Cost-effectiveness of in-kind nutritional support for impoverished persons with tuberculosis to reduce mortality and disengagement from care in India
BACKGROUND: Undernutrition is a leading risk factor for tuberculosis (TB) mortality and poor treatment outcomes. We evaluated the cost-effectiveness of providing in-kind nutritional support to impoverished persons with TB (PWTB) in India. METHODS: We developed a Markov model comparing standard care with a household-level nutritional food-basket intervention. Model parameters were informed from systematic reviews and a recent cluster-randomised st…
TB stigma in India
In India, persons with tuberculosis (PWTB) and their households experience significant disease-related stigma. The objective of this narrative review was to conduct a review of existing literature related to the types of stigma experienced by PWTB and their household members, with a focus on the effects of stigma, possible interventions, and gender differences. A literature search was conducted on PubMed, EMBASE, and Web of Science using key sear…
The feasibility of respondent-driven sampling with people who use drugs in rural Western Cape, South Africa
The Western Cape is South Africa’s epicentre for tuberculosis (TB) and smoked drug use such as methamphetamine and methaqualone (Mandrax). Despite this, there are limited studies on people who smoke drugs (PWSD) with TB disease in South Africa, partly due to recruitment challenges. Respondent-driven sampling (RDS) is a network-based sampling method used to recruit such key populations. The aim of this qualitative study is to explore the appropria…
Comparative effectiveness of adding delamanid to a multidrug-resistant tuberculosis regimen comprised of three drugs likely to be effective
Clarity about the role of delamanid in longer regimens for multidrug-resistant TB is needed after discordant Phase IIb and Phase III randomized controlled trial results. The Phase IIb trial found that the addition of delamanid to a background regimen hastened culture conversion; the results of the Phase III trial were equivocal. We evaluated the effect of adding delamanid for 24 weeks to three-drug MDR/RR-TB regimens on two- and six-month culture…
Attrition from Care Among Men Initiating ART in Male-Only Clinics Compared with Men in General Primary Healthcare Clinics in Khayelitsha, South Africa
Men have higher rates of attrition from antiretroviral therapy (ART) programs than women. In Khayelitsha, a high HIV prevalence area in South Africa, two public sector primary healthcare clinics offer services, including HIV testing and treatment, exclusively to men. We compared attrition from ART care among men initiating ART at these clinics with male attrition in six general primary healthcare clinics in Khayelitsha. We described baseline char…
People listen more to what actors say’
India has made substantial advancements in reducing the burden of tuberculosis (TB), but persons living with active TB (PLWATB) still face myriad challenges in seeking and receiving care, including TB-related stigma. To meet the END TB targets, it is critical that PLWATB engage in care and are able to adhere to treatment. This qualitative study aimed to understand TB-related stigma (perceived, enacted, and internalised) and possible interventions…
Alcohol and Tobacco Use in a Tuberculosis Treatment Cohort during South Africa’s Covid-19 Sales Bans
From these observations, we hypothesize that policies restricting alcohol and tobacco availability seem to enable some individuals to reduce their consumption. However, these appear to have little effect on the volume of AA consumed among individuals with more harmful patterns of drinking in the absence of additional behavior change interventions
Adolescent Experiences of Violence and Relation to Violence Perpetration beyond Young Adulthood among an Urban Sample of Black and African American Males
Predictors of Active Injection Drug Use in a Cohort of Patients Infected With Hepatitis C Virus
Objectives. We investigated potential risk factors for active injection drug use (IDU) in an inner-city cohort of patients infected with hepatitis C virus (HCV). Methods. We used log-binomial regression to identify factors independently associated with active IDU during the first 3 years of follow-up for the 289 participants who reported ever having injected drugs at baseline. Results. Overall, 142 (49.1%) of the 289 participants reported active …

Recent cigarette smoking and HIV disease progression
The association between smoking and HIV disease progression has been examined in several studies; however, findings have been inconsistent. We examined the effect of recent cigarette smoking on CD4(+) T cell count/μl (CD4 count) and HIV RNA concentration (HIV viral load (VL)) among two HIV-infected cohorts with alcohol problems in Massachusetts in the periods 1997-2001 and 2001-2006 using a prospective cohort design and linear mixed models. Smoki…
Use of a Population-Based Survey to Describe the Health of Boston Public Housing Residents
Objectives. We compared the health of public housing residents with other Boston residents through a random-digit-dial survey. Methods. We used data from the Boston Behavioral Risk Factor Surveillance System collected in 2001 and 2003 to make crude and demographically adjusted comparisons between public housing residents and other city residents on measures of health status, access and utilization, and health behaviors. Results. Public housing re…

Seroconversion to human immunodeficiency virus in prison inmates
We evaluated the prevalence and incidence of human immunodeficiency virus (HIV) infection in 3,837 inmates of a state prison system. Ninety-two (2.4 percent) were HIV-seropositive. The highest proportion of HIV-seropositive inmates was found among Blacks (5.4 percent), females, and those 30 years of age or older. HIV-seropositivity of entering inmates was also 2.4 percent and was unchanged over the three years of the study. Seroconversion occurre…
Increased suppressor T cells in probable transmitters of human immunodeficiency virus infection
To evaluate behavioral and immunologic factors related to transmission of human immunodeficiency virus (HIV) by homosexual intercourse, we studied a population of 329 homosexual/bisexual men (155 partner-pairs) seen in a community health center and medical outpatient clinic. Logistic regression analysis showed that behavioral risk factors for infection in the 130 HIV-infected men included: receptive anal intercourse (OR 4.6, 95% CI-1.8, 12.1); re…
