Trends in Opioid Agonist Treatment for Opioid Dependence During Pregnancy: A Population‐Based Study in New South Wales 2005–2021
Non-adherence, discontinuation, and seroconversion among people on daily HIV pre-exposure prophylaxis (PrEP) in Brazil: A nationwide cohort study
The goal of this study was to evaluate characteristics associated with Pre-exposure Prophylaxis for HIV infection (PrEP) non-adherence or discontinuation in Brazil and assess the association between these outcomes and HIV seroconversion. We used linked national dispensing and pathology data to identify people aged 14+ years initiating PrEP in 2018. We estimated non-adherence using the proportion of days covered (PDC), defining non-adherence as PD…
I would be very proud to be part of an initiative that didn’t exclude people because it was hard”: Mapping and contextualising health equity responsibilities and decision-making tensions in the implem…
The distribution of heath equity responsibilities, as we demonstrated through a ToC of a decentralised, place-based reform, present risk to health equity prioritisation. Risks were particularly present when local resourcing and capacity were stretched, and limited policy guardrails were in place to counteract decision-making tensions, such as clear health equity accountabilities, responsibilities, and actions
Trends in cause-specific mortality among people with hepatitis C virus in New South Wales, Australia
During the HCV elimination era, both liver-related and drug-related mortality have declined, suggesting direct and indirect impacts of DAA therapy, respectively. Expanded opioid agonist therapy coverage and enhanced alcohol use disorder management could further reduce mortality
Trends in prescription opioid analgesic use in Australia from 2015 to 2022
Prescribed opioid analgesic use declined between 2015 and 2022 because of reductions in PBS dispensing claims. A quarter of the reduction in PBS dispensing claims was offset by use outside the PBS. Our findings indicate a significant increase in private use, reasons for which may include accessing opioids not PBS-subsidised and circumventing PBS restrictions for PBS-subsidised opioids. Comparing multiple data sources provides a comprehensive acco…

Assessing the impact of implementing multiple adherence measures to antiretroviral therapy from dispensing data: A short report
Pharmacy dispensing data are useful for estimating adherence to therapy. Here, we implement multiple adherence measures to antiretroviral therapy (ART) and provide an online tool for visualising results. We conducted a cohort study for 2,042 people dispensed ART in Australia. We assessed adherence using the Proportion of Days Covered (PDC) within 360 days of follow-up as a continuous measure and dichotomised (PDC ≥80%). We defined a covered day a…
In-hospital outcomes by insurance type among patients undergoing percutaneous coronary interventions for acute myocardial infarction in New South Wales public hospitals
Our findings suggest patients undergoing PCI in Australian public hospitals with private health insurance experience lower in-hospital mortality compared with their publicly insured counterparts, but in-hospital complications are not related to patient health insurance status. Our findings are likely due to unmeasured confounding of broader patient selection, socioeconomic differences and pathways of care (e.g. access to emergency and ambulatory …

Feasibility and validity of using death surveillance data and SmartVA for fact and cause of death in clinical trials in rural China: A substudy of the China salt substitute and stroke study (SSaSS)
BACKGROUND: In rural China, mortality surveillance data may be an alternative to primary data collection in clinical trials; SmartVA (verbal autopsy) is also a potential alternative for endpoint adjudication. The feasibility and validity of both need to be assessed. METHODS: We used mortality data from the first 24 months of the China Salt Substitute and Stroke Study (SSaSS) trial and assessed the agreement between (1) mortality surveillance data…

Dispensing Practices of Fixed Dose Combination Controller Therapy for Asthma in Australian Children and Adolescents
The Australian Asthma Handbook does not recommend use of fixed dose combination (FDC) controller medicines for asthma in children aged ≤5 years. FDCs are only recommended in children and adolescents (aged 6-18 years) not responding to initial inhaled corticosteroid (ICS) therapy. Using Pharmaceutical Benefits Scheme dispensing claims from 2013-2018, we examined the annual incident FDC dispensing and the incident FDC dispensing without prior ICS u…
Health-related quality of life among HIV-infected patients initiating treatment in Brazil in the single-tablet regimen era
Health-related quality of life (HRQoL) is a multidimensional concept involving an individual's self-perception about how a disease or treatment impacts their daily life. In this study, we evaluated the HRQoL and factors associated with this outcome in 366 patients initiating combination Antiretroviral Therapy (cART) in Belo Horizonte, Brazil. We measured HRQoL using the EuroQoL-5D 3 level (EQ-5D) and the HIV instrument of the World Health Organiza…
Reflections on American Progressivism
"In American politics, at least since the Civil War, the great philosophical divide is between "progressives" and "founders" of the American regime. The quarrel has come to be defined in the media as a contest between liberals and conservatives. This book explores the ideological underpinnings of American progressivism. In doing so, it examines the foundations of modern liberalism and conservatism. The fundamental problem of any science of politi…
Progressive Democracy
The North Star of progressive political science always pointed toward more democracy. The theoretical problem was to define precisely what was meant by democracy that made it "progressive" and thereby separated "progressive democracy" from the popular government vocabulary of the founders. The practical problem was that the broad character of progressive democracy that gradually emerged was patterned on a European style welfare state system. Comp…
Presidential Leadership: The Political Relations of Congress and the Chief Executive
"The nature of the presidency is an issue that has been debated since the drafting of the United States Constitution. The Federalists felt a strong executive was the backbone and prime mover of a strong government. On the other side, the Anti-Federalists felt the presidency represented monarchical tendencies and could potentially subvert republican government. How does executive leadership fit in with a limited government with enumerated powers? …
Progressive Democracy
Croly explains the requirements for a genuinely popular system of representative government providing progressive liberalism with both a philosophical critique of the founding fathers' political outlook, and a political strategy for replacing it with something more in keeping with a new epoch. Although it was written in 1914, the intellectual structure remains largely intact within the liberal-progressive tradition

High risk prescribing in older adults: Prevalence, clinical and economic implications and potential for intervention at the population level
The availability of large-scale data is useful to identify opportunities for improving prescribing, and health in older adults. The size of the 45 and Up Study, along with linkage to health databases provides an important opportunity to investigate the relationship between high risk prescribing and adverse outcomes in a real-world population of older adults
It Is Tough to Be the Second Toughest Guy in a Tough Town: Ask the Man Who Shot Liberty Valance
The eternal conflict between justice and violence is the theme of director John Ford's last great film, The Man Who Shot Liberty Valance. In the world of the American West where Ford's story is set, justice does not just happen; it is a work of manly courage that encompasses a willingness—in extreme cases—to kill those men, such as Liberty Valance, who challenge law and order. Justice will require, as Plato said, a rightly ordered soul, but it wi…
