Epidemiology of traumatic brain injury in a South African major trauma cohort
Background: South Africa has one of the highest trauma burdens globally, with unique contextual challenges around health system capacity. Traumatic brain injury (TBI) is a significant contributor to post-injury mortality and morbidity. We aimed to examine TBI prevalence and outcomes in adult major trauma patients. Methods: We conducted a secondary analysis of the Epidemiology and Outcomes of Prolonged Trauma Care (EpiC) study, a prospective obser…
Preventable trauma deaths and health system strengthening recommendations
One-third of deaths were preventable or potentially preventable, demonstrating a decrease from a review of 2021 trauma deaths in the same system which found 45% to be preventable. Findings highlight a continued need to increase training and strategic utilization of limited existing resources, such as CT and operating theatres, to improve the quality of trauma care. Preventable trauma death reviews are a powerful methodology to inform actionable c…
Preventable trauma deaths in the Western Cape of South Africa
Injury causes 4.4 million deaths worldwide annually. 90% of all injury-related deaths occur in low-and-middle income countries. Findings from expert-led trauma death reviews can inform strategies to reduce trauma deaths. A cohort of trauma decedents was identified from an on-going study in the Western Cape Province of South Africa. For each case, demographics, injury characteristics, time and location of death and postmortem findings were collect…
A community-based ambulance model
The role of the emergency medical service (EMS) is changing globally as ambulance crews respond to a shifting burden of disease, as well as societal stressors such as violence and inequality. New ways of thinking about how to provide emergency care are required to shift EMS from a role primarily focused on clinical care and transporting patients to hospital. In this paper, we present the experience of the Philippi Project (PP), an innovative comm…
