It’s Unjust to Treat Pregnant People this Way”
To describe asylum-seeking women's perspectives on and lived experiences of obstetric violence, including how it is shaped by structural contexts related to stigma, discrimination, and migration policies and systems. We conducted a qualitative study involving thematic analysis of in-depth interviews conducted July 2022-April 2023 with pregnant and postpartum asylum-seeking women at the Mexico-U.S. border (N = 38). Asylum-seeking pregnant and post…
“In the end … I got pregnant. And that wasn't what the plans were”
Reproductive health and rights for migrant women intending to seek safety in the U.S. are shaped by the risk environments through which they migrate, including policy, social, economic, and physical factors. We examined migrant women's lived experiences of reproductive health and rights resulting from and perpetuated by these risk environments across migration phases. As part of a larger study examining maternal and reproductive health among U.S.…
Adverse migration experiences, border community stress, and the mental health of asylum-seeking women in transit at the Mexico-U.S. border
We evaluated the association between adverse migration experiences, border community stress, and mental health symptoms in a community-recruited sample of asylum-seeking women. As part of a mixed-methods, community-academic partnership, we analyzed cross-sectional questionnaires conducted with asylum-seeking women at the Tijuana-San Diego border (N=151) between Feb-July 2024. Symptoms of anxiety (56.6%), moderate-to-severe depression (45.9%), and…
Everyday discrimination and barriers to primary care, mental health, and substance use services
We evaluated the association between discrimination and access to primary, mental health, and substance use services among sex workers. Using baseline and semi-annual questionnaire data from a community-based cohort of sex workers in Vancouver, Canada (09/2015-02/2024), we used bivariate and multivariable logistic regression with generalized estimating equations to analyze the relationship between discrimination and access to primary care, mental…
Impacts of local, provincial, and federal immigration policies on health and social services access among women with precarious immigration status
Sanctuary City policies are recommended to advance im/migrants' human rights, reduce instances of delayed or denied care, untreated illnesses, and social isolation. Full implementation of Sanctuary principles at the local level (i.e., reduced collaboration between local service providers and federal immigration enforcement) is needed to improve access to health and other services based on need, regardless of immigration status. At the provincial …
Community-administered naloxone for overdose reversal
Over 5.5 years, over half of participants administered THN, which was associated with exposure to sex work-specific programs and occupational violence. Violence from clients, community members, and police independently increased the likelihood of THN administration. Findings suggest that marginalized sex workers, particularly those who use drugs in occupational settings, are uniquely positioned to respond to overdoses. This supports the feasibili…
Association between sex work occupational stigma and inconsistent condom use
BACKGROUND: Women sex workers face substantial health inequities due to structural barriers including criminalisation and stigma. Stigma has been associated with HIV-related inequities among marginalised populations, however, we know less about the impacts of sex work-specific occupational stigma on HIV/sexually transmitted infection (STI) risk among women sex workers. Given these research gaps and the disproportionate burden of stigma faced by s…
Sex workers' engagement with mutual aid on-the-ground and online
The relationship between neighbourhood income and youth mental health service use differs by immigration experience
Mental health service use was substantially lower among immigrant youth than non-immigrant youth, but higher proportions of immigrant youth were hospitalized involuntarily. The reverse income gradient patterns observed for community-mental health service use are noteworthy and suggest significant barriers to accessing preventable care among immigrant youth, particularly those living in lower-income neighbourhoods
Water, sanitation, and hygiene access among people who inject drugs in Tijuana and San Diego in 2020–2021
WASH access among PWID in the Tijuana-San Diego metropolitan area was low by international standards and lower than the national averages in both countries. Homelessness was significantly associated with WASH insecurity in this population. Concentrated efforts are needed to guarantee continuously available WASH services for PWID-especially those who are unsheltered
