Estimating the human development impacts and economic returns from an adolescent cash ‘plus’ programme in Kenya
The Government of Kenya plans to scale-up an adolescent-focused cash ‘plus’ programme to improve educational attainment and prevent adolescent pregnancy. We aimed to estimate the programme’s impacts, economic returns, and cost-effectiveness using model-based economic evaluation. We modelled the delivery of conditional cash transfers ‘plus’ adolescent clubs, community conversations, parenting support and psychosocial support to poor adolescents in…
Episodic memory trajectories of older adults with and without HIV
With the introduction of antiretroviral treatment (ART), life expectancy among people with HIV in South Africa has increased substantially. This ageing population faces age-related challenges, including cognitive decline. Yet, differences in cognitive trends between people with and without HIV remain poorly understood. We analyzed longitudinal data on 3,066 adults aged 40 + years in rural South Africa from the “Health and Aging in Africa: Longitu…
Characterising concerns and misinformation shaping global HIV vaccine confidence and demand
BACKGROUND: Demand for a future HIV vaccine, similar to other vaccines, is vulnerable to misinformation. A number of studies have examined HIV vaccine acceptability and demand, but few have systematically explored the specific concerns and misinformation that shape these attitudes. This structured review and content analysis of published and grey literature identifies concerns about the HIV vaccine that might fuel misinformation about the vaccine…
Violence Victimisation and Mental Health Distress Among Adolescent Girls Living with HIV in Southern Africa
Adolescent girls and young women (AGYW) living with HIV face heightened risks of mental distress, which may be compounded by exposure to violence and socioeconomic vulnerabilities. This study examined whether social protection moderates the relationship between violence exposure and mental distress among AGYW living with HIV in Eswatini, Lesotho, and Namibia. Using data from the Violence Against Children and Youth Surveys (VACS), we analysed 1,00…
“You could hear their breathing slow down”
High rates of AIDS-related mortality and HIV incidence persist due to delayed antiretroviral therapy (ART) uptake and retention. We qualitatively assessed the feasibility and acceptability of "From Now On" (FNO) - a short video-based psychosocial support intervention to increase ART uptake and improve psychosocial well-being among men newly diagnosed with HIV in South Africa. In-depth interviews were conducted with men who were shown FNO during p…
Accelerating HIV Prevention in Cameroon
Adolescent girls in Cameroon face substantial sexual and reproductive health vulnerabilities that heighten their risk of HIV exposure. Using data from 2,586 adolescent girls aged 15-19 years from the 2017-18 Cameroon Population-based HIV Impact Assessment, we examined factors associated with multiple sexual partnerships, age-disparate sex, inconsistent condom use, transactional sex, and a composite measure of high-risk sex. Overall, 4.31% reporte…
Understanding HIV Vaccine Misinformation and Vaccine Intentions Among Young Women in South Africa
An effective HIV vaccine could significantly reduce HIV incidence, but demand for future HIV vaccines will be affected by misinformation. We conducted an online survey (March-April 2024) to identify the misinformation claims most likely to deter young women in South Africa (n = 188) from receiving an HIV vaccine. Participants rated HIV-vaccine misinformation as most concerning (i.e. would make them not want to get vaccinated) and least concerning…
Accelerating the prevention of HIV and violence in adolescent girls and young women in Zimbabwe through multi-sectoral programming
In Zimbabwe, adolescent girls and young women (AGYW) encounter significant challenges, including HIV risk and gender-based violence. This study analyses data from 7,211 AGYW aged 13-24 from the 2017 Zimbabwe Violence Against Children Survey. Using a multivariable logistic regression model, it examines the associations between three protective factors: positive parenting, food security, and equitable-gender attitudes and 12 outcomes. Westfall-Youn…
Behaviourally-Informed Two-Way Text Messaging to Improve Return to HIV Care in South Africa
One-way text messaging to re-engage people in HIV care has shown promise, but little is known about two-way messaging. We evaluated a behaviourally-informed two-way text messaging intervention to increase re-engagement in care following missed visits. We conducted an individual-level randomised controlled trial between February and March 2023 in Capricorn District, South Africa. Adults aged ≥ 18 years who had missed an ART visit by > 28 days were…
Tackling the Triple Threat in Kenya
The government of Kenya's Triple Threat strategy recognises the need for urgent intervention to address the high incidence of HIV, gender-based violence (GBV) and pregnancy among adolescent girls and young women (AGYW). We used nationally representative data among AGYW aged 13-24 (n = 1344) from the 2018-19 Kenya Violence Against Children & Youth Survey (VACS) in multivariable logistic regression analyses to examine associations between three pot…

