Evaluation of the community-based suicide surveillance system in Northern Thailand using the RE-AIM framework
The Four Pillars strategy is a feasible and promising approach to reduce suicide risk through a community-based integrated surveillance and response system. Strategic local adaptations enhanced its feasibility without compromising core components. Addressing stigma, improving literacy, and strengthening multisectoral capacity are critical for broader scale-up and sustainability in Thailand and similar settings
Development of a remote implementation support strategy to enhance integration of depression treatment into primary care settings in rural India
Introduction: Contextually responsive implementation support strategies are needed to enhance the integration of mental health services into primary health care. Technical assistance is widely used as a core "capacity building" strategy, primarily for increasing the motivation and capacity of individuals (e.g., frontline workers) to adopt evidence-based interventions in healthcare settings. This article documents the systematic design of a techni…
Mechanisms explaining a pay-it-forward approach
Pay-it-forward is when an individual receives a gift and is offered the opportunity to donate a gift to another person. This prosocial behavior has been used to develop health interventions, including strategies to increase sexually transmitted disease (STD) testing among men who have sex with men (MSM). Although pay-it-forward has improved service uptake across several settings, the underlying mechanism is unclear. This study aims to assess the …
Pay-it-forward gonorrhea and chlamydia testing among men who have sex with men and male STD patients in China
The study with identification number NCT05723263 has been registered on clinicaltrials.gov
Online Focus Group Discussions to Engage Stigmatized Populations in Qualitative Health Research
Community participation in research involving stigmatized populations has been sub-optimal, and digital tools could potentially increase participation in qualitative research. This study aims to describe the implementation of an online chat-based FGD (Focus Group Discussion) with men who have sex with men (MSM) in China as part of formative research for the PIONEER project, determine the advantages and limitations associated with the approach, an…
Beyond Virtue-Signaling
As public health mourns the inequitable loss of lives to coronavirus disease 2019 (COVID-19) and confronts other major social crises, practitioners must explicitly address systems of oppression in their everyday praxis. We describe how the principles of public health critical race praxis (PHCRP) and design justice (DJ) can advance equity in public health. We begin with an overview of PHCRP and DJ, and develop an integrated approach to facilitate …
Evaluating the design and implementation fidelity of an adapted Plan-Do-Study-Act approach to improve health system performance in a Nigerian state
This study demonstrates how both adaptation and implementation fidelity are important for success of QI interventions. It also presents an approach for evaluating other QI models using Taylor's PDSA assessment framework as a guide, which might serve to strengthen the theory behind future QI models and provide guidance on their appropriate use
The community transformation map
Transforming communities to be healthier and more equitable prosents a systemic challenge best addressed by those with native knowledge of the system. Community coalitions are a promising structure for tackling local health inequities, if they approach the change process with multiple local stakeholders and with systemic change in mind. Maturity models offer a framework for system assessment by defining sequential stages toward ideal development.…
Scale-Up of Doppler to Improve Intrapartum Fetal Heart Rate Monitoring in Tanzania
High-quality intrapartum care, including intermittent monitoring of fetal heart rates (FHR) to detect and manage abnormalities, is recommended by WHO and the Government of Tanzania (GoT) and creates potential to save newborn lives in Tanzania. Handheld Doppler devices have been investigated in several low-resource countries as an alternative to Pinard stethoscope and are more sensitive to detecting accelerations and decelerations of the fetal hea…
Recognizing and engaging pharmacists in global public health in limited resource settings
Measuring and Understanding Depression in Women in Kisoro, Uganda
Depression is highly prevalent and the cause of considerable suffering for peoples across the globe. Case finding for depression is challenging because individuals often do not recognize the symptoms in themselves or may resist the diagnosis as a result of cultural stigma. Screening instruments, to be accurate, must be valid in the particular setting in which they are being applied, and diagnosis in primary care settings, is further made challeng…
Formative Evaluation and Complex Health Improvement Initiatives
Sustainable community health improvement often requires the implementation of complex interventions in complex systems. Drawing from the Four Keys to Success frame (theory, implementation, support, and evaluation), this article describes how we used a formative evaluation approach to foster a learning system capable of monitoring and addressing emerging community needs within the Spreading Community Accelerators Through Learning and Evaluation (S…
Strengthening decentralized primary healthcare planning in Nigeria using a quality improvement model
Quality improvement models have been applied across various levels of health systems with varying success leading to scepticisms about effectiveness. Health systems are complex, influenced by contexts and characterized by numerous interests. Thus, a shift in focus from examining whether improvement models work, to understanding why, when and where they work most effectively is essential. Nigeria introduced DIVA (Diagnose-Intervene-Verify-Adjust) …
Global health partnerships
BACKGROUND: In response to health care challenges worldwide, extensive funding has been channeled to the world's most vulnerable health systems. Funding alone is not sufficient to address the complex issues and challenges plaguing these health systems. To see lasting improvement in maternal and infant health outcomes in the developing world, a global commitment to the sharing of knowledge and resources through international partnerships is critic…
Evaluating Mobile Survey Tools (MSTs) for Field-Level Monitoring and Data Collection
Information and communications technologies (ICTs) such as mobile survey tools (MSTs) can facilitate field-level data collection to drive improvements in national and international development programs. MSTs allow users to gather and transmit field data in real time, standardize data storage and management, automate routine analyses, and visualize data. Dozens of diverse MST options are available, and users may struggle to select suitable options…
Evidence-Based Interventions Are Necessary but Not Sufficient for Achieving Outcomes in Each Setting in a Complex World
Many evaluations of programs tend to show few outcomes. One solution to this has been an increasing prominence of the movement that requires programs to implement evidence-based interventions (EBIs). But in a complex world with complex organizations and complex interventions, many challenges have arisen to the implementation of EBIs with fidelity to achieve outcomes at scale. This includes challenges to achieving outcomes in each setting. In this…
Challenges for Transformation
The proportion of individuals with mental disorders receiving evidence based treatments in India is very small. In order to address this huge treatment gap, programme for improving mental health care is being implemented in Sehore district of Madhya Pradesh, India. The aim of this study was to complete the situational analysis consisting of two parts; document review of Sehore district mental health programme followed by a qualitative study. The …
