The role of oxygen in shelter-based overdose response: A qualitative needs assessment across shelter sites in Toronto, Canada
BACKGROUND: The toxic drug crisis in Canada is increasingly shaped by a complex and unpredictable unregulated drug supply characterized by polysubstance toxicity, including combinations of opioids and sedating agents that can produce prolonged respiratory depression not fully reversed by naloxone alone. Concurrent reductions in harm reduction infrastructure in Ontario, including the closure of supervised consumption services, may be shifting over…
Effect of the Covid-19 pandemic on initiations of opioid agonist therapy in Ontario: A population-based interrupted time series analysis
BACKGROUND: Maintaining access to opioid agonist therapy (OAT) was a key concern at the COVID-19 pandemic onset. Consequently, Ontario introduced guidance for changes to OAT delivery, including relaxed take-home dosing and expanded virtual care. We examined how the pandemic influenced OAT initiation trends. METHODS: We conducted a population-based, retrospective interrupted time series analysis using linked administrative health data from March 2…
Women’s experiences of homelessness and violence during the Covid-19 pandemic in Canada’s largest city: An integrated qualitative analysis of clients of violence against women organizations and encamp…
Our results highlight the need for strengthened collaboration between the VAW and housing/homelessness sectors and a unified policy strategy to address homelessness that applies a gender-transformative and intersectional approach, during and beyond public health emergencies
Assessing the impact of the introduction of generic methadone to the Ontario public drug formulary
Our findings did not indicate broad treatment destabilization or increased opioid-related toxicity among methadone recipients in the months following the introduction of generic products. Early notification of formulary changes, phased implementation of the policy, as well as information resources developed for patients and clinicians may have played a role in mitigating potential disruptions for this population
“Everybody is impacted. Everybody's hurting”: Grief, loss and the emotional impacts of overdose on harm reduction workers
Our study highlights opportunities for organizational practices that better support harm reduction workers, including formal emotional supports and community-based supportive care services. Improvement to the socio-economic-political determinants of work such as adequate compensation and access to full benefit packages are also needed in the harm reduction sector for all workers
“They have their security, we have our community”: Mutual support among people experiencing homelessness in encampments in Toronto during the Covid-19 pandemic
Unaffordable housing is a growing crisis in Canada, exacerbated by the COVID-19 pandemic, yet perspectives of people living outdoors in encampments have primarily gone unheard. We conducted qualitative interviews with encampment residents to explore how mutual support occurred within the social context of encampments. We found that mutually supportive interactions helped residents meet basic survival needs, as well as health and social needs, and…
How Structural Violence, Prohibition, and Stigma Have Paralyzed North American Responses to Opioid Overdose
As of 2020, North America is now into the fifth year of an unprecedented increase in drug overdose deaths driven by a toxic, unpredictable, and unregulated drug supply. While the genesis and drivers of and response to the opioid overdose crisis have wide regional variations, structural violence, prohibitions against illicit drug use, and stigma consistently play a central role. The criminalization of users of illicit drugs has led directly not on…
Help me fix: The provision of injection assistance at an unsanctioned overdose prevention site in Toronto, Canada
Critical studies of harm reduction: Overdose response in uncertain political times
Understanding real-world adherence in the directly acting antiviral era: A prospective evaluation of adherence among people with a history of drug use at a community-based program in Toronto, Canada
This study demonstrates that strong adherence and SVR with DAAs is achievable, with appropriate supports, even in the context of substance use, and complex health/social issues
Beyond viral response: A prospective evaluation of a community-based, multi-disciplinary, peer-driven model of HCV treatment and support
