Supporting multimorbidity self -care through integration, learning and eHealth in Ireland (SMILE)- Preliminary results
Background: Approximately .3 million people in Ireland have one of the 4 main chronic diseases (CVD, Diabetes Type 2, COPD and Asthma) with increasing numbers of patients living with multimorbidity. It is estimated that between % and 5% of patients will require a case management service.People with chronic disease have frequent visits and often present late to the Emergency Department (ED), the Acute Medical Assessment Unit (AMAU) and to both GP …
Making integrated care a reality
Background: Due to an ageing population, the prevalence of chronic disease (CD) multimorbidity is rising in Ireland and internationally. In Ireland, CD care was traditionally characterised by siloed, reactive care, culminating in repeated hospital admissions. This was neither patient-centred nor sustainable. Health services needed to evolve to meet changing population need. Approach: The Integrated Model of Care for the Prevention Management of C…
Making interdisciplinary working a reality
The implementation of the Integrated Model of Care for the Prevention & Management of Chronic Disease (MoC) in Ireland aims to deliver patient centred care and is being implemented at scale. This workshop aims to present examples of implementation of this change in Ireland and how we are supporting the implementation of the MoC and ensuring fidelity. We will also demonstrate specific local examples of interdisciplinary working. Background: An int…
Irish General Practice Chronic Disease Prevention and Management Programme
Background: Chronic disease patients in Ireland had relied on hospital Out Patient Departments to provide care as General Practitioners (GPs) were not paid for chronic disease management, there were no incentives for early detection and prevention. This Programme incentivises General Practitioners for scheduled visits, with specified care which is evidence based. Approach: The programme commenced in 2020, there are 3 elements to the programme; Op…
The Integrated Care Programme for the Prevention and Management of Chronic Disease
The Integrated Model of Care for the Prevention & Management of Chronic Disease (MoC) provides a population health approach to the prevention & management of chronic disease (CD). This model is currently being rolled out across Ireland at scale. This workshop aims to explore the Irish experience of implementing integrated care as part of a population health approach to the prevention and management of CD. Our current, hospital-centric method of d…
Can cases and outbreaks of norovirus in children provide an early warning of seasonal norovirus infection
These findings suggest that monitoring cases and outbreaks of norovirus in children could provide an early warning of seasonal norovirus infection. However, both school outbreak data and syndromic surveillance data are not norovirus specific and will also capture other causes of IID. The use of school outbreak data as an early warning indicator may be improved by enhancing sampling in community outbreaks to confirm the causative organism
Family income and exposure to norovirus in childhood
Children from low income families are at greater risk of poorer health outcomes than their wealthier peers. Hospital admissions for children with gastroenteritis increase as deprivation increases. Noroviruses are responsible for 47-96% of outbreaks of acute paediatric gastroenteritis, and 5-36% of sporadic cases worldwide. However, evidence on the relationship between family income and childhood exposure to norovirus is still limited, with publis…
How timely closure can reduce outbreak duration
Care homes that closed promptly had outbreaks of shorter duration. Care home providers, and those advising them on infection control, should aim to close homes quickly to prevent lengthy disruption to services
The second study of infectious intestinal disease (IID2)
Elderly populations have a high propensity for recurrent IID. More detailed studies are needed to identify vulnerable subgroups and susceptibility factors, and inform adequate control policies among the elderly
Is there a need to include HIV, HBV and HCV viruses in the Saudi premarital screening program on the basis of their prevalence and transmission risk factors
BACKGROUND: In January 2008, the Saudi Arabian health authority included mandatory testing for HIV, HBV and HCV viruses in the premarital screening program. Epidemiologically, there were few justifications for their inclusion as disease prevalences and distributions are poorly understood in the population. This study aims to provide information about HBV, HCV and HIV prevalences and risk factors for disease transmission and so produce evidence fo…
