The identification of these success factors might inspire HCPs, providers of interprofessional education and policymakers to facilitate interprofessional pharmaceutical care
Beliefs about medication after hospital discharge in patients with polypharmacy
Introduction: Negative beliefs about medication can pose a threat to treatment adherence, which in turn can jeopardize drug effectiveness and beneficial health outcomes. In order to support and empower patients in safe medication use at home, it is important to assess patients’ preferences and beliefs affecting patients’ motivation to start and continue with treatment. Identification and description of successful initiatives between physicians, pharmacists and nurses, their context and implementation strategies are important to close this know-do gap. Objective: The aim of this …
Self-Management of Medication on a Cardiology Ward
An intervention, SelfMED, was introduced to facilitate patient self-management of medication during hospitalization. This study aimed to evaluate the feasibility and safety of the SelfMED intervention. All patients in a cardiology ward in a Belgian regional hospital were assessed for suitability for inclusion, applying an evidence-based stepped assessment tool. Patients eligible for medication self-management and willing to participate were inclu…

According to the participants, medication self-management offered many benefits, including the implementation of more structure for the patient, an ameliorated preparatory phase towards discharge, and an actual improvement of future adherence. All participants considered medication self-management to contribute to more profound medication knowledge and an…
The Nuphac-EU Framework for Nurses’ Role in Interprofessional Pharmaceutical Care

Currently, it is unclear to what extent healthcare professionals consider pharmaceutical care (PC) activities to be nurses' responsibility in order to obtain best care quality. This study aimed to create and evaluate a framework describing potential nursing tasks in PC and to investigate nurses' level of responsibility. A framework of PC tasks and contextual factors w…
The prevalence of and factors related to alcohol consumption among young people in Thailand
The prevalence of alcohol consumption during early adolescence
This cross-sectional study identified and described the prevalence of alcohol consumption among early adolescents (10–14 years). The participants were 9,509 early adolescents from Chonburi province, Thailand. Of all the adolescents, 31.01% had experience with alcohol drinking, while the current drinkers were 10.94%. Girls were more likely to drink and the amount of current drinkers increased with age (p < 0.001). Adolescents living with divorced …
Do telemonitoring projects of heart failure fit the Chronic Care Model
Limited financial resources hampered continuation after the pilot phase. Cooperation and coordination in telemonitoring appears to be major barriers but are, within primary care as well as between the lines of care, important links in follow-up. This discrepancy can be prohibitive for deployment of good chronic care. Chronic Care Model is recommended as basis for future
