Intermarriage on Subjective Social Status and Spousal Dissimilarity in Life Satisfaction of Co-resident Heterosexual South African Couples
The aim of this study is to determine how intermarriage on subjective social status is associated with spousal dissimilarity in life satisfaction in co-resident heterosexual couples in South Africa. We apply fixed effect ordered logit regression models to a sample of 8652 married and cohabiting dyads constructed from the longitudinal and nationally representative South African National Income Dynamics Study. We find no spousal dissimilarity in ov…
Conceptual causal models of socioeconomic status, family structure, family functioning and their role in public health
Social determinants of health frameworks are standard tools in public health. These frameworks for the most part omit a crucial factor: the family. Socioeconomic status moreover is a prominent social determinant of health. Insofar as family functioning is poorer in poor families and family structure and functioning are linked to health, it is critical to consider the pathways between these four constructs. In this correspondence, we reflect on ho…
The Impact of a Household Food Garden Intervention on Food Security in Lesotho
Food insecurity is a challenge in the developing world, where many are finding healthy food inaccessible due to poverty. A pre-test, post-test design was applied to determine the impact of a vegetable gardening intervention in 25 experimental and 25 control households in Lesotho. Information about sociodemographic conditions and indicators of food security was collected by trained fieldworkers. As evidenced by the Living Poverty Index of 2.5, the…
Socio-economic inequalities in the multiple dimensions of access to healthcare
In the South African health system, the socio-economically disadvantaged are discriminated against across the continuum of access. NHI offers a means to enhance ability to pay and to address affordability, while disparities between actual and perceived need warrants investment in health literacy outreach programmes
Subjective social status and inequalities in depressive symptoms
Policy makers should target a reduction in the positive contribution of SSS to depression via the implementation of programmes that improve social welfare. Given the much greater contribution to inequalities among females, these policies should target women. Policies that protect children and especially the girl child from conflict can also be useful in reducing inequalities in depression related to subjective social status during adulthood. Over…
Incidence, socio-economic inequalities and determinants of catastrophic health expenditure and impoverishment for diabetes care in South Africa
Financial protection of diabetic patients in public hospitals is limited. This observation suggests that health financing interventions amongst diabetic patients should target the poor and poor women in particular. There is also a need for targeted interventions to improve access to healthcare facilities for diabetic patients and to reduce the financial impact of transport costs when seeking healthcare. This is particularly important for the achi…
Health status, food insecurity, and time allocation patterns of patients with Aids receiving antiretroviral treatment in South Africa
For patients with AIDS receiving antiretroviral treatment (ART) in South Africa via public clinics, improvements in nutritional status and economic productivity are likely to depend on adherence to drug regimen and quality of diet reflected in protein and micronutrient intakes. This study randomized 643 patients receiving ART from public clinics in the Free State Province into a Control group, a treatment group receiving adherence support, and a …
Decomposing socioeconomic inequalities in alcohol use by men living in South African urban informal settlements
Inequality in alcohol use exists among both younger and older males and discriminate against the poor. Public policies aimed at promoting public health and the prevention of unhealthy behaviours should target younger and middle-aged men from socioeconomically disadvantaged groups. We also suggest policies that target single males in informal settlements
Family Functioning and Socioeconomic Status in South African Families
Mutual HIV status disclosure is associated with consistent condom use in public sector ART clients in Free State province, South Africa
Risky sexual behaviour in PLWHA on antiretroviral therapy threatens both prevention and treatment efforts, but disclosure promises to support safer sexual practices. This paper investigates the association between HIV self-disclosure and consistent condom use in a cohort of public sector patients on antiretroviral (ARV) treatment. Using data from the FEATS cohort study, logistic regression analysis shows that knowledge of your partner's HIV statu…
The Importance of the Family
Public sector antiretroviral treatment programme in South Africa
Although there is a high prevalence of anxiety and depression amongst people receiving antiretroviral treatment (ART), many patients are not screened, diagnosed or referred for mental health problems. This study aims to determine whether public sector health care workers in South Africa observe, screen, diagnose and refer ART patients that show symptoms of common mental disorders. It also aims to ascertain the extent of mental health training rec…

