Community health intervention through musical engagement (CHIME) in South Africa
In South Africa, perinatal depression, stress or anxiety affect an estimated 16% to 50% of women posing serious concerns for both mothers and infants. The vast majority of women receive no perinatal mental healthcare through the public health system, partly due to high levels of stigma and a lack of culturally sensitive mental health care. South African musical traditions such as group singing are culturally significant for supporting social conn…
The intergenerational legacies of research disinvestment, aid retrenchment and transactional compacts on global women’s health
Global health funding by the United States government declined precipitously in 2025, triggering a cascade of international disinvestment in women’s health in low and middle-income countries, followed by a new era of ‘America First’ aid policy. Successive budget cuts by other high-income countries compounded the effects of these abrupt policy and geopolitical changes on women’s health, safety, and access to life saving and health assuring prevent…
Strategies for spreading, scaling, and sustaining perinatal mental health interventions in Low- and Middle-Income Countries (LMICs) – A scoping review and thematic synthesis
There is a significant global burden of common perinatal mental health conditions (CPMHC), the most common complications of childbearing, with about one in five women being affected in LMICs.Health systems in these settings have not scaled, spread or sustained perinatal mental health services despite the evidence for effectiveness of several interventions. This is, in part due to a range of systems-level factors.Previous work on perinatal mental h…
Problem-solving therapy for pregnant women experiencing depressive symptoms and intimate partner violence
Evidence for the feasibility of brief psychological interventions for pregnant women experiencing intimate partner violence (IPV) in rural, low-income country settings is scarce. In rural Ethiopia, the prevalence of antenatal depressive symptoms and lifetime IPV are 29% and 61%, respectively. We aimed to assess the feasibility and related implementation outcomes of brief problem-solving therapy (PST) adapted for pregnant women experiencing IPV (P…
A mixed-methods feasibility study of Nyamekela4Care
In South Africa, health and care service providers (SPs) face many interconnected challenges at work, including inadequate training and supervision and high levels of burnout. Nyamekela4Care (N4C) was developed as an integrated intervention for SP teams, to support their ability to provide high-quality care through providing a structure for operating regular team meetings that includes training, empathic skills development, administration, case s…
Costs of common perinatal mental health problems in South Africa
Whilst important progress has been made in South Africa with regards to mental health policies and interventions that include assessment and management of perinatal mental health problems, substantial underinvestment prevents progress. Findings from this study strengthen the economic case for investing in perinatal mental health care
Food Insecurity, Maternal Mental Health, and Domestic Violence
The Secret History method and the development of an ethos of care
South Africa, like many low-and-middle-income countries, is integrating mental health services into routine Primary Health Care (PHC) through a task-shifting approach to reduce the gaps in treatment coverage. There is concern, however, that this approach will exacerbate nurses' abuse of patients currently common within PHC in the country. To address this concern, the Perinatal Mental Health Project developed its Secret History method, a critical …
The development of an ultra-short, maternal mental health screening tool in South Africa
This novel screening tool is the first measure of CPMD developed in South Africa to include depressed mood, anxiety symptoms and suicidal ideation. While the tool requires further investigation, it may be useful for the early identification of mental health symptoms and morbidity in the perinatal period
Validation of a brief mental health screening tool for pregnant women in a low socio-economic setting
Cognitive interviewing enabled testing and refining of the language and constructs of an ultra-brief screening tool. The shortened, 3-item tool is well understood and effective at identifying pregnant women with symptoms of CMD, across the three most commonly spoken languages and cultures in Cape Town
‘First 1000 days’ health interventions in low- and middle-income countries
BACKGROUND: In South Africa (SA), despite adoption of international strategies and approaches, maternal, neonatal and child (MNC) morbidity and mortality rates have not sufficiently declined. OBJECTIVES: To conduct an umbrella review (UR) that identifies interventions in low- and middle-income countries, with a high-quality evidence base, that improve MNC morbidity and mortality outcomes within the first 1000 days of life; and to assess the incor…
Patterns of use of a maternal mental health service in a low-resource antenatal setting in South Africa
The prevalence of perinatal common mental disorders in South Africa is high, yet little is known about mental health service use among pregnant and postnatal women. This paper reports on pregnant women's patterns of use of a counselling service at a primary level obstetric facility in Cape Town, South Africa, between January 2010 and December 2011. It investigates whether these are associated with demographics, severity and risk of depressive sym…