Pandemic-related challenges accessing food and primary healthcare among sex workers during the Covid-19 pandemic
Findings provide insight about the potential role community-based healthcare delivery settings (e.g., community clinics) can play in ameliorating access to basic needs such as food among those who are highly marginalized. Future pandemic response efforts should also take the most marginalized populations' needs into consideration by establishing strategies to ensure continuity of essential services providing food and other basic needs. Lastly, po…
Safe Injection Self-Efficacy is Associated with HCV and HIV Seropositivity Among People Who Inject Drugs in the San Diego–Tijuana Border Region
A mixed methods study to inform fatal overdose prevention in San Diego, California
Water, sanitation and hygiene insecurity predict abscess incidence among people who inject drugs in a binational US–Mexico metropolitan area
PWID in the Tijuana-San Diego metropolitan area reported facing high rates of insecurity accessing WASH services. Abscess incidence was higher (four to nine times) than observed rate among PWID cohorts in other settings. Access to continuously available toilet facilities, bathing infrastructure, and safe water sources for preparing drugs for injection could prevent abscesses among PWID. Accessible WASH infrastructure should be ensured among PWID …
Gaps in health coverage for racialized im/migrant sex workers in metro Vancouver
Policy and program reforms are needed to decouple health insurance access from immigration status, remove mandatory waiting periods for health insurance coverage, and ensure that provincial insurance provides sufficient coverage for marginalized women's healthcare needs
Impacts of Sex Work Criminalization and Censorship for Indoor Workers
“When I came to Canada, I almost forgot myself
For young women, im/migration can impact education, employment, and family dynamics in ways that shape the courses of their lives and health. The purpose of this study is to deepen understandings of how im/migration shapes young women's experiences of pregnancy, motherhood, and marriage. Guided by intersectionality theory and the structural and social determinants of health, we analyzed qualitative interviews conducted with young im/migrants ages…
‘We don’t have the right to get sick’
Globally, the exclusion of im/migrants from public health care systems remains a significant health and human rights issue, calling into question claims of ‘universality’ of public health systems where eligibility for coverage is determined by immigration status. We aimed to describe lived experiences of health insurance coverage and the health and social impacts of gaps in health insurance amongst im/migrant women in British Columbia (BC), Canad…
An Evaluation of Indoor Sex Workers’ Sexual Health Access in Metro Vancouver
The criminalization of sex work has been consistently shown to undermine workers' Occupational Health and Safety (OHS), including sexual health. Drawing on the 'Guide to OHS in the New Zealand Sex Industry' (the Guide), we assessed barriers to sexual health best practices among indoor sex workers in Metro Vancouver, Canada, in the context of ongoing criminalization. Part of a longstanding community-based study, this analysis drew on 47 qualitativ…
Sex, Immigration, and Patterns of Access to Primary Care in Canada
Access to primary care is crucial to immigrant health and may be shaped by sex and gender, but research is limited and inconclusive. We identified measures that reflect access to primary care using 2015-2018 Canadian Community Health Survey data. We used multivariable logistic regression models to estimate adjusted odds of primary care access and to explore interaction effects between sex and immigration group (recent immigrant: < 10 years in Can…
When you leave your country, this is what you’re in for”
Asylum deterrence policies undermine the health and safety of pregnant and parenting asylum-seeking women at the Mexico-U.S. border. There is an urgent need to end U.S. asylum deterrence policies and to provide respectful, appropriate, and adequately resourced humanitarian supports to pregnant and parenting asylum-seeking women in border cities, to reduce women's risk of GBV and trauma
Association of sex work and social-structural factors with non-fatal overdose among women who use drugs in Vancouver, Canada
Negative changes in illicit drug supply during Covid-19
Over two-thirds of participants experienced negative changes to illicit drug supply during the first year of the COVID-19 pandemic. The association between experiencing negative changes in the illicit drug supply and accessing overdose prevention services highlights the agency of women in taking measures to address overdose-related risks. Highly criminalized women who experience structural barriers to direct services are also vulnerable to fluctu…