Predictors of Covid-19 vaccine uptake among adults in South Africa
BACKGROUND: COVID-19 vaccine coverage remains low in many low and middle-income countries despite widespread access. To understand the dynamic decision-making process around vaccination and provide evidence for future vaccine promotion campaigns, we identified predictors of COVID-19 vaccine uptake among South African adults, including those who did not believe in the vaccine's safety or efficacy. METHODS: Data from two longitudinal telephone surv…
Correlates of Covid-19 vaccination intentions and opinions about mandates among four groups of adults in South Africa with distinct vaccine intentions
The profile of individuals not vaccinated against COVID-19 as of March 2022 varied markedly by self-reported vaccination intentions, underscoring the importance of tailored demand-creation efforts. This paper highlights several factors which differ significantly across these groups. These findings could inform the design of future vaccination campaigns, potentially increasing their likelihood of success. This is an important policy objective give…
Quasi-experimental evaluation of a financial incentive for first-dose Covid-19 vaccination among adults aged ≥60 years in South Africa
INTRODUCTION: COVID-19 vaccination coverage in South Africa (RSA) remains low despite increased access to vaccines. On 1 November 2021, RSA introduced the Vooma Voucher programme which provided a small guaranteed financial incentive, a Vooma Voucher redeemable at grocery stores, for COVID-19 vaccination among older adults, a population most vulnerable to serious illness, hospitalisation and death. However, the association of financial incentives …
Longitudinal changes in Covid-19 vaccination intent among South African adults
Willingness to receive a COVID-19 vaccine has increased among adults in South Africa, and those who were entrenched in their reluctance make up a small proportion of the country's population. Younger adults, those in formal housing, and those who trusted COVID-19 information on social media were more likely to be hesitant. Given that stated vaccination intent may not translate into behaviour, our finding that three-quarters of the population were…
Poor rates of linkage to HIV care and uptake of treatment after home-based HIV testing among newly diagnosed 15-to-49 year-old men and women in a high HIV prevalence setting in South Africa
Early antiretroviral therapy (ART) initiation is essential, but linkage to care following community-based services is often poor, and inadequately understood. This study examined factors influencing linkage to care following home-based HIV-testing services (HBHTS) in a hyper-endemic setting in South Africa. HBHTS was offered to participants (N = 10,236) enrolled in the second HIV Incidence Provincial Surveillance System survey (2015–2016), KwaZul…

Trajectories of HIV-related internalized stigma and disclosure concerns among art initiators and noninitiators in South Africa
Among ART-eligible South African PLHIV entering into HIV care, internalized stigma modestly declined over time but disclosure concerns persisted. PLHIV who initiated ART were more likely to have persistent disclosure concerns over time as compared with those who did not start ART
Perceived HIV-related stigma among university students in South Africa
HIV-related stigma, and particularly perceived stigma, has a negative impact across the HIV care continuum. This study adds to our understanding of stigma by assessing how perceived stigma varies from one context to another and how such differences are associated with the location where individuals would prefer an HIV test. We used self-administered questionnaire data (n = 378) obtained from a convenience sample of students (18 years and older) a…

TheEmpower Nudgelottery to increase dual protection use
The objective of this study is to measure the preliminary efficacy of a pilot intervention, grounded in behavioural economics, increasing adherence of dual protection (simultaneous use of effective modern contraception and a barrier method, such as a condom) to protect against HIV, other sexually transmitted infections, and unintended pregnancy. Between 2015 and 2016, 100 women aged 18-40 years, seeking post-abortion care in Cape Town, South Afri…
Accuracy and determinants of perceived HIV risk among young women in South Africa
Results indicate that HIV-risk perceptions are inaccurate. Our findings suggest that this inaccuracy stems from HIV-risk perceptions being driven by an incomplete understanding of epidemiological risk and being influenced by a range of psycho-social factors not directly related to sexual behaviour. Consequently, new interventions are needed to align perceived and actual HIV risk
Stumbling Blocks at the Clinic
Attitudes towards people with HIV/Aids
HIV/AIDS-related stigma is conceptualised as multi-dimensional, and indices are designed to measure its different dimensions. Findings show that it is uncommon for young adults to have high levels of negative behavioural intentions towards people living with HIV/AIDS, but the majority show some tendencies to discriminate. Expressions of negative judgement (symbolic stigma) and fear of HIV infection (instrumental stigma) are both prevalent. Instru…

Poverty, sexual behaviour, gender and HIV infection among young black men and women in Cape Town, South Africa
This article contributes methodologically and substantively to the debate over the importance of poverty, sexual behaviour and circumcision in relation to HIV infection, using panel data on young black men and women in Cape Town, South Africa. Methodological challenges included problems of endogeneity and blunt indicator variables, especially for the measurement of sexual behaviour. Noting these difficulties, we found that the importance of socio…

Social and cultural contexts of concurrency in a township in Cape Town, South Africa
Understanding the social and cultural context in which concurrent sexual partnerships exist is important, given recent interventions to reduce their prevalence. This qualitative study seeks to improve the understanding of concurrent partnerships and perceptions of the link between concurrency and HIV risk in a South African township in Cape Town. Small-group discussion and focus-group participants reported that concurrency was a common phenomenon…
