Identifying and evaluating best practices in the social and health care systems of Catalonia have been a challenge in the last years. It implies defining what integrated care models are, what these models look like, understand their dynamics, needs, commonalities and contexts. Integration of social and health HCSs resulted in a shift from institutionalization services to primary care services and day care centers, prolonging recipients' lives in their homes
Integrated Health and Social Home Care Services in Catalonia
Our study showed that the degree of implementation of integrated health and social HCS offered by the Government of Catalonia was perceived as low. The identified barriers and facilitators can be used to facilitate such implementation. Further studies should include professionals other than social workers in PHC assessments