Housing Instability and Evictions Linked to Elevated Intimate Partner and Workplace Violence Among Women Sex Workers in Vancouver, Canada
Objectives. To model the relationship of unstable housing and evictions with physical and sexual violence perpetrated against women sex workers in intimate and workplace settings. Methods. We used bivariate and multivariable logistic regression with generalized estimating equations to model the association of unstable housing exposure and evictions with intimate partner violence (IPV) and workplace violence among a community-based longitudinal co…
Sars-CoV-2 testing and Covid-19–related primary care use among people with citizenship, permanent residency, and temporary immigration status
Interwoven immigration, health, and occupational policies place people with temporary status in circumstances of precarity and higher health risk. Reducing precarity accompanying temporary status, including regularization pathways, and decoupling access to health care from immigration status can address health inequities

Not the swab!' Young men's experiences with STI testing
In Canada, STI rates are high and rising, especially amongst young men. Meanwhile, the needs of young men regarding STI testing services are poorly understood, as are the socio-cultural and structural factors that influence young men's sexual health-seeking behaviours. To better understand this phenomenon, we draw on interviews with 45 men (ages 15-25) from British Columbia, Canada. Our research reveals how structural forces (e.g. STI testing pro…

Complexities of Short-Term Mobility for Sex Work and Migration among Sex Workers
Right Here is the Gateway”
Women comprise an increasing proportion of migrants. Many voluntarily migrate for sex work or practice survival sex, while others may be trafficked for sexual exploitation. To investigate how the context of mobility shapes sex work entry and HIV risk, we conducted in-depth interviews with formerly trafficked women currently engaged in sex work (n=31) in Tijuana, Mexico and their service providers (n=7) in Tijuana and San Diego, USA from 2010-2011…
Sexually Transmitted Infection (STI) Testing among Young Oil and Gas Workers
He invited me and didn't ask anything in return” Migration and Mobility as Vulnerabilities for Sexual Exploitation among Female Adolescents in Mexico
Although human trafficking is recognized as a major human rights violation, there is limited evidence regarding the vulnerabilities that contribute to female adolescents' risk of being forced or coerced into the sex trade. Vulnerabilities such as gender-based violence, economic and social inequalities have been shown to shape the risk of sexual exploitation among adolescents. In-depth interviews (n=18) with current sex workers who reported being …
Urban gentrification and declining access to HIV/STI, sexual health, and outreach services amongst women sex workers between 2010-2014
Exploring the Context of Trafficking and Adolescent Sex Industry Involvement in Tijuana, Mexico
Coerced and adolescent sex industry involvement are linked to serious health and social consequences, including enhanced risk of HIV infection. Using ethnographic fieldwork, including interviews with 30 female sex workers with a history of coerced or adolescent sex industry involvement, we describe contextual factors influencing vulnerability to coerced and adolescent sex industry entry and their impacts on HIV risk and prevention. Early gender-b…
Heteronormativity hurts everyone
Heteronormative assumptions can negatively influence the lives of young gay and bisexual men, and recent sociological analyses have identified the negative impacts of heteronormativity on heterosexual men (e.g. 'fag discourse' targeted at heterosexual adolescents). However, insights into how heteronormative discourses may be (re)produced in clinical settings and how they contribute to health outcomes for gay, bisexual and heterosexual men are poo…
Barriers to Governmental Income Supports for Sex Workers during Covid-19
The COVID-19 pandemic has brought into stark focus the economic inequities faced by precarious, criminalized and racialized workers. Sex workers have been historically excluded from structural supports due to criminalization and occupational stigma. Given emerging concerns regarding sex workers' inequitable access to COVID-19 income supports in Canada and elsewhere, our objective was to identify prevalence and correlates of accessing emergency in…
How important are venue-based HIV risks among male clients of female sex workers? A mixed methods analysis of the risk environment in nightlife venues in Tijuana, Mexico
Impacts of Sex Work Criminalization and Censorship for Indoor Workers
It's not just about being here, but what brought you here”
