  • Chronic Pain and Opioids in the Elderly

Open Access•Manuel Glauco Carbone, Icro Maremmani et al.•ARTICLE•International Journal of…•2026

Pain management in older adults may benefit from moving beyond symptom-focused approaches toward a neuropsychiatric model of care aimed at preserving homeostatic balance across sensory, emotional, and motivational domains. Within this framework, opioid therapy can be conceptualised as a potential means of functional and neuroaffective restoration, rather than solely as a strategy for risk reduction

  • Stimulant Treatment Gap in ADHD Patients with Heroin Use Disorder

Open Access•Alessandro Pallucchini, Maurizio Varese et al.•ARTICLE•International Journal of…•2025

Although limited by its cross-sectional nature and setting-related confounders, the study indicates that OAT alone may not be sufficient to manage neurodevelopmental symptoms in ADHD+HUD patients. Further research is necessary to assess the safety and efficacy of integrated stimulant-based treatments, ideally within dual disorder services combining psychiatric and addiction expertise

  • World addiction medicine reports

Open Access•Hamed Ekhtiari, Arash Khojasteh Zonoozi et al.•ARTICLE•Frontiers in Psychiatry•2024

Addiction medicine is a dynamic field that encompasses clinical practice and research in the context of societal, economic, and cultural factors at the local, national, regional, and global levels. This field has evolved profoundly during the past decades in terms of scopes and activities with the contribution of addiction medicine scientists and professionals globally. The dynamic nature of drug addiction at the global level has resulted in a cr…

  • Chronic Cocaine Use and Parkinson’s Disease

Open Access•Manuel Glauco Carbone, Icro Maremmani•ARTICLE•International Journal of…•2024

Over the years, the growing "epidemic" spread of cocaine use represents a crucial public health and social problem worldwide. According to the 2023 World Drug Report, 0.4% of the world's population aged 15 to 64 report using cocaine; this number corresponds to approximately 24.6 million cocaine users worldwide and approximately 1 million subjects with cocaine use disorder (CUD). While we specifically know the short-term side effects induced by co…

  • Characteristics of Stress Sensitivity in Heroin Use Disorder Patients during Their Opioid Agonist Treatment

Open Access•Filippo Della Rocca, Angelo Giovanni Icro Maremmani et al.•ARTICLE•International Journal of…•2023

In the present study, performed on a sample of Heroin Use Disorder (HUD) patients undergoing Opioid Agonist Treatment (OAT), we attempted to explore the relationships between stress sensitivity and heroin addiction-related clinical aspects. HUD patients' stress sensitivity was evaluated with the Heroin/PTSD-Spectrum questionnaire (H/PSTD-S). The Drug Addiction History Questionnaire (DAH-Q), the Symptomatological Check List-90 (SCL-90), and The Be…

  • Psychomotor Symptoms in Chronic Cocaine Users

Open Access•Davide Cenci, Manuel Glauco Carbone et al.•ARTICLE•International Journal of…•2022

According to the latest estimates, there are around 24.6 million cocaine users worldwide, and it is estimated that around a quarter of the population worldwide has used cocaine at some point in their lifetime. It follows that such widespread consumption represents a major risk for public health. Long-term use of cocaine, in addition to being related to many cerebral and cardiovascular diseases, is increasingly associated with a higher incidence o…

  • Non-medical Use of Prescription Gabapentinoids (Gabapentin and Pregabalin) in Five European Countries

Open Access•Francina Fonseca, William Lenahan et al.•ARTICLE•Frontiers in Psychiatry•2021

Background: Non-medical use (NMU) of prescription GABA analogs (pregabalin and gabapentin) has been reported especially in opiate dependent persons. However, by now the prevalence of NMU of gabapentinoids in the general population has not been sufficiently evaluated. The aim of this research paper is to determine the prevalence of prescription GABA analog NMU and associated demographics in five European countries with special detail of Spain. Met…

  • Adult-Attention Deficit Hyperactive Disorder Symptoms Seem Not to Influence the Outcome of an Enhanced Agonist Opioid Treatment

Open Access•Angelo Giovanni Icro Maremmani, Pasqualina Rocco et al.•ARTICLE•International Journal of…•2021

