Medicaid Primary Care Utilization and Area-Level Social Vulnerability
The findings of this cross-sectional study suggest that Medicaid policy should focus on addressing geography-based disparities in access to care using new measures to target resources. The multidimensional SVI is likely a useful tool to identify small geographic areas with barriers to accessing adequate health care. The FQHC findings suggest that substantially increasing investments and support for FQHCs would address geographic inequities in acc…
Variation in State Education Regulations for RNs
Background: There is variation in nursing education quality, measured as first-time NCLEX (National Council Licensure Examination)-RN pass rates (FTPR), both across and within the United States. Current research examines program-level characteristics associated with performance. Method: This study examines state-level policies and their relationship to FTPR (both associate and baccalaureate nursing degrees) to identify policies that enhance nursi…
Use of Temporary Primary Care Providers in Federally Qualified Health Centers
OBJECTIVE: This study examines the use of temporary providers in federally qualified health centers (FQHCs) in recent years and identifies associated factors. METHODS: Using 2013-2017 federal administrative data of 1,028 FQHCs, we describe trends in the number and percentage of FQHCs that used temporary primary care physicians and advanced practice providers (nurse practitioners, physician assistants, and certified nurse midwives). We employed de…
Health Workforce for Health Equity
The health workforce is a critical component of any health care system. There is no health care without the people who provide service. As such, the health workforce has a central role in addressing (or maintaining) health disparities. Health disparities are significant and have worsened over the last 20 years in the United States.1 A growing body of evidence has exposed the role of health care systems in contributing to these disparities. Based …
Coronavirus Disease 2019 Planning and Response
BACKGROUND: As coronavirus disease 2019 (COVID-19) rapidly progressed throughout the United States, increased demand for health workers required health workforce data and tools to aid planning and response at local, state, and national levels. OBJECTIVE: We describe the development of 2 estimator tools designed to inform health workforce planning for COVID-19. RESEARCH DESIGN: We estimated supply and demand for intensivists, critical care nurses,…
The Effect of National Health Service Corps Clinician Staffing on Medical and Behavioral Health Care Costs in Community Health Centers
OBJECTIVE: Prior studies of community health centers (CHCs) have found that clinicians supported by the National Health Service Corps (NHSC) provide a comparable number of primary care visits per full-time clinician as non-NHSC clinicians and provide more behavioral health care visits per clinician than non-NHSC clinicians. This present study extends prior research by examining the contribution of NHSC and non-NHSC clinicians to medical and behav…
Alternative Approaches to Ensuring Adequate Nurse Staffing
OBJECTIVE: The objective of this study was to addresses the basic question of whether alternative legislative approaches are effective in encouraging hospitals to increase nurse staffing. METHODS: Using 16 years of nationally representative hospital-level data from the American Hospital Association (AHA) annual survey, we employed a difference-in-difference design to compare changes in productive hours per patient day for registered nurses (RNs),…
Parenting With a Substance Abuse History
Although numerous studies have shown that child obesity is associated with internalizing symptoms, relatively few studies have examined the role of parenting behaviors on this relationship. Youth meeting obesity status may be at higher risk of psychosocial maladjustment when exposed to more vulnerable parenting contexts. The current study interviewed mothers with a history of substance abuse to assess whether parenting behaviors moderated the rel…
The Role of the National Health Service Corps Clinicians in Enhancing Staffing and Patient Care Capacity in Community Health Centers
OBJECTIVE: The National Health Service Corps (NHSC) is a federal program to increase the supply of health professionals in underserved communities, but its role in enhancing the capacity of community health centers (CHCs) has not been investigated. This study examined the role of NHSC clinicians in improving staffing and patient care capacity in primary, dental, and mental health care in CHCs. METHODS: Using 2013-2016 administrative data from CHC…
Does the Patient-centered Medical Home Model Change Staffing and Utilization in the Community Health Centers
Our findings suggest that the implementation of PCMH actively reengineers staff composition and this, in turn, results in changes in marginal utilization by each staff type
Obesity and Cigarette Smoking
Findings suggest that electronic tobacco use may fit well into previously established relationships between higher weight status and tobacco use. Future research should examine the longitudinal processes and pathways underlying the relationship between weight status and electronic tobacco use
Alternative Approaches to the Governance of Transnational Labor Recruitment
As globalization advances, the governance challenges relating to cross-border labor recruitment have also grown. Transnational companies that manage the employment-based migration process often take advantage of individuals seeking work abroad. While some states have implemented recruitment regulations, a combination of jurisdictional constraints and economic interests have limited states’ capacity and political will to take action. Supplemental …
Cross-sector learning among researchers and policy-makers
This paper assesses the preliminary results of a research funding strategy that alters the structure and process of research by requiring interaction between researchers and policy-makers. The five research teams focused on different aspects of expanding social protection in health in Latin America and the Caribbean. Preliminary results revealed negotiation of the research questions at the start of the process, influencing not only the project de…
An assessment of the impact of Health Workers for Change in Avellaneda, Province of Buenos Aires, Argentina
This paper describes the evaluation of the Health Workers for Change (HWFC) workshop series in a primary health care clinic in Avellaneda, Argentina. The study found that there was an important impact at the facility level 2 months after the intervention (T2). Health workers were motivated and willing to examine their own practices critically in an effort to improve quality of care. Informants from the community also perceived that patients were …
An evaluation of Health Workers for Change in seven settings
This paper presents the findings of a multi-centre study assessing the impact of Health Workers for Change (HWFC) workshops in seven different primary care sites, based on the common core protocol described in this paper. The paper discusses a common methodology used by the studies, consisting of a triangulation of qualitative and quantitative methods. Such methodologies are inherently complex as they require comparisons across systems, sites and…
