    BACKGROUND: Six states expanding Medicaid under the Affordable Care Act have obtained waivers to incorporate cost-sharing.     Since 2012, states have had the option of expanding Medicaid to all low-income adults, with nearly full financing from the federal government. To date, 36 states and the District of Columbia have expanded their programs in this way, while 14 states have not.