The role of opioids and opioid medications in ADHD symptoms is still largely understudied. We tested the hypothesis that, in Heroin Use Disorder (HUD), when patients are treated with Agonist Opioid medications (AOT), treatment outcome is associated with the presence of Adult Attention-Deficit/Hyperactive Disorder (A-ADHD) symptomatology. A retrospective cohort study of 130 HUD patients in Castelfranco Veneto, Italy, covering 30 years, was divided…

  • Symptomatological Variants and Related Clinical Features in Adult Attention Deficit Hyperactive Disorder

Open Access•Alessandro Pallucchini, Marco Carli et al.•ARTICLE•International Journal of…•2021

A large amount of the current literature has focused on the characteristic symptoms of attention deficit hyperactivity disorder (ADHD) in children and adolescents. In contrast, less attention has been devoted to ADHD clinical subtypes in adult patients. We evaluated 164 consecutive adult ADHD (A-ADHD) outpatients using DSM-5 criteria and many specific rating scales and questionnaires. A principal component factor analysis was performed on clinica…

  • Substance Use Disorder in Adult-Attention Deficit Hyperactive Disorder Patients

Open Access•Vincenza Spera, Alessandro Pallucchini et al.•ARTICLE•International Journal of…•2020

At treatment entry, the presence of different comorbid SUD clusters do not affect ADHD-specific symptomatology or severity

  • Opioid-related deaths in Europe

Open Access•Hannu Alho, Maurice Dematteis et al.•ARTICLE•International Journal of Drug…•2020

Use of illicit opioids and misuse of prescription opioids are the main causes of drug-related deaths across the world, and the continuing rise in opioid-related mortality, especially affecting North America, Australia and Europe, is a public health challenge. Strategies that may help to decrease the high levels of opioid-related mortality and morbidity and improve care across Europe include risk assessment and interventions to improve the use of …

  • Remarkable Reduction of Cocaine Use in Dual Disorder (Adult Attention Deficit Hyperactive Disorder/Cocaine Use Disorder) Patients Treated with Medications for ADHD

Open Access•Corrado Manni, Giada Cipollone et al.•ARTICLE•International Journal of…•2019

Background : Cocaine use disorder (CUD) is a growing public health concern, but so far no effective pharmacotherapies have been demonstrated. Stimulant medications have proved to be promising in CUD treatment. The self-medication hypothesis (SMH) can help to explain this phenomenon better, especially in cases where CUD co-occurs with adult attention deficit hyperactivity disorder (A-ADHD). Methods : In the present retrospective study, a sample of…

  • Patterns of Management of Patients With Dual Disorder (Psychosis) in Italy

Open Access•Massimo Clerici, Andrea De Bartolomeis et al.•ARTICLE•Frontiers in Psychiatry•2018

Patients with severe psychotic disorders such as schizophrenia, schizoaffective, and bipolar disorders frequently suffer from concomitant substance use disorders (SUDs)-Dual Disorder (DD) patients. In order to better understand current practices for management of patients with psychotic episodes and concomitant SUD in Italy, we carried out a survey of psychiatrists on current routine practice among prescribers. These aspects can help to identify …

  • Harm Reduction as “Continuum Care” in Alcohol Abuse Disorder

Open Access•Icro Maremmani, Mauro Cibin et al.•ARTICLE•International Journal of…•2015

Alcohol abuse is one of the most important risk factors for health and is a major cause of death and morbidity. Despite this, only about one-tenth of individuals with alcohol abuse disorders receive therapeutic intervention and specific rehabilitation. Among the various dichotomies that limit an effective approach to the problem of alcohol use disorder treatment, one of the most prominent is integrated treatment versus harm reduction. For years, …

  • Six-Month Outcome in Bipolar Spectrum Alcoholics Treated with Acamprosate after Detoxification

Open Access•Angelo Giovanni Icro Maremmani, Angelo Icro Maremmani et al.•ARTICLE•International Journal of…•2014

Bipolarity seems to be correlated with the efficacy of acamprosate treatment in inducing patients to refrain from alcohol use after detoxification (while avoiding relapses) in depressed alcoholics. Placebo-controlled clinical trials are now warranted to check the validity of this hypothesis

