic Hyperplasia
Patients often want considerable information about their conditions, and enhanced patient participation might reduce unwanted practice variation and improve medical decisions. The authors assessed how men with benign prostatic hyperplasia reacted to an education program designed to facilitate participation in decisionmaking, and how strongly ratings of their symptom state and the prospect of complications predicted their treatment choice. A prosp…
Deliberations on the Dissemination of Port Products
This report outlines the activities undertaken by the Inter-PORT Dissemination work group during its first 2 years of operation. The work group's initial purpose was to assist the individual PORTs in developing their plans for both disseminating research findings and evaluating the effectiveness of these strategies. However, it became quickly apparent that in a discipline little more than a decade old, a commonly understood vocabulary had yet to …

Assessing Patients?? Utilities
Each of the elements of a utility assessment strategy--defining and describing health states of interest, identifying subjects, choosing a scaling task, aggregating across subjects, determining reliability and validity--is controversial. The controversy is in part explained by the interdisciplinary nature of the problem; different disciplinary conceptualizations of utility lead to different priorities for methodologic problem solving. Controversy…
Nursing Home Patients Admitted to a Medical Intensive Care Unit
To investigate admissions from nursing homes to a medical intensive care unit (ICU), the authors detailed the major interventions, costs, and outcomes for such patients (n = 67) over a 3-year period and then compared them with those for ICU patients receiving home care or visiting nurse services (240 patients) before admission and all others older than 65 years of age (949 patients). These three groups comprised 37% of total ICU admissions. In co…
