Case management for the mentally ill
This paper reports on client satisfaction with community mental health services. A Client Satisfaction Interview was used as one outcome measure in a randomized controlled study of clinical case management vs. standard community care. The Interview had high internal consistency reliability (Cronbach's alpha = 0.89) with very high inter-rater reliability for overall scores (r= 0.99) and good inter-rater reliability for individual items (kappa over…
Evaluating the Use of a Psychiatric Intensive Care Unit
Objective: This paper presents a descriptive study, undertaken in 1993, of a Psychiatric Intensive Care Unit (PICU) serving a deprived inner-city area, investigating the role of ethnicity as a risk factor for admission to the unit. Methods: Clinical and demographic data were collected on consecutive admission to a PICU. Global Assessment of Function Scale scores were rated on admission and at discharge from the unit. Results: The majority of pati…

Subjective Quality of Life, Psychopathology, Satisfaction With Care and Insight
We have investigated the determinants of global subjective quality of life (GSQOL) using data from a controlled trial of intensive case management for the severely mentally ill. In a multiple regression analysis depression, age, objective quality of life and domain-specific quality of life together explained 59% of the variance in GSQOL. GSQOL was not significantly correlated with measures of positive psychotic symptomatology, insight or attitude…
The New Long Stay in an Inner City Service
We assessed the long-stay inpatients (length of stay >6 months) from a deprived inner-city catchment area with a population of 210000 in 1993 and 1995 on a variety of measures, following up both cohorts after 24 months. Total numbers of long-stay inpatients were reduced from 56 (26.7 per 100,000 total population) to 35 (16.7 per 100,000) between 1993 and 1995, in line with the closure of dedicated long-stay beds. The 1995 cohort were more symptom…
The Camberwell Assessment of Need
BACKGROUND: People with severe mental illness often have a complex mixture of clinical and social needs. The Camberwell Assessment of Need (CAN) is a new instrument which has been designed to provide a comprehensive assessment of these needs. There are two versions of the instrument: the clinical version has been designed to be used by staff to plan patients' care; whereas the research version is primarily a mental health service evaluation tool.…
'Not Waving But Drowning'
This paper presents the results of the latest in a series of surveys of an inner-city public psychiatric inpatient service. Severe pressures on the service had led to the purchase of inpatient beds in the private hospital sector and intensive care facilities in a large psychiatric hospital. On a census day the nominal occupancy for the inpatient unit was 117%. Implications of the survey, trends over time and possible solutions to the difficulties…

Case Management for the Mentally Ill
Case management is now accepted in the United States as a key element in systems designed to support elderly, mentally handicapped and chronically mentally ill people in the community (Intagliata, 1982). The concept of case management (CM) has recently been enshrined in British Government policy for community care (HMSO, 1989a). The
A Two Year Follow Up of New Long-Stay Patients in an Inner City District General Hospital
In 1967 Magnus described 'the new chronics', patients admitted to a psychiatric hospital who then stayed for more than one year. Since that time many studies have examined this patient group but the majority have focussed on psychiatric hospital populations. There is some doubt, however, as to whether the characteristics of new- long stay patients in District General Hospital (DGH) psychiatric in-patient units are comparable. The current paper de…
Psychiatric Day Care
The literature on consumer opinion about psychiatric day care is reviewed and a study of the views of day care attenders living in an inner city catchment area who are in prolonged contact with psychiatric services is presented. Attenders were broadly satisfied with their care. Analysis of their verbatim comments suggested that attenders perceived the value of attendance at a day unit in social rather than therapeutic terms. The implications of t…