  • Gender Differences in the Relationship between Maladaptive Behaviors and Post-Traumatic Stress Disorder. A Study on 900 L’ Aquila 2009 Earthquake Survivors

    Open Access•Liliana Dell’Osso, Liliana eDell'Osso et al.•ARTICLE•Frontiers in Psychiatry•2013

Our results show high rates of maladaptive behaviors among earthquake survivors with PTSD suggesting a greater severity among men. Interestingly, post-traumatic stress symptomatology appears to be a better correlate of these behaviors among women than among men, suggesting the need for further studies based on a gender approach

  • Correlations Between Awareness of Illness (Insight) and History of Addiction in Heroin-Addicted Patients

Open Access•Angelo Giovanni Icro Maremmani, Luca Rovai et al.•ARTICLE•Frontiers in Psychiatry•2012

In a group of 1066 heroin addicts, who were seeking treatment for opioid agonist treatment, we looked for differences in historical, demographic, and clinical characteristics, between patients with different levels of awareness of illness (insight). The results showed that, in the cohort studied, a majority of subjects lacked insight into their heroin-use behavior. Compared with the impaired-insight group, those who possessed insight into their i…

  • Symptomatological Features of Patients with and without Ecstasy Use during Their First Psychotic Episode

Open Access•Fabio Rugani, Silvia Bacciardi et al.•ARTICLE•International Journal of…•2012

psychosis with a high level of aggressiveness and violence constitutes an important 'side-effect' that surely runs counter to the expected entactogenic action of Ecstasy. At a patient psycho-educational level, this study suggests that the use of Ecstasy may be counterproductive with respect to user expectations

  • Can Ropinirole Modulate Reinforcing Subjective Effects of Cocaine in Humans

Open Access•Angelo Giovanni Icro Maremmani, Matteo Pacini et al.•ARTICLE•Frontiers in Psychiatry•2011

In this study we evaluated, by means of the "cocaine rush visual analog scale," the impact of ropinirole on the expected rush induced by cocaine in a group of heroin addicts abusing cocaine; the self-reported reaction to the rush blockade (if any) on cocaine consumption, and the correlations between this self-reported reaction and individual, clinical, and therapeutic parameters. Nineteen cocaine abuser heroin-dependent patients entered the study…

  • Long-Term γ-Hydroxybutyric Acid (GHB) and Disulfiram Combination Therapy in GHB Treatment-Resistant Chronic Alcoholics

Open Access•Angelo Giovanni Icro Maremmani, P Pani et al.•ARTICLE•International Journal of…•2011

Leading Italian studies support the use of γ-hydroxybutyric acid (GHB), not only in the treatment of the alcohol withdrawal syndrome, but also in maintaining alcohol abstinence. GHB gives a better result than naltrexone and disulfiram in maintaining abstinence, and it has a better effect on craving than placebo or disulfiram. The problem is that about 30-40% of alcoholics are non-responders to GHB therapy. In our clinical practice, we speculate t…

  • Differential Substance Abuse Patterns Distribute According To Gender In Heroin Addicts

Icro Maremmani, Stefania Canoniero et al.•ARTICLE•Journal of Psychoactive Drugs•2010

This study attempts to analyse potential gender differences among a group of heroin addicts seeking treatment at a university-based medical centre. The central modality of treatment at this centre is the use of methadone maintenance. Among those patients entering this program there seems to be an emerging pattern of males who tend to use heroin as their opiate of choice, and are more likely to combine it with cannabis, while females are more like…

  • Diagnostic and Symptomatological Features in Chronic Psychotic Patients According to Cannabis Use Status

Icro Maremmani, Antonio Lazzeri et al.•ARTICLE•Journal of Psychoactive Drugs•2004

The prevalence and the clinical meaning of cannabis use in patients with chronic psychosis has not been systematically explored. The authors have compared the diagnostic and symptomatological characteristics of 111 male patients affected by chronic psychosis with and without past or current use of cannabis. Sixty-six patients were still using or had used cannabis; in all cases the use preceded the onset of psychotic symptoms. Forty-three patients…
