Peer-led recruitment for equitable engagement of people who inject drugs
BACKGROUND: Engaging people who inject drugs (PWID) in research is challenging due to stigma, distrust, and structural barriers. Peer-led recruitment may enhance reach and equity, yet few studies have quantitatively evaluated its outcomes. METHODS: Between Feb 2021-Dec 2023, a Partnerships Lead with Lived Experience implemented three new recruitment strategies within the HEPCO cohort study of PWID in Montreal, Canada. Strategies extended research…
Gender differences in hepatitis C virus incidence in and out of opioid agonist treatment among people who inject drugs in Montréal, Canada, March 2011–March 2024
We found similar HCV incidence among women and men who inject drugs in Montréal; however, among OAT-eligible men, OAT engagement was strongly associated with a lower risk of HCV acquisition. These findings highlight that gender disparities in HCV, as observed in some settings, are not consistent. Further research is needed to determine factors underlying gender differences
A scoping review and concept analysis to inform Canada’s safe(r) opioid supply research agenda
Safe(r) supply encompasses prescribed opioid alternatives interventions (safer supply) and non-medicalized (safe supply) approaches with shared antecedents but distinct attributes and consequences. This study highlights the need to better define and standardize the parameters of safer supply approaches, including population, dosing, and intended objectives, to enable a more precise assessment of their potential benefits and risks. This nuanced un…

Overall, our study demonstrated a high prevalence of HCV testing and subsequent disclosure of HCV status within injecting partnerships. This presents an opportunity to leverage these relationships for treatment linkage and prevention messaging
Characteristics of supervised injection site clients and factors associated with requiring overdose intervention, Montreal, Canada, 2018–2022
Across four Montréal SIS, site visits with an overdose requiring intervention were associated with transgender identity, younger age, stable housing, intending to inject fentanyl, injecting less frequently than daily, injecting for either more than five years or less than a year, single site attendance, and injecting most often in a public place. Montréal SIS serve a diverse clientele who inject drugs, with differing risks of onsite overdose
Trends in accidental drug overdose mortality in Canada
Canada's overdose epidemic reflects a segmented trajectory, with marked accelerations in 1996 and 2013, driven by healthcare practices, evolving drug markets, and social vulnerabilities. Regional and demographic disparities underscore the need for targeted, historically informed public health strategies

Using an intersectionality lens to explore barriers and enablers to hepatitis C point-of-care testing
The application of an intersectionality lens provides a nuanced understanding of multilevel barriers and enablers to point-of-care HCV testing. Findings underscore the need for tailored strategies that address stigma, improve provider roles and communication, and foster an inclusive environment for equitable HCV care. Using an intersectionality lens in implementation research can offer valuable insights, guiding the design of equity-focused imple…

Services de navigation des soins
Contexte.

A rapid review of the impacts of “Big Events ” on risks, harms, and service delivery among people who use drugs
BACKGROUND: "Big Events" are major disruptions to physical, political, and economic environments that can influence vulnerability to drug-related harms. We reviewed the impacts of Big Events with relevance to the COVID-19 pandemic on drug-related risk and harms and access to drug treatment and harm reduction services. METHODS: We conducted a rapid review of quantitative, qualitative, and mixed methods literature relating to the following Big Even…
Needle exchange, HIV infection, and the politics of science
An epidemiological cohort study of injection drug users in Montréal documented a significant statistical association between HIV seroconversion and needle exchange patronage from 1989 to 1995. The polemics generated by this counterintuitive quantitative data demonstrate the urgent need for a cross‐methodological dialogue between epidemiology and ethnography. Participant observation on the streets of Montréal documented the multiple HIV risks asso…
Associations between HIV-related injection behaviour and distance to and patterns of utilisation of syringe-supply programmes
BACKGROUND: Studies indicate that needle-exchange programmes (NEPs) can reduce the incidence of HIV infection; however, a positive impact of syringe-supply programmes has not been consistently demonstrated. The associations between high-risk injection behaviour and distance to and patterns of utilisation of syringe-supply programmes in injection drug users (IDUs) were investigated. METHODS: Participants in a cohort of IDUs (n = 456) residing in M…
High Rates of HIV Infection among Injection Drug Users Participating in Needle Exchange Programs in Montreal
Needle exchange programs (NEPs) are designed to prevent human immunodeficiency virus (HIV) transmission among injection drug users. Although most studies report beneficial effects in terms of behavior modification, a direct assessment of the effectiveness of NEPs in preventing HIV infection has been lacking. A cohort study was conducted to assess the association between risk behaviors and HIV seroprevalence and seroincidence among injection drug …

Surviving the sex trade
In Canada, very little is known about the factors and processes that cause drug-related harm among female intravenous drug users (IDUs). Women who inject drugs and participate in the survival sex trade are considered to be at increased risk for sexual and drug-related harms, including HIV infection. Between September 1999 and September 2000, women participating in the VIDUS cohort in Vancouver and the St. Luc Cohort in Montreal completed intervie…

Exploring the contingent reality of biomedicine
Hepatitis C is an emerging theme of contemporary public health discourses related to illicit drug injection practices. Such discourses differentiate injecting from non-injecting users in an actuarial risk logic, targeting drug-injecting users as a population in need of support in the management of risks attributed to their practices. Public health strategies suppose, among other things, that injecting drug users adopt a homogeneous vision of hepa…
