Bringing the value equation into play in value-based healthcare
Introduction: Value-based healthcare (VBHC) proposes a framework for managing healthcare systems, connecting health and economic outcomes to determine the value of healthcare. The value equation remains ambiguous, serving more as a theoretical framework than a practical decision-making tool. The key challenge lies in estimating and interpreting the value equation. The purpose of this study is to provide a methodological proof-of-concept to addres…
Probabilistic cost-utility analysis and expected value of perfect information for the Oncotype multigenic test
To carry out a cost-utility analysis of the application of the Oncotype genomic test to inform the decision to use or not to use chemotherapy in the Basque Country (Spain). The cost-utility study was carried out using a discrete event simulation model representing the natural history of breast cancer. The decision of treatment with chemotherapy based on Oncotype was compared with the standard of treatment based on clinical-pathological criteria. …
Application of two-part models and Cholesky decomposition to incorporate covariate-adjusted utilities in probabilistic cost-effectiveness models
This study aimed to explain a joint statistical procedure (two-part models and Cholesky decomposition) to incorporate second-order uncertainty from covariate adjusted mean utility functions in probabilistic cost-effectiveness models. First, two-part models were applied to obtain parameters for the utility function. Second, a new set of correlated parameters for each simulation was obtained by Cholesky decomposition. The procedure was applied to E…

Evaluation of the colorectal cancer screening Programme in the Basque Country (Spain) and its effectiveness based on the Miscan-colon model
The Basque Country CRC Programme results are aligned to its strategy and comparable to other programmes. MISCAN model was found to be a useful tool to predict the benefits of the programme in the future. The effectiveness of the Programme has not been formally established as case control studies are required to determine long term benefits from the screening strategy
Modeling approach to managing the deployment of integrated organizational models for multi-morbid patients in the CareWell study. The Basque Country case
Introduction: An integrated care approach supported by information and communication technologies (ICT) is being deployed in the Basque Country within the European CareWell project, to best respond to the complex needs of multi-morbid patients. Our statistical analysis carried out in Donostialdea County has demonstrated that the profile of resource consumption has not shown the expected changes. The fact that no significant differences were found…
