Applicability of the adjusted morbidity groups algorithm for healthcare programming
The flexibility of the AMG, together with its ability to support policymakers and clinical sector, could favour its implementation in different scenarios across Europe. A clear strategy for the adoption of HRA tools and related key elements and lessons learnt for a successful transferability at the EU level were defined. HRA strategies should be considered a pillar of healthcare policies and programming to achieve person-centred care and promote …
Toward Adoption of Health Risk Assessment in Population-Based and Clinical Scenarios
Successful adoption of the AMG algorithm was achieved in the two sites confirming transferability. Marche identified the key requirements for a population-based HRA strategy, whereas Viljandi Hospital proved its potential for clinical use paving the way toward value-based healthcare strategies
In the era of remote access to the healthcare system, the accurate measures of patient’s health complexity is a mainstay for a person-centred service allocation
The International Journal of Integrated Care (IJIC) is an online, open-access, peer-reviewed scientific journal that publishes original articles in the field of integrated care on a continuous basis.IJIC has an Impact Factor of 2.913 (2021 JCR, received in June 2022)The IJIC 20th Anniversary Issue was published in 2021
Performance of quantitative measures of multimorbidity
The addition of multimorbidity measures to models for explaining healthcare outcomes increase the performance. The GMA index has high performance in explaining relevant healthcare outcomes and may be useful for clinical practice, resource planning, and public health research

Health risk assessment and stratification in an integrated care scenario
Background: Health risk assessment and stratification have proven highly relevant for large-scale implementation of integrated care by facilitating services design and case identification.Aim: The principal objective was to analyse five health-risk assessment strategies used in the five regions participating in the Advancing Care Coordination and TeleHealth Deployment (ACT) program (www.act-programme.eu): Scotland (UK), Basque Country (ES), Catal…
