Long‐term consumer involvement in cancer research
BACKGROUND: Meaningful consumer involvement in health research is important. There are limited data on how to maintain long-term consumer involvement. OBJECTIVE: To identify barriers and facilitators to meaningful long-term consumer involvement in research. DESIGN: Six semi-structured interviews were conducted with members of the Primary Care Collaborative Cancer Clinical Trials Group (PC4) Community Advisory Group (CAG) and included the review o…
Barriers to employment of Australian cancer survivors living with geographic or socio‐economic disadvantage
BACKGROUND: Opportunities for cancer survivors' employment can both reflect and perpetuate health inequities, as employment is an important social determinant of health. Socio-economic and geographic disadvantage is associated with greater difficulty finding work, but little is known about work needs of Australian cancer survivors living with disadvantage. OBJECTIVE: This study examined survivor and health-care professional (HCP) perspectives on …
Cancer survivorship care and general practice
Primary care providers, including general practice teams (GPTs), are well positioned within the community to integrate cancer survivorship care into ongoing health management. However, roles of GPT members in delivery of cancer survivorship care have not been explored. The purpose of this study is to explore these roles from the perspectives of General Practitioners (GPs), Practice Nurses (PNs) and Practice Managers (PMs). An interpretive qualita…
Comparing Pathways to Diagnosis and Treatment for Rural and Urban Patients With Colorectal or Breast Cancer
PURPOSE: Colorectal cancer patients living in rural areas have poorer outcomes than urban counterparts, but such disparities are not found for breast cancer. Although time to care may contribute to rural-urban disparities, few studies examine patient experiences to understand how and why delays may occur. We compared rural and urban patient experiences of pathways to colorectal or breast cancer diagnosis and treatment in Victoria, Australia. METH…
Development of a fragile X syndrome ( FXS ) knowledge scale
BACKGROUND: Genetic carrier screening is increasingly possible for many conditions, but it is important to ensure decisions are informed. The multidimensional measure of informed choice (MMIC) is a quantitative instrument developed to evaluate informed choice in prenatal screening for Down syndrome, measuring knowledge, attitudes and uptake. To apply the MMIC in other screening settings, the knowledge scale must be modified. OBJECTIVE: To develop…
P2-169 Do mental health clients lack access to general practitioner services
Introduction Emerging evidence indicates an association between mental illness and low-intensity of physical healthcare. To test this, we compared rates of visits to a general practitioner (GP) between mental health clients (MHCs) and non-MHCs. Methods Population-based retrospective cohort study of 204 727 MHCs and 294 076 matched non-MHCs in Western Australia from 1 January 1990 to 30 June 2006, based on linked records of the use of mental healt…
P2-167 Mental illness related disparities in diabetes prevalence, quality of care and outcomes
Introduction To compare the prevalence of diabetes, quality of care and outcomes between mental health clients (MHCs) and non-MHCs. Methods Population-based longitudinal study of 139 208 MHCs and 294 180 matched non-MHCs in Western Australia (WA) from 1990 to 2006, using linked data of WA State mental health registry, electoral roll registrations, hospital admissions, emergency department attendances, deaths, and Commonwealth Medicare and pharmac…
Mortality in Western Australian seniors with chronic respiratory diseases
Older patients with CRD not using ICS experienced the worst survival in this study and may benefit from an escalation in therapeutic regime. Males had a higher risk of death than females, which was more pronounced in the highest pharmacotherapy level group. Hence, primary health care should more actively direct disease management to mild-to-moderate disease patients
Regular Primary Care Decreases the Likelihood of Mortality in Older People With Epilepsy
OBJECTIVES: To investigate the effect of regular general practitioner (GPs) visits, separately from the frequency of visits, on the likelihood of all-cause death and epilepsy-related hospitalization in older Western Australian (WA) patients (>or=65 years) with diagnosed epilepsy. METHODS: We used routinely collected, whole-population linked, administrative medical data to ascertain the study population, the exposure, and outcomes. We identified 3…
Designing and evaluating complex interventions to improve health care
Determining the effectiveness of complex interventions can be difficult and time consuming. Neil C Campbell and colleagues explain the importance of ground work in getting usable results
The GRAIDS Trial
The development and evaluation of computer decision support for the assessment of cancer genetic risk in primary care is reported with two series of studies described: the RAGs (Risk Assessment in Genetics) studies and the GRAIDS (Genetic Risk Assessment in an Intranet and Decision Support) Trial. In the GRAIDS Trial, 45 general practices in Eastern England have been recruited and randomised. Comparison practices attend an educational session and…
Is informed choice in genetic testing a different breed of informed decision‐making? A discussion paper
Traditionally genetic counselling has promoted a non‐directive approach to patients’ decision‐making but the feasibility of this has been questioned. Unlike most branches of medicine, which are shifting away from a paternalistic model, genetic counselling is approaching shared decision‐making from a different perspective. There are certain features of genetic counselling and genetic testing which may complicate the drive towards shared decision‐m…
