Lifetime Health Effects and Cost-Effectiveness of Tirzepatide and Semaglutide in US Adults
This economic evaluation found that although tirzepatide and semaglutide offered substantial long-term health benefits, they were not cost-effective at current net prices. Efforts to reduce the net prices of new antiobesity medications are essential to ensure equitable access to highly effective antiobesity medications
Fiscal Impact of Expanded Medicare Coverage for GLP-1 Receptor Agonists to Treat Obesity
This economic evaluation estimates that expanded Medicare coverage for GLP-1RAs would increase access and reduce obesity-related comorbidities but impose substantial costs over 10 years. Even with a moderate scenario (5% uptake, 20% adherence, and 30% additional price discount), net spending was still projected to reach $8 billion over a decade, underscoring the need for further price reductions, lower-cost strategies to prevent weight regain, an…

Common Data Element (CDE) Success from Community-Engagement Not Mandates: If You Build It [Data Warehouse], Will They [Participants] Come?, an article from American Journal of Public Health, Vol 115 Issue S2
Collaborators in Health
Effective community-engaged research is critical for designing and testing solutions to decrease the 10-year life expectancy gap between Black and non-Black residents of Chicago, Illinois. In community listening sessions, the Chicago Chronic Conditions Engagement Network found that community members’ highest priority health issues were social determinants of health and that there is a strong mistrust of research. Conversations with leaders of com…

Costs and Health Care Utilization Analysis of Medical Group Visits for Adults With Type 2 Diabetes in Community Health Centers
OBJECTIVE: We evaluated the economic impact of group visits (GVs) in adults with uncontrolled diabetes in community health centers (CHCs) in the United States. RESEARCH DESIGN AND METHODS: In this prospective controlled trial, we implemented 6 monthly GV sessions in 5 CHCs and compared intervention patients (n=49) to control patients (n=72) receiving usual care within the same CHCs. We conducted patient chart reviews to obtain health care utiliza…
Response to Letter to the Editor by Drs Cook and Stange
We appreciate the insights presented by Drs Cook and Stange on the growth and development of Federally Qualified Health Centers (FQHCs) over the last decade and the numerous policy changes, interventions, and secular events (COVID-19 pandemic) that impact current diabetes care and outcomes. We prioritized a national analysis of beneficiarylevel Medicaid data, which forced us to utilize older data because of the lag time on the availability of nati…
Quality of Care for US Adults With Medicaid Insurance and Type 2 Diabetes in Federally Qualified Health Centers Compared With Other Primary Care Settings
OBJECTIVE: The objective of this study was to evaluate indicators of diabetes quality of care for US nonelderly, adult Medicaid enrollees with type 2 diabetes and compare federally qualified health centers (FQHCs) versus non-FQHCs. RESEARCH DESIGN AND METHODS: We analyzed diabetes process measures and acute health services utilization with 2012 US fee-for-service and managed care Medicaid claims in all 50 states and DC. We compared FQHC (N=121,97…
Model Exploration of an Information-Based Healthcare Intervention Using Parallelization and Active Learning
This paper describes the application of a large-scale active learning method to characterize the parameter space of a computational agent-based model developed to investigate the impact of CommunityRx, a clinical information-based health intervention that provides patients with personalized information about local community resources to meet basic and self-care needs. Additionally, the diffusion of information about community resources and their …
Neighborhood Disadvantage and Hospital Quality Ratings in the Medicare Hospital Compare Program
BACKGROUND: The Centers for Medicare and Medicaid Services provide nationwide hospital ratings that may influence reimbursement. These ratings do not account for the social risk of communities and may inadvertently penalize hospitals that service disadvantaged neighborhoods. OBJECTIVE: This study examines the relationship between neighborhood social risk factors (SRFs) and hospital ratings in Medicare's Hospital Compare Program. RESEARCH DESIGN: …
CommunityRx
Objectives. To test the effect of CommunityRx, a scalable, low-intensity intervention that matches patients to community resources, on mental health-related quality of life (HRQOL) (primary outcome), physical HRQOL, and confidence in finding resources. Methods. A real-world trial assigned publicly insured residents of Chicago, Illinois, aged 45 to 74 years to an intervention (n = 209) or control (n = 202) group by alternating calendar week, Decem…