Anxiety and depression amongst patients enrolled in a public sector antiretroviral treatment programme in South Africa
The findings from the study provide valuable insights into the psychosocial aspects of the Free State public-sector ART programme. Combined with the literature on the intricate link between mental health problems and treatment outcomes our results emphasise firstly, the necessity that resources be allocated for both screening and treating mental health problems and, secondly, the need for interventions that will encourage support-group participat…
Who is accessing public-sector anti-retroviral treatment in the Free State, South Africa? An exploratory study of the first three years of programme implementation
Our analysis showed significant changes in the demographic, socio-economic, geographic, and medical characteristics of the patients during the first three years of the programme. Knowledge of the characteristics of these patients can assist policy makers in developing measures to retain them in care. The information reported here can also be usefully applied to target patient groups that are currently not reached in the implementation of the ART …
Using an Asset Index to Assess Trends in Poverty in Seven Sub-Saharan African Countries
Highly active antiretroviral treatment and health related quality of life in South African adults with human immunodeficiency virus infection
HAART was associated with improved HRQoL in patients enrolled in a public sector treatment program in South Africa. Our finding that the EuroQol instrument was sensitive to HAART supports its use in future evaluation of HIV/AIDS care in South Africa. Longitudinal studies are needed to evaluate changes in individuals' HRQoL

Out-Migration in the Context of the HIV/Aids Epidemic
In this article I investigate the characteristics and determinants of out-migration in the context of the HIV/AIDS epidemic, using data from a panel designed to investigate the household impact of the epidemic. Departure models show that individual attributes, notably age and gender, play an important role in explaining out-migration from households that have not experienced morbidity or mortality. In affected households, a number of household-le…
Ties that Bind
The last decade has witnessed a renewed interest in the concept of social capital. However, much confusion still exists about the exact meaning of social capital. Thus, to create a foundation for future studies on social capital, this article develops a conceptual and theoretical framework to depict the stocks and flows of social capital. As such, it defines social capital formation in transactional terms so as to exploit fully the capital analog…
Relationships between HIV/Aids, income and expenditure over time in deprived South African households
We compared 202 households with an HIV-infected member, and 202 unaffected neighbouring households, in one rural and one urban area in Free State province, South Africa. Interviews were conducted with a key informant in each household, at baseline and 6, 12 and 18 months later, with 87% follow-up. Results of the baseline and 6 months study were published previously; this paper reports on trends over the full 18 months of follow-up. We analysed ch…
HIV/Aids, poverty and risky sexual behaviour in South Africa
This paper employs data from the 1998 South African Demographic and Health Survey in exploring the nature of socio-economic inequalities in and determinants of risky sexual behaviour. Risky sexual behaviour was associated with poverty only in the case of multiple partnerships. Affluent women that have engaged in risky sexual behaviour were shown to be more likely to have cited negative perceptions about condom use as main reason for not using a c…
Social grants as safety net for HIV/Aids-affected households in South Africa
South Africa has a well-developed system of social security and the reach of the social grant safety net has expanded rapidly over the past five years. Social grants are likely to play an important role in mitigating the impact of HIV/AIDS, given that eligibility for these grants is driven largely by the increasing burden of chronic illness, the mounting orphan crisis and the impoverishment of households associated with the epidemic. This paper i…

Children of the storm
This paper explores the impact of HIV/AIDS on children, using data from a longitudinal household impact study and focus groups conducted in two communities in the Free State province of South Africa. Non‐attendance at school among children in general and particularly older children, although relatively low, is disconcerting, being higher in affected than in non‐affected households. A large and growing number of children have lost their mother or …