This qualitative study aimed to understand how migration experiences shape im/migrant women's needs, desire for, and expectations of healthcare in the British Columbia (BC), Canada context. Interviews with 33 im/migrant women (December 2018-January 2020) highlighted that traumatic experiences across migration increased healthcare needs; insufficient prior health system information contributed to poor experiences; and comparative healthcare experi…
Third Parties (Venue Owners, Managers, Security, etc.) and Access to Occupational Health and Safety Among Sex Workers in a Canadian Setting
Objective. To determine the impact of engagement with third parties (i.e., managers, receptionists, or owners of in-call venues; advertisers; security; spotters; and others) on sex workers’ occupational health access. Methods. We drew longitudinal data from An Evaluation of Sex Workers’ Health Access, a community-based cohort of more than 900 women sex workers. We used multivariable logistic regression and generalized estimating equations to (1) …

Sexually Transmitted Infection (STI) Testing among Young Oil and Gas Workers
Psychosexual and social-cognitive correlates of sexual risk behavior among male clients of female sex workers in Tijuana, Mexico
Male clients of female sex workers (FSWs) may act as a bridge to the general population contributing to the spread of HIV and other sexually transmitted infections (STIs) in the USA and Mexico. This study used cross-sectional data to identify psychosexual and social-cognitive factors associated with sexual risk behavior in a bi-national sample of 300 male clients of FSWs recruited in Tijuana, Mexico from June to October 2008. In a multiple regres…
And they call this progress? Consequences for young people of living and working in resource-extraction communities
Communities dependent on natural resource extraction (e.g. oil/gas, mining) are routinely exposed to rhetoric that associates notions of progress (including health and social welfare) with a booming economy. These places frequently experience demographic and social disruptions associated with reliance on resource-extraction sectors (e.g. the influx of young male workers and money, increasing rates of drug/alcohol consumption, infrastructural shor…

Situating HIV risk in the lives of formerly trafficked female sex workers on the Mexico–US border
Due to stigma and the psychosocial repercussions of past trauma and abuse, survivors of sex trafficking may experience increased susceptibility to violence, revictimization, and various harmful health outcomes, including HIV infection. Given the paucity of research characterizing the experiences of formerly trafficked female sex workers (FSWs), we set out to describe and contextualize perceptions of HIV risk among women who have experienced past …
Venue-level correlates of female sex worker registration status
In Tijuana, Mexico, sex work is regulated by the municipal government, through registration cards issued to female sex workers (FSWs) for an annual fee. Registration has been associated with decreased drug use and increase condom use and HIV testing. Previously, it was demonstrated that FSWs operating in bars were more likely than street-based FSWs to be registered. This implies that certain venues may be more accessible to local authorities for …

Condoms and sexual health education as evidence
BACKGROUND: Despite a large body of evidence globally demonstrating that the criminalization of sex workers increases HIV/STI risks, we know far less about the impact of criminalization and policing of managers and in-call establishments on HIV/STI prevention among sex workers, and even less so among migrant sex workers. METHODS: Analysis draws on ethnographic fieldwork and 46 qualitative interviews with migrant sex workers, managers and business…

Migration, violence, and safety among migrant sex workers
Despite reports of high levels of violence among women migrants in Central America, limited evidence exists regarding the health and safety of migrant sex workers in Central America. This study is based on 16 months of field research (November 2012-February 2014), including ethnographic fieldwork, in-depth interviews, and focus groups conducted with 52 internal and international migrant female sex workers in Tecún Umán and Quetzaltenango, Guatema…
Recent im/migration to Canada linked to unmet health needs among sex workers in Vancouver, Canada
Despite universal health care in Canada, sex workers (SWs) and im/migrants experience suboptimal health care access. In this analysis, we examined the correlates of unmet health needs among SWs in Metro Vancouver over time. Data from a longitudinal cohort of women SWs (An Evaluation of Sex Workers Health Access [AESHA]) were used. Of 742 SWs, 25.5% reported unmet health needs at least once over the 4-year study period. In multivariable logistic r…