Objectives. To compare health care use and spending of Medicaid enrollees seen at federally qualified health centers versus non–health center settings in a context of significant growth. Methods. Using fee-for-service Medicaid claims from 13 states in 2009, we compared patients receiving the majority of their primary care in federally qualified health centers with propensity score–matched comparison groups receiving primary care in other settings…
Ethnic Differences in Geriatric Conditions and Diabetes Complications Among Older, Insured Adults With Diabetes
Among these insured older adults, ethnic health patterns varied substantially; differences were frequently small and rates were often better among select minority groups, suggesting progress toward the Healthy People 2020 objective to reduce health disparities
Revisiting Disparities in Quality of Care Among US Adults With Diabetes in the Era of Individualized Care, NHANES 2007–2010
BACKGROUND: Diabetes quality of care standards promote uniform goals and are used routinely for performance measurement and reimbursement. Diabetes health disparities have been characterized using these universal goals. However, guidelines emphasize individualized goals. OBJECTIVES: To assess diabetes care disparities using individualized goals to (1) determine their racial/ethnic distribution and (2) compare disparities using individualized vers…
Glycosylated Hemoglobin Testing in the National Social Life, Health, and Aging Project
Collection of HbA1C in Wave 2 permits researchers to examine the relationship between HbA1C and new health and social measures added in Wave 2, and to identify factors related to the change in HbA1C. Changes in collection protocol and labs between waves may have yielded small systematic differences that require analysts to carefully interpret absolute HbA1C values. We recommend analytic methods for cross wave differences in HbA1C and steps to ens…
Survey Field Methods for Expanded Biospecimen and Biomeasure Collection in NSHAP Wave 2
Improved field methods included in-home collection, temperature control, establishment of a central survey biomeasure laboratory, and shipping, all of which were crucial for successful collection by the field interviewers and accurate laboratory assay of the biomeasures (92.1% average co-operation rate and 97.3% average assay success rate). Developed for home interviews, these biomeasures are readily applicable to other surveys
Associations Between Medical Home Characteristics and Support for Patient Activation in the Safety Net
BACKGROUND: Few studies have evaluated whether the patient-centered medical home (PCMH) supports patient activation and none have evaluated whether support for patient activation differs among racial and ethnic groups or by health status. This is critical because activation is lower on average among minority patients and those in poorer health. OBJECTIVE: To assess the association between clinic PCMH characteristics and patient perception of clin…
Defining and Measuring Chronic Conditions
Current trends in US population growth, age distribution, and disease dynamics foretell rises in the prevalence of chronic diseases and other chronic conditions. These trends include the rapidly growing population of older adults, the increasing life expectancy associated with advances in public health and clinical medicine, the persistently high prevalence of some risk factors, and the emerging high prevalence of multiple chronic conditions. Alt…
Health Utilities for Children and Adults With Type 1 Diabetes
OBJECTIVE: We studied health utilities in patients with type 1 diabetes to understand potential differences in health utilities as function of age, type of respondent (self report vs. proxy report), and method of assessment (direct vs. indirect). RESEARCH DESIGN AND METHODS: We elicited self-reported health utilities for adults (n=213) and children (n=238) with type 1 diabetes, and by parent proxy report (n=223) for overall quality of life [Healt…
Positive and Negative Spillovers of the Health Disparities Collaboratives in Federally Qualified Health Centers
INTRODUCTION: Quality improvement (QI) interventions are usually evaluated for their intended effect; little is known about whether they generate significant positive or negative spillovers. METHODS: We mailed a 39-item self-administered survey to the 1256 staff at 135 federally qualified health centers (FQHC) implementing the Health Disparities Collaboratives (HDC), a large-scale QI collaborative intervention. We asked about the extent to which …
Changes in racial/ethnic disparities in the prevalence of Type 2 diabetes by obesity level among US adults
Racial/ethnic disparities in diabetes prevalence have become most pronounced among normal and overweight groups. Eliminating racial/ethnic disparities in diabetes will require prevention efforts not only in obese minority individuals, but also in normal and overweight minority individuals
Insurance Status and Quality of Diabetes Care in Community Health Centers
Objectives. We sought to compare quality of diabetes care by insurance type in federally funded community health centers. Method. We categorized 2018 diabetes patients, randomly selected from 27 community health centers in 17 states in 2002, into 6 mutually exclusive insurance groups. We used multivariate logistic regression analyses to compare quality of diabetes care according to 6 National Committee for Quality Assurance Health Plan Employer D…
Variation in Treatment Preferences and Care Goals Among Older Patients With Diabetes and Their Physicians
BACKGROUND: Older persons with diabetes are heterogeneous with respect to life expectancy and frailty, and new guidelines recommend individualizing care. OBJECTIVES: (1) To describe variation in the preferences of older patients with diabetes regarding aggressiveness of glycemic control and avoiding diabetic complications. (2) To determine correlates of patient preferences and physician treatment goals. (3) To assess whether physicians' goals wer…

Implications of New Geriatric Diabetes Care Guidelines for the Assessment of Quality of Care in Older Patients
BACKGROUND: Current approaches to assessing quality of diabetes care do not account for the heterogeneity of older patients. OBJECTIVE: We sought to compare conclusions regarding adequacy of glucose and blood pressure control using current quality assessment approaches and a stratified approach based on geriatric care guidelines. DESIGN: This was a cross-sectional evaluation of diabetes care. SUBJECTS: We studied patients older than 65, living wi…
Diabetes Health Disparities
Racial and ethnic minorities bear a disproportionate burden of the diabetes epidemic; they have higher prevalence rates, worse diabetes control, and higher rates of complications. This article reviews the effectiveness of health care interventions at improving health outcomes and/or reducing diabetes health disparities among racial/ethnic minorities with diabetes. Forty-two studies met inclusion criteria. On average, these health care interventio…
