Health impacts of extreme heat in medically at-risk populations
Our findings indicated that chronic diseases are associated with higher vulnerability to adverse health effects due to extreme heat. We observed notable variations across geographic, demographic, and socioeconomic subgroups, emphasizing the need for targeted public health strategies
Pneumococcal vaccination coverage and adherence to recommended dosing schedules in adults
Pneumococcal vaccine coverage is slowly increasing in Flanders, displaying seasonal peaks in sync with influenza vaccination campaigns. However, with less than one-fourth of the target population vaccinated, less than 60% high-risk and approximately 74% of 50 + with comorbidities and 65+ healthy persons with an adherent schedule, there is still much room for improvement. Furthermore, adults with poor socioeconomic status had lower odds of primary…
The Adem project
COPD is a chronic, progressive disease, for which a multidisciplinary approach is crucial. Appropriate follow-up is crucial to improve the quality of life of COPD-patients and reduce associated healthcare costs. A ‘Good medicines use conversation’ COPD is a pharmaceutical care intervention, conducted by the community pharmacist, consisting of a tailored-made education of the COPD-patient on the disease and its treatment. Scientific research (PHAR…
Data driven population health management
In 2018 we started the Leuven Cares project in the city of Leuven. The aim of this project is to develop, implement and evaluate the necessary changes to enable integrated care. We installed a proactive approach to manage the health and wellbeing of the population of Leuven, to move away from the provision of reactive, demand-led care. All inhabitants of Leuven are targeted, 102000 people. All relevant healthcare organizations participate in the …
Implementation of integrated care in Belgium
Belgium has chosen for a national movement towards integrated care. The Belgian Health Care Reform “Integrated Care for a better health” (Integreo.be) was approved by all responsible ministers in 2015. This plan is based on the principles of quintuple Aim. The core strategy of the plan is to drive change in a guided bottom-up approach, with large-scale locoregional piloting projects. Over the past 5 years, professional caregivers and care organiz…
The integration movement by a central stimulation team or integrator
The “Leuven Cares” (Zorgzaam Leuven) project started in 2018 as a government supported change management project, focusing chronic care integration. It covers and uses the officially defined primary care zone of Leuven (102.000 inhabitants). The project Zorgzaam Leuven is created thanks to the funding of the federal government Integreo.be, and the contribution of 60 local organizations in Leuven. As a basic methodology Leuven Cares opts for an ‘i…
The Effects of Heatwaves on Human Morbidity in Primary Care Settings
Heatwaves are associated with increased heat-related morbidities and decreased respiratory infection risk. The study of heatwaves' effects in primary care settings helps evaluate the impact of heatwaves on the general population. Primary care settings might be not suitable to study acute life-threatening morbidities

Impact of Heat Waves on Hospitalisation and Mortality in Nursing Homes
Climate change leads to more days with extremely hot temperatures. Previous analyses of heat waves have documented a short-term rise in mortality. The results on the relationship between high temperatures and hospitalisations, especially in vulnerable patients admitted to nursing homes, are inconsistent. The objective of this research was to examine the discrepancy between heat-related mortality and morbidity in nursing homes. A time-stratified c…
