Structural barriers to health care as risk factors for preterm and small-for-gestational-age birth among US-born Black and White mothers
The Fading of Protective Roots
The "Hispanic Paradox" of better health despite lower socioeconomic status, was not observed in foreign-born or US-born Hispanics aged 65 years and older. Among elderly immigrants, those with fewer years of residency had the greatest hypertensive risk
Socioeconomic Status & Health Disparities
Research indicates that the relationship between socioeconomic status (SES) and health outcomes are robust, though no ‘gold standard’ as to what best captures SES exists. Many studies use individual proxies to control for SES, but there are a number of limitations to doing so. Additionally, little research has been conducted as to how to develop a US-based composite SES index that can be utilized across various datasets. The aim of the study is t…
Provider Practice Competition and Adoption of Medicare’s Oncology Care Model
BACKGROUND: There is a concern that the Oncology Care Model (OCM), a voluntary bundled payment program, may incentivize mergers and acquisitions among physician practices leading to reduced competition and price increases. These concerns are heightened if OCM is preferentially adopted in competitive health care markets because it could result in reduced competition, but little is known about the characteristics of markets where OCM is adopted. OB…
The nexus between health and time use among racially and ethnically diverse women
Researchers and policy makers designing culturally sensitive physical activity health-related interventions should consider patterns of time use that are associated with poor health. Programs designed to improve sleep time and reduce sedentary television-viewing time may be as important as interventions designed to increase physical activity time. These broader patterns of time use mediated the relationships between race/ethnicity and physical an…
Factors associated with health service utilization under the New Rural Cooperative Medical System (NRCMS) in Hainan, China
In 2003, the Chinese rural health care system underwent substantial reform with the introduction of the New Rural Cooperative Medical System (NRCMS). In order to identify factors associated with health services utilization among rural residents enrolled in NRCMS in Hainan Province, China, we conducted a cross-sectional household survey in rural villages from six randomly selected Han and ethnic minority cities or counties in Hainan in 2009 to ide…
Reevaluating the Benefits of Folic Acid Fortification in the United States
Before a 1996 US regulation requiring fortification of enriched cereal-grain products with folic acid, 3 economic evaluations projected net economic benefits or cost savings of folic acid fortification resulting from the prevention of pregnancies affected by a neural tube defect. Because the observed decline in neural tube defect rates is greater than was forecast before fortification, the economic gains are correspondingly larger. Applying both …
Phantom of the area
OBJECTIVES: The purpose of the study was to conduct a national multivariate analysis on poverty-area residence and mortality in the United States. METHODS: Proportional hazards analyses were performed of the effect of poverty-area residence on the risk of mortality among adult examinees in the 1971 through 1974 National Health and Nutrition Examination Survey who were followed through 1987. RESULTS: Poverty-area residence was associated with sign…

Folic acid fortification of grain
OBJECTIVES. The purpose of this study was to compare the economic costs and benefits of fortifying grain with folic acid to prevent neural tube defects. METHODS. A cost-benefit analysis based on the US population, using the human capital approach to estimate the costs associated with preventable neural tube defects, was conducted. RESULTS. Under a range of assumptions about discount rates, baseline folate intake, the effectiveness of folate in pr…
The effects of occupational class transitions on hypertension
OBJECTIVES. This study was undertaken to examine how the interaction between occupational class transitions and race affects the incidence of hypertension. METHODS. A cohort of 1982 men (183 Black), ages 25 to 55, received a baseline medical exam between 1971 and 1975 and a follow-up between 1982 and 1984. Logistic regressions were estimated for hypertension at follow-up controlling for hypertension at baseline, other risk factors associated with…
Double Jeopardy
The purpose of this paper is to examine the hypothesis that marital and poverty status interact in their effects on mortality risks beyond their main effects. This study examines the epidemiological bases for applying an additive rather than a multiplicative specification when testing for interaction between two discrete risk factors. We specifically predict that risks associated with being nonmarried and with being poor .interact to produce mort…
