Closing the equity gap? An overview of progress toward implementing South Africa’s national integrated early childhood development policy
Early childhood development (ECD) intervention outcomes will be optimal when policies and interventions are multi-sectoral, integrated and responsive to the multiple needs of young children and their families. South Africa’s inaugural 2015 National Integrated Early Childhood Development Policy commits the state to provide publicly available, essential services for young children and their families and foregrounds the needs of vulnerable children.…
Belonging
Health care systems are social institutions simulating microcosms of wider societies where unequal distribution of power and resources translate into inequities in health outcomes, experiences and access to services. Growing research on participatory women’s groups positively highlights the influence of group-based care on health and wellbeing for women, their infants, families and wider communities across different countries. With similarities i…
Priority interventions for reducing stunting in the complementary feeding age group in South Africa
In South Africa, approximately one in four children under the age of five is stunted (NDoH, Stats SA, SAMRC & ICF, 2019) – a rate that has remained persistently high. The enduring social and economic impact of stunting, coupled with rising food prices, underscores the need for interventions that are both effective and affordable. This review synthesises global evidence on strategies for children aged 6–24 months and evaluates their suitability wi…
Stunting in South Africa – How do we shift the needle
1. Stunting remains one of the most persistent public health challenges facing children in South Africa. The prevalence of stunting among children under 5 years in 2021–3 was about 29% – a figure t
Depressive symptoms and suicidal ideation associated with women’s experience of recent economic intimate partner violence in Gauteng, South Africa
This study investigated the prevalence of economic intimate partner violence (IPV) and its association with mental health outcomes among women in Gauteng Province, South Africa. Economic IPV, involving controlling behaviours related to employment, earnings, housing, and financial support, is a critical yet understudied form of abuse with potential impacts on women’s mental health. A secondary analysis was conducted using data from a representativ…
Love, You Need to Give Your Child Love”
Background: Nurturing care of young children is aimed at promoting lifelong, intergenerational health and well‐being, as well as social and economic benefits. This study is aimed at qualitatively exploring maternal perceptions related to nurturing care, their access to information and support for caregiving, the home and community environments and practices, and how caregivers promote infants’ health and well‐being in Soweto, South Africa. Method…
“This can certainly work…”
Introduction: Caregiver training is a key component of rehabilitation for children with complex lifelong disabilities such as cerebral palsy. However critical shortages of therapists in low- and middle-income countries like Malawi, reduce access to therapy. Introducing expert caregivers to assist with the provision of basic training on the condition for fellow caregivers offers a potential solution. However, there is a paucity of evidence regardi…
Maternal perceptions of infant's body weight and childhood obesity in South Africa
From a socio‐anthropological study focusing on maternal body weight perceptions and dietary practices towards infants living in Soweto (South Africa), we studied how lay sociocultural traits may lead to early childhood obesity. Most mothers tended to socially value and normalize fatness. This propensity led mothers, particularly older women at home, to adopt high‐calorie feeding practices towards infants, although some mothers tended to question …
Measurement for Change
In this review paper, we explore how on-the-ground Early Childhood Development (ECD) innovators are using monitoring, evaluation, and learning (MEL) systems to guide the design and implementation of ECD programs, as well as how MEL systems can influence policy and support the achievement of impact at scale. We reflect on articles in the Frontiers series “ Effective delivery of integrated interventions in early childhood: innovations in evidence u…
An Opportunity During Antenatal Services to Strengthen Nurturing Care
Introduction: The Healthy Pregnancy, Healthy Baby study (HPHB) augments a routine service (pregnancy ultrasound) with information about fetal and infant development and the importance of parent wellbeing and infant care, to assess whether it will improve child development and growth, parent-infant attachment, parental wellbeing and routine clinic attendance. This paper outlines the process of intervention development and implementation in a compl…
Father involvement, maternal depression and child nutritional outcomes in Soweto, South Africa
Father involvement in South Africa is low, despite evidence that it can improve maternal and child health and wellbeing. Within a larger randomised controlled trial, we assessed whether father involvement during and after pregnancy increased birth weight and exclusive breastfeeding through improved maternal mental health. At 6-week postnatal, mothers completed questionnaires on birth, feeding practices, social support, father involvement and post…
Maternal perspectives on infant feeding practices in Soweto, South Africa
Objective: To (i) describe the infant feeding practices of South African women living in Soweto and (ii) understand from the mothers’ perspective what influences feeding practices. Design: Semi-structured focus group discussions (FGD) and in-depth interviews (IDI) were conducted, and data were analysed using thematic analysis. Setting: Soweto, South Africa. Participants: Nineteen mothers were stratified into three FGD according to their baby’s ag…
The Aspirations of Measurement for Change
This Perspective presents the five key aspirations of an approach to data use, decision making and monitoring, evaluation, and learning (MEL) in Early Childhood Development (ECD) referred to as Measurement for Change. The core ideas of Measurement for Change gave rise to this series, and many of the papers submitted in this series speak to this approach, whether directly or indirectly. The five aspirations describe interconnected concepts that ad…
Measurement for Change
Measurement for Change proposes an integration of monitoring, evaluation, and learning into decision-making systems that support sustainable transition of interventions to scale. It was developed using a cyclical, interactive 1-year dialogue between early childhood development (ECD) practitioners and academics from across the globe. Details are presented in Krapels et al. ( 1 ) as part of this special issue in Frontiers. In this paper, we trace t…
Health system factors affecting implementation of integrated management of childhood illness (IMCI)
The Integrated Management of Childhood Illness (IMCI) strategy has been adopted by 102 countries including South Africa, as the preferred primary health care (PHC) delivery strategy for sick children under 5 years. Despite substantial investment to support IMCI in South Africa, its delivery remains sub-optimal, with varied implementation in different settings. There is scarce research globally, and in the local context, examining the effects of h…
